Monthly Premium

Deductibles

Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $10100.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $10100.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

Prescription drugs are not covered by Humana USAA Honor Giveback (Regional PPO).
The Humana USAA Honor Giveback (Regional PPO) plan offers comprehensive coverage with varying cost-sharing. It includes benefits like inpatient hospital stays with copays, outpatient services with copays or coinsurance, and coverage for emergency services. The plan also features coverage for primary care, preventive, hearing, vision, and dental services. Additionally, it covers services such as ambulance, home health, skilled nursing, and medical equipment with copays or coinsurance.
Inpatient Hospital benefits are covered, including Inpatient Hospital-Acute and Inpatient Hospital Psychiatric. For Inpatient Hospital-Acute, you will pay a $425 copay for days 1-5, and no copay for days 6-90, while for Inpatient Hospital Psychiatric, you will pay a $370 copay for days 1-5, and no copay for days 6-90.
Outpatient Services include coverage for all outpatient hospital services, observation services, ambulatory surgical center services, outpatient substance abuse services, and outpatient blood services. Outpatient hospital services have a copay ranging from $0 to $325, while observation services have a $425 copay. Ambulatory Surgical Center Services and Outpatient Blood Services have no copay. Outpatient substance abuse services including individual and group sessions each have a 25% coinsurance and a copay between $40 and $40.
Partial Hospitalization is covered under the Humana USAA Honor Giveback (Regional PPO) plan, with a $35 copay. Prior authorization is required for this benefit.
Ambulance and Transportation Services are covered by the Humana USAA Honor Giveback (Regional PPO) plan. Ground ambulance services have a $315 copay, while air ambulance services have a 20% coinsurance. Transportation services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered by the Humana USAA Honor Giveback (Regional PPO) plan. Emergency Services has a $125 copay, and Urgently Needed Services has a $55 copay, but both have no coinsurance, while Worldwide Emergency Services, Worldwide Urgent Coverage, and Worldwide Emergency Transportation all have a $125 copay and no coinsurance.
The Humana USAA Honor Giveback (Regional PPO) plan covers primary care physician services with a $10 copay, chiropractic services with a $20 copay, occupational therapy services with a $35 copay, physician specialist services with a $40 copay, mental health specialty services with a $40 copay for individual and group sessions, physical therapy and speech-language pathology services with a $35 copay, additional telehealth benefits with a copay between $0 and $55, and opioid treatment program services with a 25% coinsurance and a $40 copay. Routine chiropractic care and podiatry services are not covered.
The Humana USAA Honor Giveback (Regional PPO) plan covers a range of preventive services, including an annual physical exam with no copay. Other services, such as Health Education, In-Home Safety Assessment, and others are not covered.
Hearing services are covered, including hearing exams with a $40 copay, routine hearing exams with no copay, and fitting/evaluation for hearing aids with no copay. Prescription hearing aids are partially covered with a copay between $699 and $999, but inner ear, outer ear, and over-the-ear hearing aids are not covered; OTC hearing aids are covered with a maximum benefit of $50 every three months.
Vision Services includes coverage for eye exams with a copay between $0 and $40, and eyewear with no copay. Contact lenses and eyeglasses (lenses and frames) are covered with no copay, while eyeglass lenses, eyeglass frames, and upgrades are not covered.
Dental services include coverage for Medicare dental services with a $40 copay, oral exams, dental x-rays, other diagnostic dental services, prophylaxis (cleaning), other preventive dental services, restorative services with 30-40% coinsurance, adjunctive general services, endodontics, periodontics, prosthodontics (removable) with 30% coinsurance, prosthodontics (fixed) with 30-40% coinsurance, and oral and maxillofacial surgery with no copay. Fluoride treatment, maxillofacial prosthetics, implants, and orthodontics are not covered. The plan has a maximum benefit coverage of $3,000 per year for both in-network and out-of-network services.
Home Infusion bundled Services are covered, but require prior authorization. For Medicare Part B Insulin Drugs, there is a $35 copay and a coinsurance between 0% and 20%. For Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs, there is a coinsurance between 0% and 20%.
Dialysis Services are covered with a coinsurance of 20%. Prior authorization is required.
Medical equipment is covered, including durable medical equipment (DME), prosthetics/medical supplies, and diabetic equipment. Durable Medical Equipment has a 20% coinsurance, and Prosthetic Devices and Medical Supplies also have a 20% coinsurance; however, Durable Medical Equipment for use outside the home is not covered. Diabetic Supplies have a 10-20% coinsurance with no copay, and Diabetic Therapeutic Shoes/Inserts have no copay.
Diagnostic and Radiological Services are covered by the Humana USAA Honor Giveback (Regional PPO) plan. Diagnostic Procedures/Tests have a maximum copay of $55 and a coinsurance of at least 25%, Lab Services have no copay, Diagnostic Radiological Services have a maximum copay of $350, Therapeutic Radiological Services have a maximum copay of $40 and a coinsurance of at least 20%, and Outpatient X-Ray Services have a $10 copay.
Home Health Services are covered by the Humana USAA Honor Giveback (Regional PPO) plan with no copay and no coinsurance. However, additional hours of care and personal care services are not covered.
Cardiac Rehabilitation Services are covered with prior authorization required. However, Intensive Cardiac Rehabilitation Services, Pulmonary Rehabilitation Services, and SET for PAD Services are not covered.
Skilled Nursing Facility (SNF) benefits are covered by the Humana USAA Honor Giveback (Regional PPO) plan, but require prior authorization. For days 1-20, the copay is $10, and for days 21-100, the copay is $203. Additional days beyond Medicare-covered and non-Medicare-covered stays for SNF are not covered.
The Humana USAA Honor Giveback (Regional PPO) plan covers acupuncture with a $40 copay, OTC items with a $50 maximum benefit every three months, and a meal benefit with no copay. However, the plan does not cover Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, or several other services.
