Once your total out-of-pocket costs reach $14000.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

The AARP Medicare Advantage from UHC FL-0031 (Regional PPO) plan has an "Enhanced Alternative" drug benefit. The plan has a deductible of $570.00. In the initial coverage phase, after you pay the deductible, you will pay a copay of $8.00 for standard generic drugs, $47.00 for standard generic drugs, and $100.00 for preferred brand drugs. Non-preferred drugs have a 26% coinsurance. Once your total drug costs reach $2000.00, you enter the next coverage phase.
The AARP Medicare Advantage from UHC FL-0031 (Regional PPO) plan offers a range of benefits with varying costs. Inpatient hospital stays have a copay of $395 for the first four days, then no copay for the remainder of the stay. Outpatient services have copays between $0 and $395, and emergency services have a $90 copay. Outpatient Services include coverage for outpatient hospital services with a copay between $0 and $395, observation services with a $395 copay, and ambulatory surgical center services with no copay. Outpatient substance abuse services are covered with a copay between $0 and $25 for individual sessions and a $15 copay for group sessions, and outpatient blood services are covered with no copay.
Partial Hospitalization is covered, but requires prior authorization. You will pay a $55 copay for this benefit.
Ambulance and Transportation Services are covered by the AARP Medicare Advantage from UHC FL-0031 (Regional PPO) plan. Ground and Air Ambulance Services have a $105 copay, with no coinsurance, while other Transportation Services are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered by this plan. Emergency Services have a $90 copay, while Urgently Needed Services have a copay between $0 and $30; both have no coinsurance. Worldwide Emergency Coverage, Worldwide Urgent Coverage, and Worldwide Emergency Transportation all have no copay and no coinsurance.
The AARP Medicare Advantage from UHC FL-0031 (Regional PPO) plan covers primary care physician services with no copay, chiropractic services with a $15 copay, occupational therapy services with a $0-$20 copay, physician specialist services with a $0-$45 copay, mental health specialty services with a $0-$25 copay for individual sessions and a $15 copay for group sessions, podiatry services with a $35 copay, other health care professional services with a $0-$45 copay, psychiatric services with a $0-$25 copay for individual sessions and a $15 copay for group sessions, physical therapy and speech-language pathology services with a $0-$20 copay, additional telehealth benefits with no copay, and opioid treatment program services with no copay. Routine chiropractic care is not covered.
The AARP Medicare Advantage from UHC FL-0031 (Regional PPO) plan covers preventive services, including an annual physical exam with no copay. Additional preventive services, including Fitness Benefit, Glaucoma Screening, Diabetes Self-Management Training, Barium Enemas, Digital Rectal Exams, and EKG following Welcome Visit, are covered with no copay. However, other services like Health Education, In-Home Safety Assessment, and others are not covered.
Hearing services include routine hearing exams with no copay, but are limited to one exam per year. Prescription hearing aids are partially covered, with copays between $199 and $1249 for all types of hearing aids, but not for inner ear, outer ear, or over-the-ear hearing aids. OTC hearing aids are covered with a copay between $99 and $829 for up to two hearing aids per year.
Vision services include routine eye exams with no copay, and are covered once per year. Eyewear benefits are covered, but contact lenses, eyeglasses (lenses and frames), eyeglass lenses, eyeglass frames, and upgrades are not covered.
Dental services are partially covered, with 20% coinsurance for Medicare Dental Services; however, orthodontic services, restorative services, adjunctive general services, endodontics, periodontics, prosthodontics (removable and fixed), maxillofacial prosthetics, implant services, and oral and maxillofacial surgery are not covered.
Home Infusion bundled Services are covered, with prior authorization required. For Medicare Part B Insulin Drugs, there is a $35 copay, and coinsurance between 0% and 20%. Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs have coinsurance between 0% and 20%.
Dialysis Services are covered under the AARP Medicare Advantage from UHC FL-0031 (Regional PPO) plan. This benefit requires prior authorization and has a coinsurance of 20%.
Medical Equipment benefits include Durable Medical Equipment (DME) with 20% coinsurance, Prosthetics/Medical Supplies with 20% coinsurance, and Diabetic Equipment, which includes Diabetic Supplies with no copay and Diabetic Therapeutic Shoes/Inserts with 20% coinsurance. Durable Medical Equipment for use outside the home is not covered.
Diagnostic and Radiological Services, including diagnostic procedures/tests, lab services, diagnostic radiological services, therapeutic radiological services, and outpatient X-ray services, are covered. Diagnostic Procedures/Tests have a copay of $25, while Lab Services have no copay. Diagnostic Radiological Services have a maximum copay of $140, Therapeutic Radiological Services have a minimum copay of $80, and Outpatient X-Ray Services have a copay of $15.
Home Health Services are covered by the AARP Medicare Advantage from UHC FL-0031 (Regional PPO) plan with no copay and no coinsurance; however, Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are covered by AARP Medicare Advantage from UHC FL-0031 (Regional PPO), but the plan does not cover any of the sub-services. Prior authorization is required.
Skilled Nursing Facility (SNF) services are covered by the AARP Medicare Advantage from UHC FL-0031 (Regional PPO) plan. There is no copay for days 1-20, and a $203 copay per day for days 21-100. Additional days beyond Medicare-covered for SNF and non-Medicare-covered stays for SNF are not covered.
Other Services includes a meal benefit with no copay, while acupuncture, over-the-counter items, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered.
