Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $14000.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $14000.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

Prescription drugs are not covered by Humana USAA Honor Giveback (Regional PPO).
The Humana USAA Honor Giveback (Regional PPO) plan offers a range of benefits, including coverage for inpatient and outpatient hospital services, with varying copays. The plan also includes coverage for emergency services, primary care, and preventive services, often with no copay. You can also expect coverage for hearing, vision, and dental services with copays and coinsurance, as well as home health services and skilled nursing facilities with specific copay structures.
Inpatient Hospital services are covered, with a $425 copay for days 1-5 and no copay for days 6-90 for Inpatient Hospital-Acute and Inpatient Hospital Psychiatric. Additional days for Inpatient Hospital-Acute have no copay for days 91-999. Non-Medicare-covered stays and upgrades for Inpatient Hospital-Acute and Additional days and Non-Medicare-covered stays for Inpatient Hospital Psychiatric are not covered.
Outpatient Services are covered, including all outpatient hospital services, observation services, ambulatory surgical center services, outpatient substance abuse services, and outpatient blood services. Outpatient Hospital Services have a copay between $0 and $450, Observation Services have a $425 copay, Ambulatory Surgical Center Services have no copay, and Outpatient Blood Services have no copay. Outpatient Substance Abuse Individual and Group Sessions each have a copay between $45 and $100.
Partial Hospitalization is covered with a $80 copay. Prior authorization is required.
Ambulance and Transportation Services are covered by the Humana USAA Honor Giveback (Regional PPO) plan. Both ground and air ambulance services have a $315 copay, with no coinsurance, while transportation services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered under the Humana USAA Honor Giveback (Regional PPO) plan. Emergency Services and Worldwide Emergency Coverage have a $110 copay, while Urgently Needed Services have a $45 copay; all of these services have no coinsurance.
The Humana USAA Honor Giveback (Regional PPO) plan covers primary care physician services with no copay, chiropractic services with a $15 copay, and occupational therapy services with a $25 copay. The plan also covers physician specialist services with a $50 copay, mental health specialty services with a $45 copay, physical therapy and speech-language pathology services with a $25 copay, and psychiatric services with a $45 copay. Additionally, additional telehealth benefits are covered with a copay between $0 and $50, and Opioid Treatment Program Services are covered with a minimum copay of $45 and a maximum copay of $100. Routine chiropractic care and podiatry services are not covered.
Preventive services include coverage for Medicare-covered preventive services and annual physical exams with no copay. Additional preventive services are also covered, and other services such as Health Education, In-Home Safety Assessment, and Personal Emergency Response System are not covered.
Hearing exams are covered with a $50 copay, while routine hearing exams and fitting/evaluation for hearing aids have no copay. Prescription hearing aids are covered with a copay between $699 and $999 depending on the type of hearing aid, but prescription hearing aids - inner ear, outer ear, and over the ear are not covered, and OTC hearing aids are not covered.
Vision services include eye exams and eyewear. Eye exams have a copay of $0-$50, while routine eye exams have no copay. Eyewear has no copay, but eyeglass lenses, eyeglass frames, and upgrades are not covered.
Dental services include coverage for Medicare dental services with a $50 copay, oral exams, dental x-rays, other diagnostic dental services, prophylaxis (cleaning), other preventive dental services, restorative services with 30-40% coinsurance, adjunctive general services, endodontics, periodontics, prosthodontics (removable) with 30% coinsurance, prosthodontics (fixed) with 30-40% coinsurance, and oral and maxillofacial surgery with no copay. Fluoride treatment, maxillofacial prosthetics, implant services, and orthodontics are not covered.
Home Infusion bundled Services are covered, but require prior authorization. Medicare Part B Insulin Drugs have a $35 copay and between 0-20% coinsurance. Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs have between 0-20% coinsurance.
Dialysis Services are covered by the Humana USAA Honor Giveback (Regional PPO) plan. There is a 20% coinsurance for dialysis services.
Medical Equipment is covered by Humana USAA Honor Giveback (Regional PPO), with Durable Medical Equipment (DME) subject to 15% coinsurance and Prosthetic Devices, and Medical Supplies also covered, with 15% coinsurance on each. Diabetic Supplies have a 10-20% coinsurance and no copay, while Diabetic Therapeutic Shoes/Inserts have a $10 copay.
Diagnostic and Radiological Services, including all diagnostic services, are covered under the Humana USAA Honor Giveback (Regional PPO) plan, with a copay for some services ranging from $0 to $120. Lab services have no copay, while diagnostic radiological services have a maximum copay of $325.00, and therapeutic radiological services have a maximum copay of $50 and a minimum coinsurance of 20%. Outpatient X-Ray Services have no copay.
Home Health Services are covered by the Humana USAA Honor Giveback (Regional PPO) plan with no copay and no coinsurance. However, additional hours of care and personal care services are not covered.
Cardiac Rehabilitation Services are covered, but the specific services of Medicare-covered Intensive Cardiac Rehabilitation Services, Medicare-covered Pulmonary Rehabilitation Services, Medicare-covered Supervised Exercise Therapy (SET) for Symptomatic Peripheral Artery Disease (PAD) Services, and Additional Cardiac Rehabilitation Services are not covered. Prior authorization is required.
Skilled Nursing Facility (SNF) services are covered by the Humana USAA Honor Giveback (Regional PPO) plan, but require prior authorization. For days 1-20, there is no copay, and for days 21-100, the copay is $214.
The Humana USAA Honor Giveback (Regional PPO) plan covers acupuncture with a $50 copay and meal benefits with no copay; however, over-the-counter items, dual eligible SNPs, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered.
