For Tier 1 preferred generic drugs, members pay no copay at standard pharmacies and standard mail order for up to a 3-month supply. Tier 2 generic drugs are also available with no copay through standard mail order, while standard pharmacy purchases require a copay of $5 for a 1-month, $10 for a 2-month, or $15 for a 3-month supply. Brand-name and specialty medications under this plan are subject to coinsurance rather than flat copays. Tier 3 preferred brand drugs require 25% coinsurance, and Tier 4 non-preferred drugs carry a 35% coinsurance, applicable to both standard pharmacies and standard mail order. Tier 5 specialty drugs require a 29% coinsurance for a 1-month supply at standard pharmacies and standard mail order.
