This plan has no drug deductible. Your prescription medication coverage will start immediately.
Out-of-Pocket Maximums
This plan has a Maximum Out-Of-Pocket cost of $5800.00 for out-of-network services. You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $0.00 for in-network covered services, the plan will pay 100% of in-network covered costs for the rest of the year.

The Cigna Preferred Medicare (HMO) plan has a $0 deductible for prescription drugs. During the initial coverage phase, you will pay a copay for your prescriptions depending on the drug tier and pharmacy you use. For example, preferred generic drugs have a $10 copay at preferred pharmacies. Once your total drug costs reach $2,000, you enter the catastrophic coverage phase where you will pay nothing for your Part D covered drugs. However, you may still pay a share of the costs for excluded drugs covered under any enhanced benefit.
The Cigna Preferred Medicare (HMO) plan offers comprehensive coverage, including inpatient and outpatient hospital services, with varying copays. You'll have no copay for many services, such as routine hearing exams, and outpatient x-rays. The plan also covers dental, vision, and hearing services, with set copays or coinsurance amounts for specific services like hearing aids and eyewear. This plan includes benefits like ambulance services, with copays or coinsurance depending on the type of transport, and covers home health services with no copay. Additionally, the plan provides coverage for home infusion, dialysis, and medical equipment, with associated copays or coinsurance. However, certain services like cardiac rehabilitation, additional hours of care, and specific types of care are not covered.
Inpatient Hospital coverage includes acute and psychiatric services with a copay of $320 for days 1-6, and no copay for days 7-90. Additional days for inpatient hospital-acute and psychiatric, and non-Medicare-covered stays for both are not covered.
Outpatient services, including outpatient hospital services, observation services, ambulatory surgical center (ASC) services, outpatient substance abuse services, and outpatient blood services, are covered. Outpatient hospital services have a copay between $0 and $350, while observation services have a $350 copay. Ambulatory Surgical Center (ASC) Services have no copay, and individual and group outpatient substance abuse sessions have a $15 copay.
Partial Hospitalization is covered by the Cigna Preferred Medicare (HMO) plan. This benefit requires prior authorization and has a copay of $105.
Ambulance and Transportation Services are covered by Cigna Preferred Medicare (HMO), with prior authorization required. Ground ambulance services have a $270 copay, while air ambulance services have a 20% coinsurance. Transportation Services to a plan-approved health-related location are covered for up to 50 one-way trips per year, but transportation services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered by the Cigna Preferred Medicare (HMO) plan. Emergency Services and Worldwide Emergency Coverage have a $125 copay, and Urgently Needed Services have a $30 copay, and all have no coinsurance.
The Cigna Preferred Medicare (HMO) plan covers primary care physician services, chiropractic services with a $20 copay, occupational therapy services with a $25 copay, physician specialist services with a $15 copay, and physical therapy and speech-language pathology services with a $25 copay. The plan also covers additional telehealth benefits with a copay between $0 and $15, and opioid treatment program services with a $15 copay. However, routine chiropractic care, individual and group sessions for mental health specialty services, individual and group sessions for psychiatric services, and podiatry services are not covered.
Preventive services are covered, including Medicare-covered services, annual physical exams, and additional preventive services. The plan does not cover in-home safety assessments, personal emergency response systems, medical nutrition therapy, post-discharge in-home medication reconciliation, re-admission prevention, wigs for hair loss related to chemotherapy, weight management programs, alternative therapies, therapeutic massage, adult day health services, nutritional/dietary benefits, home-based palliative care, in-home support services, support for caregivers of enrollees, additional sessions of smoking and tobacco cessation counseling, enhanced disease management, telemonitoring services, remote access technologies, home and bathroom safety devices and modifications, or counseling services.
Hearing services include routine hearing exams with a $15 copay, and fitting/evaluation for hearing aids with no copay. Prescription hearing aids (all types) are covered with a copay between $399 and $1800, while prescription hearing aids for the inner ear, outer ear, and over the ear are not covered. OTC hearing aids are not covered.
Vision services include eye exams with a copay of $0-$10, and eyewear, including contact lenses, eyeglasses, eyeglass lenses, and eyeglass frames. Eyewear has a combined maximum plan benefit coverage of $200 per year.
The Cigna Preferred Medicare (HMO) plan covers dental services, including oral exams with a $15 copay, dental x-rays, other diagnostic dental services, prophylaxis (cleaning), fluoride treatment, other preventive dental services, restorative services, adjunctive general services, endodontics, periodontics, prosthodontics, removable, maxillofacial prosthetics, implant services, prosthodontics, fixed, oral and maxillofacial surgery, and orthodontics. This plan has a maximum benefit of $2,100 per year for other dental services.
Home Infusion bundled Services are covered by the Cigna Preferred Medicare (HMO) plan, and require prior authorization. Insulin has a $35 copay and a coinsurance between 0% and 20%, while Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs have a coinsurance between 0% and 20%.
Dialysis Services are covered by the Cigna Preferred Medicare (HMO) plan, but require prior authorization. You will pay 20% coinsurance.
Medical equipment is covered, with a 20% coinsurance for durable medical equipment, prosthetic devices, and medical supplies. Diabetic therapeutic shoes/inserts are covered with a 20% coinsurance, while diabetic supplies and durable medical equipment for use outside the home are not covered.
Diagnostic and Radiological Services are covered by the Cigna Preferred Medicare (HMO) plan. Diagnostic Procedures/Tests have a copay between $0 and $200, while Lab Services have no copay. Diagnostic Radiological Services have a copay up to $200, Therapeutic Radiological Services have a $60 copay, and Outpatient X-Ray Services have no copay.
Home Health Services are covered by the Cigna Preferred Medicare (HMO) plan with no copay or coinsurance, but authorization is required. Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are not covered by the Cigna Preferred Medicare (HMO) plan. Prior authorization is required for this benefit.
Skilled Nursing Facility (SNF) services are covered by the Cigna Preferred Medicare (HMO) plan with prior authorization required. You will have no copay for days 1-20, and a $214 copay per day for days 21-100. Additional days beyond Medicare-covered, and non-Medicare-covered stays are not covered.
The Cigna Preferred Medicare (HMO) plan does not cover acupuncture, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, or Self-Directed Personal Assistance Services. Over-the-counter items are covered with a maximum benefit of $70 every three months, and the plan offers a meal benefit for chronic illnesses and medical conditions that require the enrollee to remain at home.
