Out-of-Pocket Maximums

This plan has a combined Maximum Out-Of-Pocket cost of $6250.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $6250.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

Specialist Visits:

Visits to specialists are covered and will have a copay of $45.00 and coinsurance of 0% (no coinsurance). Specialist visits may require a referral from your primary care doctor or prior authorization.

Emergency Room:

Trips to the Emergency Room are covered, and will have a copay of $125.00 and coinsurance of 0% (no coinsurance). Coverage may vary for in-network and out-of-network hospitals.

Urgent Care:

Trips to Urgent Care arecovered and will have a copay of $0.00 - $50.00 and coinsurance of 0% (no coinsurance). Coverage may vary for in-network and out-of-network hospitals.

In the initial coverage phase, you'll pay a copay for generic drugs, and coinsurance for brand-name and non-preferred drugs. Once your total drug costs reach $2,000, you enter the catastrophic coverage phase, where you will pay nothing for your Part D covered drugs. This plan may also reduce your premium if you qualify for the low-income subsidy.

The Medicare Plus Blue PPO Essential (PPO) plan covers dental services with a maximum benefit of $1,500 per year for both in-network and out-of-network services, including oral exams with no copay for two visits per year, dental x-rays, prophylaxis (cleaning) with no copay for two visits per year, fluoride treatment with no copay for one visit per year, and restorative services with no copay, as well as oral and maxillofacial surgery with no copay for two visits per lifetime. Maxillofacial prosthetics and orthodontics are not covered.

Home Infusion bundled Services are covered, including Medicare Part B Insulin Drugs with a $35 copay and 0-20% coinsurance, and Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs with 0-20% coinsurance. Prior authorization is required for these services.

The Medicare Plus Blue PPO Essential (PPO) plan does not cover acupuncture, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, or Self-Directed Personal Assistance Services. Over-the-counter (OTC) items are covered, with a maximum benefit of $95 every three months, and the plan offers Nicotine Replacement Therapy (NRT) and Naloxone coverage. The plan covers a meal benefit for chronic illnesses, and other services include a copay of $90 for ambulance non-transport services.
