Monthly Premium
The Monthly Premium for this plan is $56.10. This is the amount you must pay every month. Additionally, this plan comes with a Part B Premium reduction of $3.00. You must continue to pay paying your reduced Part B Premium.
Deductibles
This plan does not have a health deductible. Your insurance coverage on covered health services will start immediately.
This plan has a $590.00 drug deductible. You will need to pay this amount towards covered prescriptions before your insurance coverage for prescription medications kicks in.
Out-of-Pocket Maximums
This plan has a Maximum Out-Of-Pocket cost of $8500.00 for out-of-network services. You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $0.00 for in-network covered services, the plan will pay 100% of in-network covered costs for the rest of the year.

The Nascentia Dual Advantage (HMO D-SNP) plan has a $590 deductible for prescription drugs. After the deductible, you will pay the costs for drugs in each tier until your total drug costs reach $2000. If you qualify for the low-income subsidy (LIS) or "Extra Help", your monthly Part D premium will be $56.10. After your yearly out-of-pocket drug costs reach $2000, you will pay nothing for Medicare Part D covered drugs.
The Nascentia Dual Advantage (HMO D-SNP) plan offers a range of benefits with varying cost-sharing. Many services, like primary care and outpatient services, have a 20% coinsurance. The plan also covers home health services with no copay or coinsurance, and includes benefits for hearing, vision, and dental services. This plan provides additional coverage for services such as ambulance, emergency services, and medical equipment, often with a 20% coinsurance. The plan also offers an Over-the-Counter (OTC) benefit with a monthly maximum. However, certain services like cardiac rehabilitation, and some vision and hearing services are not covered.
Inpatient Hospital benefits are covered, including Inpatient Hospital-Acute and Inpatient Hospital Psychiatric, but additional days for either are not covered, nor are non-Medicare-covered stays or upgrades. The plan utilizes the Medicare-defined cost share for tier 1, with coinsurance details available in the plan documents.
Outpatient Services include coverage for outpatient hospital services and observation services, each with a 20% coinsurance, as well as ambulatory surgical center (ASC) services and outpatient substance abuse services. Outpatient blood services are not covered.
Partial Hospitalization is covered under the Nascentia Dual Advantage (HMO D-SNP) plan. You will pay 20% coinsurance for this benefit.
Ambulance and Transportation Services are covered by the Nascentia Dual Advantage (HMO D-SNP) plan. Ground and Air Ambulance Services have a 20% coinsurance, and there is no copay. Transportation Services to any health-related location are not covered.
Emergency Services are covered under the Nascentia Dual Advantage (HMO D-SNP) plan with a 20% coinsurance, and no copay. Urgently Needed Services are also covered with a 20% coinsurance and no copay, while Worldwide Emergency Services, Worldwide Urgent Coverage, and Worldwide Emergency Transportation are not covered.
The Nascentia Dual Advantage (HMO D-SNP) plan covers primary care physician services, chiropractic services, occupational therapy services, physician specialist services, mental health specialty services, other health care professional services, psychiatric services, physical therapy and speech-language pathology services, additional telehealth benefits, and opioid treatment program services, with a 20% coinsurance for most services. Routine chiropractic care and podiatry services are not covered.
Preventive services include coverage for Medicare-covered services with no copay, as well as Fitness Benefit (Memory Fitness), Glaucoma Screening, Diabetes Self-Management Training, Barium Enemas, Digital Rectal Exams, and EKG following Welcome Visit. Kidney Disease Education Services have a 20% coinsurance. The plan does not cover annual physical exams, Health Education, In-Home Safety Assessment, Personal Emergency Response System (PERS), Medical Nutrition Therapy (MNT), Post discharge In-Home Medication Reconciliation, Re-admission Prevention, Wigs for Hair Loss Related to Chemotherapy, Weight Management Programs, Alternative Therapies, Therapeutic Massage, Adult Day Health Services, Nutritional/Dietary Benefit, Home-Based Palliative Care, In-Home Support Services, Support for Caregivers of Enrollees, Additional Sessions of Smoking and Tobacco Cessation Counseling, Enhanced Disease Management, Telemonitoring Services, Remote Access Technologies, Home and Bathroom Safety Devices and Modifications, or Counseling Services.
Hearing Services include hearing exams, with a coinsurance of at most 20%, and fitting/evaluation for hearing aids. Routine hearing exams, and OTC hearing aids are not covered, and prescription hearing aids for inner ear, outer ear, and over the ear are not covered.
Vision Services include coverage for eye exams with a 20% coinsurance, as well as eyewear, also with a 20% coinsurance and a combined maximum of $700 per year for contact lenses and eyeglasses. Eyeglass lenses and frames are not covered.
Dental services are covered, including Medicare Dental Services with 20% coinsurance. Other dental services include oral exams (2 visits per year), dental x-rays (1 per year), other diagnostic dental services, prophylaxis (cleaning) (2 per year), fluoride treatment, other preventive dental services, restorative services, adjunctive general services, endodontics, periodontics, prosthodontics (removable and fixed), maxillofacial prosthetics, implant services, oral and maxillofacial surgery, and orthodontics.
Home Infusion bundled Services are covered by the Nascentia Dual Advantage (HMO D-SNP) plan, including Medicare Part B Insulin Drugs with a $35 copay. Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs are also covered, with a coinsurance between 0% and 20%.
Dialysis Services are covered under the Nascentia Dual Advantage (HMO D-SNP) plan. You will pay 20% coinsurance for these services.
Medical Equipment, including Durable Medical Equipment (DME), Prosthetics, and Medical Supplies are covered. DME has a 20% coinsurance and requires authorization, while medical supplies and prosthetic devices have a 20% coinsurance with authorization required.
Diagnostic and Radiological Services are covered under the Nascentia Dual Advantage (HMO D-SNP) plan. There is no copay for these services, but you may have to pay up to 20% coinsurance for Diagnostic Procedures/Tests, Lab Services, Diagnostic Radiological Services, Therapeutic Radiological Services, and Outpatient X-Ray Services.
Home Health Services are covered by the Nascentia Dual Advantage (HMO D-SNP) plan with no copay and no coinsurance, but additional hours of care and personal care services are not covered. This benefit requires authorization.
Cardiac Rehabilitation Services are not covered by the Nascentia Dual Advantage (HMO D-SNP) plan. Intensive Cardiac Rehabilitation Services, Pulmonary Rehabilitation Services, and SET for PAD Services are also not covered.
Skilled Nursing Facility (SNF) services are covered, but require prior authorization. This plan does not cover additional days beyond Medicare-covered for SNF, and it does not cover non-Medicare-covered stays for SNF.
The Nascentia Dual Advantage (HMO D-SNP) plan's "Other Services" benefit covers Over-the-Counter (OTC) items with a maximum benefit of $225.00 every month, and meal benefits that require prior authorization. Acupuncture, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered.
