Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $9000.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $9000.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

The Peak Advantage Summit (PPO) plan offers prescription drug coverage with a $0 drug deductible, meaning your benefits start immediately. For Tier 1 preferred generic drugs, there is no copay when using a preferred pharmacy or standard mail order. Tier 2 generic drugs are also highly affordable, costing a low $4 copay for a one-month supply at preferred pharmacies and standard mail order. For brand-name and specialty medications, copays and coinsurance vary depending on the drug tier and pharmacy type. Tier 3 preferred brand drugs carry a $42 copay at preferred pharmacies, while Tier 4 non-preferred drugs cost $95 for a one-month supply. Specialty drugs in Tier 5 require a 33% coinsurance at both preferred and standard pharmacies.
The Peak Advantage Summit (PPO) offers comprehensive medical coverage with no copay and no coinsurance for primary care visits, telehealth, and routine preventive care. Specialist visits, physical therapy, routine eye exams, and routine hearing tests are highly affordable with a $20 copay and no coinsurance. Inpatient hospital stays feature no coinsurance and copays up to $800, while emergency room visits require a $90 copay that is waived if you are admitted. Beyond medical services, the plan provides robust dental benefits with no copay for preventive care and a 50% coinsurance for comprehensive care up to a $3,500 annual limit. Members also benefit from no-copay eyewear, affordable prescription hearing aids, and a quarterly $120 allowance for over-the-counter items. Additionally, the plan covers up to 36 one-way transportation trips per year to approved locations and home health services with no copay.

Prescription hearing aids are covered with a copay ranging from $399 to $899 and no coinsurance, though OTC hearing aids and inner-ear, outer-ear, and over-the-ear prescription models are not covered.

There is no copay for days 1 through 20, followed by a $218 daily copay for days 21 through 100, with no coverage for additional days beyond the Medicare-covered limit.
