Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $9000.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $9000.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

The Peak Advantage Summit (PPO) Medicare plan features a $0 drug deductible, meaning your prescription coverage begins immediately. For Tier 1 preferred generic drugs, you will pay no copay for a 1-month or 3-month supply at preferred pharmacies and through standard mail order, while standard pharmacies charge a $15 copay for a 1-month supply. Tier 2 generic medications cost a low $4 copay for a 1-month supply at preferred pharmacies and standard mail order, compared to a $20 copay at standard pharmacies. For Tier 3 preferred brand drugs, 1-month copays start at $42 at preferred pharmacies and standard mail order, while Tier 4 non-preferred drugs carry a $95 copay at these same locations. Using a standard pharmacy increases these 1-month copays to $47 for Tier 3 and $100 for Tier 4. Specialty tier drugs in Tier 5 require a 33% coinsurance for a 1-month supply at both preferred and standard pharmacies.
The Peak Advantage Summit (PPO) plan offers comprehensive medical coverage featuring no copay and no coinsurance for primary care visits, telehealth services, and annual preventive exams. If you require specialist visits or therapy, you will pay a copay ranging from $20 to $35 with no coinsurance. Emergency room visits carry a $90 copay, while inpatient hospital stays require copayments up to $800 with no coinsurance. For additional health needs, the plan provides preventive dental care with no copay and comprehensive dental services up to a $3,500 annual limit with a 50% coinsurance. Vision and hearing routine exams require a $20 copay, which includes up to a $200 annual allowance for eyewear and prescription hearing aid copays starting at $399. Members also benefit from a quarterly over-the-counter allowance of $120 with no copay, and home health services are fully covered with no copay or coinsurance.
