Monthly Premium
The Monthly Premium for this plan is $0.00. This is the amount you must pay every month.
This plan does not come with a Part B Premium reduction. You must continue to pay your Part B premium.
Deductibles
This plan has a $500.00 health deductible. This means, every calendar year, you pay this amount towards covered services before your insurance coverage kicks in.
This plan has no drug deductible. Your prescription medication coverage will start immediately.
Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $10000.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $10000.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

The Cigna True Choice Medicare (PPO) plan has a $0 deductible for prescription drugs. During the initial coverage phase, you will pay a copay depending on the drug tier and pharmacy you use. For example, preferred generic drugs have a $10 copay at a preferred pharmacy. Once your total drug costs reach $2000, you enter the catastrophic coverage phase where you pay nothing for Part D covered drugs. However, you may still pay a share of the costs for excluded drugs covered under any enhanced benefit.
The Cigna True Choice Medicare (PPO) plan offers comprehensive coverage, including inpatient and outpatient hospital services, with varying copays. You'll have no copay for many preventive services, as well as home health services. The plan also includes coverage for hearing, vision, and dental services with copays and coinsurance. This plan provides coverage for emergency services, ambulance services, and primary care, with copays ranging from $0 to $375 depending on the service. Additional benefits include coverage for prescription hearing aids, eyewear, and a quarterly allowance for over-the-counter items. However, some services like personal care services and additional hours of care are not covered.
Inpatient Hospital benefits, including acute and psychiatric, are covered. For days 1-6, the copay is $305, and days 7-90 have no copay.
Outpatient services, including outpatient hospital services, observation services, ambulatory surgical center services, outpatient substance abuse services, and outpatient blood services, are covered by the Cigna True Choice Medicare (PPO) plan. Outpatient hospital services have a copay between $0 and $375, observation services have a $375 copay, individual and group outpatient substance abuse sessions have a $40 copay, and ambulatory surgical center services have no copay.
Partial Hospitalization is covered by the Cigna True Choice Medicare (PPO) plan. This benefit requires prior authorization and has a copay of $105.
Ambulance and Transportation Services are covered by the Cigna True Choice Medicare (PPO) plan. Ground ambulance services have a $300 copay, while air ambulance services have a 20% coinsurance; transportation services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered by the Cigna True Choice Medicare (PPO) plan. Emergency Services and Worldwide Emergency Coverage have a $125 copay, while Urgently Needed Services have a $55 copay; all services have no coinsurance. Worldwide Emergency Services have a maximum plan benefit of $50,000.
The Cigna True Choice Medicare (PPO) plan covers primary care physician services, chiropractic services with a $20 copay, occupational therapy services with a $40 copay, physician specialist services with a $40 copay, and physical therapy and speech-language pathology services with a $40 copay. Mental health specialty services, podiatry services, and psychiatric services are partially covered, as individual and group sessions for mental health and psychiatric services are not covered. The plan also covers additional telehealth benefits with a copay between $0 and $40 and opioid treatment program services with a $40 copay.
The Cigna True Choice Medicare (PPO) plan covers preventive services, including Medicare-covered zero-dollar preventive services, annual physical exams, health education, kidney disease education, glaucoma screening, diabetes self-management training, barium enemas, digital rectal exams, and EKGs following a welcome visit. Additional services like in-home safety assessments, personal emergency response systems, medical nutrition therapy, and others are not covered.
Hearing Services are covered by the Cigna True Choice Medicare (PPO) plan, including hearing exams with a $20 copay. Prescription hearing aids are covered with a copay between $399 and $1800, but Prescription Hearing Aids - Inner Ear, Outer Ear, and Over the Ear are not covered, and OTC hearing aids are also not covered.
Vision Services includes routine eye exams with a copay between $0 and $50, and eyewear with a combined maximum benefit of $100 every year. Contact lenses, eyeglasses (lenses and frames), eyeglass lenses, eyeglass frames, and upgrades are also covered.
Cigna True Choice Medicare (PPO) covers dental services, including oral exams with a $40 copay, dental x-rays, other diagnostic dental services, prophylaxis (cleaning), fluoride treatment, and other preventive dental services. The plan also covers orthodontic services, with a maximum benefit of $1000 per year, as well as restorative services, adjunctive general services, endodontics, periodontics, prosthodontics, maxillofacial prosthetics, implant services, prosthodontics, fixed, oral and maxillofacial surgery, and orthodontics.
Home Infusion bundled Services are covered, with prior authorization required. For Medicare Part B insulin drugs, there is a $35 copay and coinsurance between 0% and 20%. Other Medicare Part B drugs, as well as Medicare Part B chemotherapy/radiation drugs, have coinsurance between 0% and 20%.
Dialysis Services are covered by the Cigna True Choice Medicare (PPO) plan, but require prior authorization. You will pay 20% coinsurance for this benefit.
Medical Equipment benefits are covered by the Cigna True Choice Medicare (PPO) plan, with a 20% coinsurance for Durable Medical Equipment, Prosthetic Devices, and Medical Supplies. Diabetic Therapeutic Shoes/Inserts are covered with a 20% coinsurance, but Durable Medical Equipment for use outside the home and Diabetic Supplies are not covered.
The Cigna True Choice Medicare (PPO) plan covers diagnostic and radiological services. Diagnostic Procedures/Tests have a copay between $0 and $150, Lab Services have no copay, Diagnostic Radiological Services have a copay of at most $225, Therapeutic Radiological Services have a copay of $60, and Outpatient X-Ray Services have a $50 copay.
Home Health Services are covered by the Cigna True Choice Medicare (PPO) plan with no copay and no coinsurance. Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are covered, but the plan does not cover any of the sub-services, including Medicare-covered Intensive Cardiac Rehabilitation Services, Medicare-covered Pulmonary Rehabilitation Services, Medicare-covered Supervised Exercise Therapy (SET) for Symptomatic Peripheral Artery Disease (PAD) Services, and Additional Cardiac Rehabilitation Services. Prior authorization is required.
Skilled Nursing Facility (SNF) services are covered, but require prior authorization. For days 1-20, there is no copay, and for days 21-100, the copay is $214 per day; additional days beyond Medicare-covered and non-Medicare-covered stays are not covered.
Other Services includes coverage for Over-the-Counter (OTC) items with a maximum benefit of $55 every three months, as well as a meal benefit for chronic illnesses or medical conditions. Acupuncture, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered.
