Plan Costs:

The cost of a Medicare Advantage Plan is made up of four main parts.

Monthly Premium

This plan does not come with a Part B Premium reduction. You must continue to pay your Part B premium.

Deductibles

This plan has a $500.00 health deductible. This means, every calendar year, you pay this amount towards covered services before your insurance coverage kicks in.

This plan has no drug deductible. Your prescription medication coverage will start immediately.

Out-of-Pocket Maximums

This plan has a combined Maximum Out-Of-Pocket cost of $11000.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $11000.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

Doctor Visits:

Regular visits to your primary care doctor are covered and will have a copay of $0 (no copay) and coinsurance of 0% (no coinsurance).

Specialist Visits:

Visits to specialists are covered and will have a copay of $35.00 and coinsurance of 0% (no coinsurance). Specialist visits may require a referral from your primary care doctor or prior authorization.

Emergency Room:

Trips to the Emergency Room are covered, and will have a copay of $110.00 and coinsurance of 0% (no coinsurance). Coverage may vary for in-network and out-of-network hospitals.

Urgent Care:

Trips to Urgent Care arecovered and will have a copay of $45.00 and coinsurance of 0% (no coinsurance). Coverage may vary for in-network and out-of-network hospitals.

In the initial coverage phase, you'll pay a copay for your prescriptions, which varies depending on the drug tier and pharmacy. For example, preferred generic drugs have a $5 copay at preferred pharmacies. After your total drug costs reach $2,000, you enter the catastrophic coverage phase, where you'll pay nothing for your Part D covered drugs. This plan's premium may be reduced if you qualify for the low-income subsidy.

Inpatient Hospital See details

Inpatient Hospital coverage includes a $250 copay for days 1-6, and no copay for days 7-90 for both Inpatient Hospital-Acute and Inpatient Hospital Psychiatric. Additional days and non-Medicare-covered stays for both Inpatient Hospital-Acute and Inpatient Hospital Psychiatric are not covered.

Outpatient Services See details

Outpatient services, including all outpatient hospital services, observation services, ambulatory surgical center services, outpatient substance abuse services, and outpatient blood services, are covered. Outpatient hospital services have a copay between $0 and $375, observation services have a $375 copay, individual and group outpatient substance abuse sessions have a $35 copay, and ambulatory surgical center services have no copay.

Partial Hospitalization See details

Partial Hospitalization is covered by the Cigna True Choice Plus Medicare (PPO) plan. This benefit requires prior authorization and has a copay of $80.

Ambulance and Transportation Services See details

The Cigna True Choice Plus Medicare (PPO) plan covers various preventive services including Medicare-covered services with no copay, annual physical exams, health education, kidney disease education services, glaucoma screenings, diabetes self-management training, barium enemas, digital rectal exams, and EKGs following a welcome visit. However, in-home safety assessments, personal emergency response systems, medical nutrition therapy, post-discharge in-home medication reconciliation, re-admission prevention, wigs for hair loss related to chemotherapy, weight management programs, alternative therapies, therapeutic massage, adult day health services, nutritional/dietary benefits, home-based palliative care, in-home support services, support for caregivers, additional sessions of smoking and tobacco cessation counseling, enhanced disease management, telemonitoring services, remote access technologies, home and bathroom safety devices and modifications, and counseling services are not covered. The plan also covers fitness benefits.

Hearing Services See details

Hearing Services include routine hearing exams with a $30 copay, and fitting/evaluation for hearing aids, both covered once per year. Prescription hearing aids (all types) are covered with a copay between $399 and $1800 twice per year, while prescription hearing aids for the inner, outer, and over the ear are not covered, and OTC hearing aids are not covered.

Vision Services See details

Vision services include eye exams with a copay of $0-$35, and eyewear benefits including contact lenses, eyeglasses (lenses and frames), eyeglass lenses, eyeglass frames, and upgrades. Eyewear has a combined maximum plan benefit of $200 every year.

Dental Services See details

Dental Services includes coverage for Medicare Dental Services with a $35 copay and other dental services, with a maximum benefit of $3,100 per year. Oral exams, dental x-rays, other diagnostic dental services, prophylaxis (cleaning), fluoride treatment, other preventive dental services, restorative services, adjunctive general services, endodontics, periodontics, prosthodontics (removable and fixed), maxillofacial prosthetics, implant services, oral and maxillofacial surgery, and orthodontics are all covered.

Home Infusion bundled Services See details

Home Infusion bundled Services are covered, but require prior authorization. For Medicare Part B Insulin Drugs, there is a $35 copay and coinsurance between 0-20%. For Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs, there is coinsurance between 0-20%.

Dialysis Services See details

Dialysis Services are covered by the Cigna True Choice Plus Medicare (PPO) plan, but require prior authorization. The coinsurance for dialysis services is 20%.

Medical Equipment See details

The Cigna True Choice Plus Medicare (PPO) plan covers Over-the-Counter (OTC) Items with a maximum benefit of $125.00 every three months, including Nicotine Replacement Therapy and Naloxone coverage. Acupuncture, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered.
