This plan does not have a health deductible. Your insurance coverage on covered health services will start immediately.
This plan has no drug deductible. Your prescription medication coverage will start immediately.
Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $7700.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $7700.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

The Cigna True Choice Medicare (PPO) plan has a $0 deductible for prescription drugs. In the initial coverage phase, after the deductible, you'll pay a copay for each drug, which varies depending on the drug tier and the pharmacy you use. For example, preferred generic drugs have a $4 copay at preferred pharmacies, while standard generic drugs have a $45 copay. Once your total drug costs reach $2000, you enter the catastrophic coverage phase, where you pay nothing for Part D covered drugs.
The Cigna True Choice Medicare (PPO) plan offers a range of benefits with varying costs. The plan includes coverage for inpatient and outpatient hospital services, as well as primary care and specialist visits, with copays ranging from $0 to $275 depending on the service. You'll also find coverage for preventive, hearing, vision, and dental services. The plan also provides benefits such as ambulance services, emergency care, and home health services. Additionally, it covers medical equipment, diagnostic and radiological services, and skilled nursing facility stays with specific copays. For acute care, you pay a $250 copay for days 1-6 and no copay for days 7-90. Psychiatric care has a $275 copay for days 1-6 and no copay for days 7-90. Additional days and upgrades for both acute and psychiatric care are not covered.
Outpatient Services are covered by the Cigna True Choice Medicare (PPO) plan. Outpatient Hospital Services have a copay between $0 and $250, Observation Services have a $250 copay, and Ambulatory Surgical Center (ASC) Services have no copay. Outpatient Substance Abuse Services have a $30 copay for both individual and group sessions, and Outpatient Blood Services are also covered.
Partial Hospitalization is covered by the Cigna True Choice Medicare (PPO) plan, with a $105 copay. Prior authorization is required for this benefit.
Ambulance and Transportation Services are covered by the Cigna True Choice Medicare (PPO) plan. Ground ambulance services have a $275 copay, while air ambulance services have a 20% coinsurance; transportation services to health-related locations are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered. Emergency Services and Worldwide Emergency Coverage, Urgent Coverage, and Transportation have a $125 copay, while Urgently Needed Services have a $55 copay; all have no coinsurance.
The Cigna True Choice Medicare (PPO) plan covers primary care physician services, occupational therapy services, physician specialist services, physical therapy and speech-language pathology services, and opioid treatment program services. Chiropractic services have a $15 copay, while occupational therapy services and physical therapy/speech-language pathology services have a $30 copay. Physician specialist services have a $30 copay, and additional telehealth benefits have a copay ranging from $0 to $30. Psychiatric services and mental health specialty services do not cover individual or group sessions, while podiatry services are not covered.
The Cigna True Choice Medicare (PPO) plan covers preventive services, including Medicare-covered preventive services, annual physical exams, health education, kidney disease education services, glaucoma screenings, diabetes self-management training, barium enemas, digital rectal exams, and EKGs following a welcome visit. However, in-home safety assessments, personal emergency response systems, medical nutrition therapy, post-discharge in-home medication reconciliation, re-admission prevention, wigs for hair loss related to chemotherapy, weight management programs, alternative therapies, therapeutic massage, adult day health services, nutritional/dietary benefits, home-based palliative care, in-home support services, support for caregivers, additional sessions of smoking and tobacco cessation counseling, enhanced disease management, telemonitoring services, remote access technologies, home and bathroom safety devices, and counseling services are not covered. The plan also covers fitness benefits.
Hearing services with the Cigna True Choice Medicare (PPO) plan include hearing exams with a $25 copay, routine hearing exams, and fitting/evaluation for hearing aids. Prescription hearing aids (all types) are covered with a copay between $399 and $1800, while prescription hearing aids - inner ear, outer ear, and over the ear are not covered, and OTC hearing aids are not covered.
Vision services are covered, including routine eye exams with a copay between $0 and $30. Eyewear is covered, with a combined maximum benefit of $250 every year for both in and out-of-network services, and contact lenses, eyeglasses (lenses and frames), eyeglass lenses, eyeglass frames, and upgrades are also covered.
The Cigna True Choice Medicare (PPO) plan covers dental services, including oral exams, dental x-rays, and other diagnostic dental services with no copay. The plan has a maximum benefit of $1500 per year for both in-network and out-of-network services, and there is a $30 copay for Medicare dental services.
Home Infusion bundled Services are covered under the Cigna True Choice Medicare (PPO) plan and require prior authorization. For Medicare Part B Insulin Drugs, there is a $35 copay and a coinsurance between 0% and 20%. Other covered benefits include Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs, both with a coinsurance between 0% and 20%.
Dialysis Services are covered under the Cigna True Choice Medicare (PPO) plan, but require prior authorization. The coinsurance for these services is 20%.
Medical Equipment benefits under the Cigna True Choice Medicare (PPO) plan include Durable Medical Equipment (DME) with 20% coinsurance and Prosthetic Devices with a 20% coinsurance, but Durable Medical Equipment for use outside the home is not covered. The plan also covers Medical Supplies with a 20% coinsurance.
Diagnostic and Radiological Services are covered. Diagnostic Procedures/Tests have a copay between $0 and $75, while Lab Services have no copay. Diagnostic Radiological Services have a copay up to $150, Therapeutic Radiological Services have a copay of $80, and Outpatient X-Ray Services have no copay.
Home Health Services are covered by the Cigna True Choice Medicare (PPO) plan with no copay and no coinsurance, but additional hours of care and personal care services are not covered. This benefit requires authorization.
Cardiac Rehabilitation Services are covered, but the specific services of Cardiac Rehabilitation, Intensive Cardiac Rehabilitation, Pulmonary Rehabilitation, and SET for PAD are not covered. Prior authorization is required.
Skilled Nursing Facility (SNF) services are covered by the Cigna True Choice Medicare (PPO) plan, with a $0 copay for days 1-20 and a $214 copay for days 21-100. Additional days beyond Medicare-covered for SNF and Non-Medicare-covered stays for SNF are not covered.
The Cigna True Choice Medicare (PPO) plan does not cover acupuncture, meal benefits, Dual Eligible SNPs with Highly Integrated Services, or the additional Other Services listed. Over-the-counter items are covered with a maximum benefit of $45.00 every three months, and the plan offers Nicotine Replacement Therapy (NRT) and Naloxone coverage as OTC benefits.
