Specialist Visits:

Visits to specialists are covered and will have a copay of $40.00 and coinsurance of 0% (no coinsurance). Specialist visits may require a referral from your primary care doctor or prior authorization.

Emergency Room:

Trips to the Emergency Room are covered, and will have a copay of $110.00 and coinsurance of 0% (no coinsurance). Coverage may vary for in-network and out-of-network hospitals.

Urgent Care:

Hearing exams are covered with a $30 copay. Routine hearing exams and fitting/evaluation for hearing aids are covered once per year. Prescription hearing aids (all types) are covered with a copay between $399 and $1800 for up to two visits per year, but prescription hearing aids for the inner ear, outer ear, and over the ear are not covered. OTC hearing aids are not covered.

Vision Services See details

Vision services are covered, including eye exams with a copay of $0-$40, and eyewear with a combined maximum plan benefit coverage of $100 per year. Eyeglasses (lenses and frames), eyeglass lenses, and eyeglass frames are limited to 1 per year, while contact lenses and upgrades are unlimited.

Dental Services See details
