Monthly Premium
The Monthly Premium for this plan is $0.00. This is the amount you must pay every month. Additionally, this plan comes with a Part B Premium reduction of $30.00. You must continue to pay paying your reduced Part B Premium.
Deductibles
This plan does not have a health deductible. Your insurance coverage on covered health services will start immediately.
Drugs are not covered by this plan, so a prescription drug deductible is not applicable.
Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $8950.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $8950.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

Prescription drugs are not covered by Cigna True Choice Courage Medicare (PPO).
The Cigna True Choice Courage Medicare (PPO) plan offers a wide range of benefits, including inpatient and outpatient hospital services, with varying copays depending on the service. Emergency, urgent, and worldwide emergency services are covered, along with primary care, preventive, hearing, vision, and dental services. The plan also covers home health, home infusion, and skilled nursing facility services, but other services like acupuncture and over-the-counter items are not covered.
Inpatient Hospital services are covered, with a copay of $380 per day for days 1-6, and no copay for days 7-90. Additional days for Inpatient Hospital-Acute are covered, while Non-Medicare-covered Stay and Upgrades for Inpatient Hospital-Acute are not covered. Inpatient Hospital Psychiatric services are covered, with a copay of $380 per day for days 1-6, and no copay for days 7-90, while Additional Days and Non-Medicare-covered Stay for Inpatient Hospital Psychiatric are not covered.
Outpatient Services are covered by the Cigna True Choice Courage Medicare (PPO) plan, including outpatient hospital services, observation services, ambulatory surgical center services, outpatient substance abuse services, and outpatient blood services. Outpatient hospital services have a copay between $0 and $395, observation services have a $395 copay, ambulatory surgical center services have no copay, and outpatient substance abuse services have a $45 copay for both individual and group sessions. Outpatient blood services are covered with a waived three-pint deductible.
Partial Hospitalization is covered under the Cigna True Choice Courage Medicare (PPO) plan, with a copay of $85.00. Prior authorization is required for this benefit.
Ambulance and Transportation Services are covered by the Cigna True Choice Courage Medicare (PPO) plan. Ground ambulance services have a $230 copay, while air ambulance services have a 20% coinsurance; however, transportation services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered by the Cigna True Choice Courage Medicare (PPO) plan. Emergency Services and Worldwide Emergency Coverage have a $120 copay, while Urgently Needed Services has a $30 copay; all services have no coinsurance.
The Cigna True Choice Courage Medicare (PPO) plan covers primary care physician services, chiropractic services with a $15 copay, occupational therapy services with a $45 copay, physician specialist services with a $45 copay, physical therapy and speech-language pathology services with a $45 copay, telehealth services with a $0-$45 copay, and opioid treatment program services with a $45 copay. Individual and group sessions for mental health and psychiatric services are not covered, and routine chiropractic care is not covered. Podiatry services are not covered.
The Cigna True Choice Courage Medicare (PPO) plan covers preventive services, including an annual physical exam, health education, kidney disease education services, glaucoma screenings, diabetes self-management training, barium enemas, digital rectal exams, and EKGs following a welcome visit. However, in-home safety assessments, personal emergency response systems (PERS), medical nutrition therapy (MNT), post-discharge in-home medication reconciliation, re-admission prevention, wigs for hair loss related to chemotherapy, weight management programs, alternative therapies, therapeutic massage, adult day health services, nutritional/dietary benefits, home-based palliative care, in-home support services, support for caregivers of enrollees, additional sessions of smoking and tobacco cessation counseling, fitness benefits, enhanced disease management, telemonitoring services, remote access technologies, home and bathroom safety devices and modifications, and counseling services are not covered.
Hearing Services include routine hearing exams with a $25 copay, and fitting/evaluation for hearing aids. Prescription hearing aids are covered, with a copay between $399 and $1800 depending on the type of hearing aid, but prescription hearing aids for the inner ear, outer ear, and over the ear are not covered, and OTC hearing aids are not covered.
Vision Services are covered, including eye exams with a copay of $0-$45 and routine eye exams once per year. However, contact lenses, eyeglasses (lenses and frames), eyeglass lenses, eyeglass frames, and upgrades are not covered.
Dental Services are covered, with a $1,200 annual maximum benefit for both in-network and out-of-network services. Medicare Dental Services require prior authorization and have a $45 copay, while other services like oral exams, dental x-rays, and orthodontics are covered.
Home Infusion bundled Services are covered, including Medicare Part B Insulin Drugs, Medicare Part B Chemotherapy/Radiation Drugs, and Other Medicare Part B Drugs. Medicare Part B Insulin Drugs have a $35 copay, with coinsurance between 0-20%.
Dialysis Services are covered by the Cigna True Choice Courage Medicare (PPO) plan. The coinsurance is between 20% and 20%.
Medical Equipment is covered by the Cigna True Choice Courage Medicare (PPO) plan, with a 20% coinsurance for Durable Medical Equipment, Prosthetic Devices, and Medical Supplies. Durable Medical Equipment for use outside the home and Diabetic Supplies are not covered, while Diabetic Therapeutic Shoes/Inserts also have a 20% coinsurance.
The Cigna True Choice Courage Medicare (PPO) plan covers diagnostic and radiological services, with a copay for some services. Diagnostic Procedures/Tests have a copay between $0 and $50, Lab Services have no copay, and Outpatient X-Ray Services have a $10 copay. Diagnostic Radiological Services have a copay of at most $150, and Therapeutic Radiological Services have a copay of $60.
Home Health Services are covered by the Cigna True Choice Courage Medicare (PPO) plan with no copay and no coinsurance, but additional hours of care and personal care services are not covered. Home health services require authorization.
Cardiac Rehabilitation Services are covered, but the plan does not cover Cardiac Rehabilitation Services, Intensive Cardiac Rehabilitation Services, Pulmonary Rehabilitation Services, or SET for PAD Services. Prior authorization is required.
Skilled Nursing Facility (SNF) services are covered by the Cigna True Choice Courage Medicare (PPO) plan, but require prior authorization. For days 1-20, the copay is $10, and for days 21-100, the copay is $214. Additional days beyond Medicare-covered for SNF and non-Medicare-covered stays for SNF are not covered.
Other Services are not covered by the Cigna True Choice Courage Medicare (PPO) plan. These services include Acupuncture, Over-the-Counter (OTC) Items, Meal Benefit, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services.
