Monthly Premium
The Monthly Premium for this plan is $0.00. This is the amount you must pay every month. Additionally, this plan comes with a Part B Premium reduction of $50.00. You must continue to pay paying your reduced Part B Premium.
Deductibles
This plan has a $650.00 health deductible. This means, every calendar year, you pay this amount towards covered services before your insurance coverage kicks in.
This plan has no drug deductible. Your prescription medication coverage will start immediately.
Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $9350.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $9350.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

The Cigna True Choice Savings Medicare (PPO) plan has a $0 deductible for prescription drugs. During the initial coverage phase, you will pay a copay for your prescriptions. The copay depends on the drug tier and the pharmacy you use, ranging from $5 to $100 for a 30-day supply. Once your total drug costs reach $2000, you enter the catastrophic coverage phase, where you will pay nothing for your Part D covered drugs. However, if you qualify for the low-income subsidy, you will pay $0 for Part D drugs.
The Cigna True Choice Savings Medicare (PPO) plan offers a variety of benefits with varying costs. This plan includes coverage for inpatient hospital stays with a $295 copay, outpatient services with copays ranging from $0-$310, and emergency services with a $125 copay. The plan also covers primary care and specialist visits, hearing and vision services, and dental services with copays ranging from $0-$40. Additional benefits include home health services with no copay, medical equipment with 20% coinsurance, and diagnostic services with copays between $0-$250.
Inpatient Hospital benefits, including Acute and Psychiatric, are covered. For Inpatient Hospital-Acute, you will pay a $295 copay for days 1-7, and no copay for days 8-90. For Inpatient Hospital Psychiatric, you will pay a $295 copay for days 1-7, and no copay for days 8-90. Additional days and upgrades for Inpatient Hospital-Acute, and additional days and non-Medicare covered stays for Inpatient Hospital Psychiatric are not covered.
Outpatient Services include coverage for outpatient hospital services with a copay between $0 and $310, observation services with a $310 copay, ambulatory surgical center (ASC) services with no copay, outpatient substance abuse services with a $40 copay for both individual and group sessions, and outpatient blood services with a waived deductible. Prior authorization is required for many services.
Partial Hospitalization is covered by the Cigna True Choice Savings Medicare (PPO) plan, but requires prior authorization. The copay for this benefit is $105.
Ambulance and Transportation Services are covered, including both ground and air ambulance services. Ground ambulance services have a $245 copay, while air ambulance services have a 20% coinsurance. Transportation services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered. Emergency Services and Worldwide Emergency Coverage have a $125 copay, while Urgently Needed Services has a $40 copay; all have no coinsurance. Worldwide Emergency Services has a maximum plan benefit of $50,000.
The Cigna True Choice Savings Medicare (PPO) plan covers primary care physician services, chiropractic services with a $20 copay, occupational therapy services with a $40 copay, physician specialist services with a $40 copay, podiatry services with a $35-$40 copay, other health care professional services with a $0-$40 copay, physical therapy and speech-language pathology services with a $40 copay, additional telehealth benefits with a $0-$40 copay, and opioid treatment program services with a $40 copay. Mental health and psychiatric individual and group sessions are not covered.
The Cigna True Choice Savings Medicare (PPO) plan covers preventive services including annual physical exams, health education, kidney disease education services, glaucoma screenings, diabetes self-management training, barium enemas, digital rectal exams, and EKGs following a Welcome Visit. Additionally, the plan offers support for caregivers and a fitness benefit. However, in-home safety assessments, personal emergency response systems, medical nutrition therapy, and several other services are not covered.
Hearing services are covered, including routine hearing exams with a $40 copay. Prescription hearing aids (all types) are covered with a copay between $399 and $1800, but inner ear, outer ear, and over the ear prescription hearing aids are not covered, and OTC hearing aids are not covered.
Vision services are covered by the Cigna True Choice Savings Medicare (PPO) plan, including eye exams with a copay of $0-$40, and eyewear with a combined maximum plan benefit coverage of $175 per year. Contact lenses, eyeglasses (lenses and frames), eyeglass lenses, eyeglass frames, and upgrades are also covered.
Dental services are covered, with a $40 copay for Medicare dental services. The plan has a maximum benefit of $1,000 per year for both in-network and out-of-network services.
Home Infusion bundled Services are covered, with prior authorization required. Medicare Part B Insulin Drugs have a $35 copay with coinsurance between 0% and 20%, while Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs have coinsurance between 0% and 20%.
Dialysis services are covered by the Cigna True Choice Savings Medicare (PPO) plan. You will pay 20% coinsurance for these services, and prior authorization is required.
Medical Equipment is covered under the Cigna True Choice Savings Medicare (PPO) plan. Durable Medical Equipment (DME) and Prosthetic Devices have a 20% coinsurance, while Durable Medical Equipment for use outside the home and Diabetic Supplies are not covered.
Diagnostic and Radiological Services include coverage for all diagnostic services and radiological services. Diagnostic Procedures/Tests have a copay between $0 and $60, Lab Services have no copay, Diagnostic Radiological Services have a copay up to $250, Therapeutic Radiological Services have 20% coinsurance, and Outpatient X-Ray Services have a $20 copay.
Home Health Services are covered by the Cigna True Choice Savings Medicare (PPO) plan with no copay and no coinsurance, but additional hours of care and personal care services are not covered. Authorization is required for this benefit.
Cardiac Rehabilitation Services are covered, but the plan does not cover any of the sub-services, so in practice, it is not covered. Prior authorization is required.
Skilled Nursing Facility (SNF) services are covered by the Cigna True Choice Savings Medicare (PPO) plan, with a $0 copay for days 1-20, and a $214 copay for days 21-100. Additional days beyond Medicare-covered for SNF and Non-Medicare-covered stays for SNF are not covered.
The Cigna True Choice Savings Medicare (PPO) plan's "Other Services" benefit covers over-the-counter (OTC) items with a maximum benefit of $55 every three months, and also covers a meal benefit for chronic illnesses or conditions requiring the enrollee to stay home. However, acupuncture, Dual Eligible SNPs, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered.
