Monthly Premium
The Monthly Premium for this plan is $0.00. This is the amount you must pay every month.
This plan does not come with a Part B Premium reduction. You must continue to pay your Part B premium.
Deductibles
This plan has a $225.00 health deductible. This means, every calendar year, you pay this amount towards covered services before your insurance coverage kicks in.
This plan has no drug deductible. Your prescription medication coverage will start immediately.
Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $14000.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $14000.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

The Cigna True Choice Medicare (PPO) plan has a $0 deductible for prescription drugs. During the initial coverage phase, you will pay a copay for your prescriptions depending on the drug tier and pharmacy you use. For example, you can expect to pay a $4 copay for preferred generic drugs at a preferred pharmacy, and a 33% coinsurance for non-preferred drugs. After your total drug costs reach $2000, you enter the catastrophic coverage phase, where you pay nothing for Part D covered drugs.
The Cigna True Choice Medicare (PPO) plan offers a wide range of benefits, including coverage for inpatient and outpatient services, emergency care, and various therapies, with copays varying by service. The plan also includes coverage for hearing, vision, and dental services, with specific copays and annual maximums. Additionally, the plan covers home health services, medical equipment, and other diagnostic and therapeutic services. This plan provides coverage for preventive services at no cost, and has a $85 quarterly benefit for over-the-counter items. The plan offers coverage for ambulance services, and includes coverage for prescription hearing aids. Inpatient Hospital coverage includes Inpatient Hospital-Acute and Inpatient Hospital Psychiatric, both requiring prior authorization. For Inpatient Hospital-Acute, there is a $255 copay for days 1-5 and no copay for days 6-90, and for Inpatient Hospital Psychiatric, there is a $325 copay for days 1-5 and no copay for days 6-90. Additional days for Inpatient Hospital-Acute are covered, but Non-Medicare-covered Stay and Upgrades for Inpatient Hospital-Acute are not covered, and Additional Days and Non-Medicare-covered Stay for Inpatient Hospital Psychiatric are also not covered.
Outpatient Services include coverage for outpatient hospital services with a copay between $0 and $325, and observation services with a $325 copay. Ambulatory Surgical Center (ASC) Services have no copay, and outpatient substance abuse services include individual and group sessions with a copay of $35. Outpatient blood services are also covered.
Partial Hospitalization is covered with a $80 copay. Prior authorization is required.
Ambulance and Transportation Services are covered by the Cigna True Choice Medicare (PPO) plan, with a $250 copay for ground ambulance services. Air ambulance services have a 20% coinsurance, while transportation services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered. Emergency Services and Worldwide Emergency Coverage have a $110 copay, while Urgently Needed Services have a $30 copay; all services have no coinsurance. Worldwide Emergency Services has a maximum plan benefit coverage of $50,000.
The Cigna True Choice Medicare (PPO) plan covers primary care physician services, chiropractic services with a $15 copay, occupational therapy services with a $35 copay, physician specialist services with a $35 copay, physical therapy and speech-language pathology services with a $35 copay, additional telehealth benefits with a copay between $0 and $35, and opioid treatment program services with a $35 copay. However, routine chiropractic care, individual and group sessions for mental health and psychiatric services, and podiatry services are not covered.
The Cigna True Choice Medicare (PPO) plan covers preventive services including Medicare-covered zero-dollar services, annual physical exams, and other preventive services, with no copay or coinsurance. Additional benefits like Health Education and Fitness Benefits are covered, while In-Home Safety Assessment, Personal Emergency Response System (PERS), and several other services are not covered.
Hearing Services are covered under the Cigna True Choice Medicare (PPO) plan, including routine hearing exams with a $20 copay and fitting/evaluation for hearing aids. Prescription hearing aids are covered, but inner ear, outer ear, and over-the-ear hearing aids are not covered, and the copay ranges from $399 to $1800. OTC hearing aids are not covered.
The Cigna True Choice Medicare (PPO) plan covers vision services, including eye exams with a copay of $0-$30, and eyewear including contact lenses, eyeglasses (lenses and frames), and eyeglass lenses and frames, with a combined maximum benefit of $300 every year. Routine eye exams are covered once per year.
Dental services are covered under the Cigna True Choice Medicare (PPO) plan, with a $2,300 maximum benefit each year for both in-network and out-of-network services. Medicare dental services have a $35 copay, while other dental services, including oral exams, dental x-rays, other diagnostic dental services, prophylaxis (cleaning), fluoride treatment, other preventive dental services, restorative services, adjunctive general services, endodontics, periodontics, prosthodontics (removable and fixed), maxillofacial prosthetics, implant services, oral and maxillofacial surgery, and orthodontics, are covered without any copay or coinsurance.
Home Infusion bundled Services are covered, including Medicare Part B Insulin Drugs, Medicare Part B Chemotherapy/Radiation Drugs, and Other Medicare Part B Drugs. For Medicare Part B Insulin Drugs, you will pay a $35 copay and between 0% and 20% coinsurance, while other services have between 0% and 20% coinsurance.
Dialysis Services are covered by the Cigna True Choice Medicare (PPO) plan, but require prior authorization. You will pay 20% coinsurance for these services.
Medical Equipment is covered by the Cigna True Choice Medicare (PPO) plan, with Durable Medical Equipment (DME) subject to 20% coinsurance, while Durable Medical Equipment for use outside the home is not covered. Prosthetics/Medical Supplies, Prosthetic Devices, and Medical Supplies are covered with 20% coinsurance, and Diabetic Therapeutic Shoes/Inserts have a 20% coinsurance, but Diabetic Supplies are not covered.
Diagnostic and Radiological Services are covered by the Cigna True Choice Medicare (PPO) plan. Diagnostic Procedures/Tests have a copay between $0 and $75, Lab Services have no copay, Diagnostic Radiological Services have a copay up to $250, Therapeutic Radiological Services have a copay of $60, and Outpatient X-Ray Services have a $15 copay.
Home Health Services are covered by the Cigna True Choice Medicare (PPO) plan, with no copay or coinsurance. Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are covered by the Cigna True Choice Medicare (PPO) plan, but the plan does not cover any of the specific services. Prior authorization is required for these services.
Skilled Nursing Facility (SNF) services are covered by the Cigna True Choice Medicare (PPO) plan, but require prior authorization. For days 1-20, there is no copay, and for days 21-100, the copay is $214. Additional days beyond Medicare-covered for SNF and non-Medicare-covered stays for SNF are not covered.
Other Services include Over-the-Counter (OTC) Items, with an $85 benefit every three months, but acupuncture, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered.
