Monthly Premium
The Monthly Premium for this plan is $0.00. This is the amount you must pay every month.
This plan does not come with a Part B Premium reduction. You must continue to pay your Part B premium.
Deductibles
This plan has a $200.00 health deductible. This means, every calendar year, you pay this amount towards covered services before your insurance coverage kicks in.
This plan has no drug deductible. Your prescription medication coverage will start immediately.
Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $10000.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $10000.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

The Cigna True Choice Medicare (PPO) plan has a $0 deductible for prescription drugs. In the initial coverage phase, you will pay a copay for your prescriptions depending on the drug tier and the pharmacy you use. Once your total drug costs reach $2,000, you enter the catastrophic coverage phase where you pay nothing for your Part D covered drugs. This plan offers an enhanced alternative drug benefit.
The Cigna True Choice Medicare (PPO) plan offers a wide range of benefits with varying cost-sharing. You'll pay a copay for services like inpatient hospital stays, outpatient services, primary care visits, and emergency services. The plan also includes coverage for preventive services, hearing, vision, and dental care, with specific copays and maximum benefits for each. This plan provides coverage for services such as ambulance, home health, and skilled nursing facilities. There is also coverage for medical equipment and diagnostic services, but some services require prior authorization. However, it is important to note that certain services like acupuncture, over-the-counter items, and some other services are not covered under this plan.
Inpatient Hospital benefits are covered, including Inpatient Hospital-Acute and Inpatient Hospital Psychiatric. For Inpatient Hospital-Acute, you pay a $305 copay for days 1-6, and no copay for days 7-90; additional days, non-Medicare covered stays, and upgrades are not covered. For Inpatient Hospital Psychiatric, you pay a $305 copay for days 1-5, and no copay for days 6-90; additional days and non-Medicare covered stays are not covered.
Outpatient Services, including outpatient hospital services, are covered by the Cigna True Choice Medicare (PPO) plan. The copay for outpatient hospital services is between $0 and $340, and the copay for observation services is $340. Ambulatory Surgical Center (ASC) Services have no copay, and Outpatient Substance Abuse Services have a $45 copay for individual and group sessions. Outpatient Blood Services are also covered, with a waived three-pint deductible.
Partial Hospitalization is covered by the Cigna True Choice Medicare (PPO) plan, but requires prior authorization. You will have a $105 copay for this benefit.
Ambulance and Transportation Services are covered by Cigna True Choice Medicare (PPO). Ground Ambulance Services have a $300 copay, while Air Ambulance Services have a 20% coinsurance. Transportation Services to a health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered under the Cigna True Choice Medicare (PPO) plan. Emergency Services have a $125 copay, Urgently Needed Services have a $55 copay, and Worldwide Emergency Services have a $125 copay for Worldwide Emergency Coverage, Worldwide Urgent Coverage, and Worldwide Emergency Transportation; all services have no coinsurance.
The Cigna True Choice Medicare (PPO) plan covers primary care physician services, chiropractic services with a $20 copay for routine care, occupational therapy services with a $35 copay, physician specialist services with a $45 copay, physical therapy and speech-language pathology services with a $35 copay, and opioid treatment program services with a $45 copay. The plan does not cover routine chiropractic care, individual and group sessions for mental health specialty services, individual and group sessions for psychiatric services, and podiatry services. The plan also offers additional telehealth benefits with a copay between $0 and $45.
The Cigna True Choice Medicare (PPO) plan covers preventive services, including Medicare-covered preventive services, annual physical exams, health education, kidney disease education services, and other preventive services such as glaucoma screening, diabetes self-management training, barium enemas, digital rectal exams, and EKGs following a Welcome Visit. Some services such as in-home safety assessments, personal emergency response systems, and others are not covered.
Hearing Services are covered by Cigna True Choice Medicare (PPO), including hearing exams with a $20 copay. Prescription hearing aids (all types) are covered with a copay between $399 and $1800, while prescription hearing aids for the inner ear, outer ear, and over the ear are not covered.
Vision services include eye exams with a copay of $0-$35, and routine eye exams once per year. Eyewear is covered with a combined maximum benefit of $150 every year for both in and out-of-network services, and includes contact lenses, eyeglasses (lenses and frames) once per year, eyeglass lenses once per year, and eyeglass frames once per year. Upgrades are also covered.
Dental Services are covered by the Cigna True Choice Medicare (PPO) plan with a $45 copay for Medicare Dental Services, and the plan has a maximum benefit of $1,300 per year for both in-network and out-of-network services. Oral exams, dental X-rays, other diagnostic dental services, prophylaxis (cleaning), fluoride treatment, other preventive dental services, restorative services, adjunctive general services, endodontics, periodontics, prosthodontics (removable and fixed), maxillofacial prosthetics, implant services, oral and maxillofacial surgery, and orthodontics are also covered.
Home Infusion bundled Services are covered, and require prior authorization. For Medicare Part B Insulin Drugs, you will pay a $35 copay, and the coinsurance will be between 0% and 20%. For Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs, the coinsurance will be between 0% and 20%.
Dialysis Services are covered by the Cigna True Choice Medicare (PPO) plan. You will pay 20% coinsurance for these services.
Medical Equipment is covered under the Cigna True Choice Medicare (PPO) plan. Durable Medical Equipment (DME) has a 20% coinsurance, and Prosthetic Devices, and Medical Supplies have a 20% coinsurance with no copay. Diabetic Equipment is covered, but Diabetic Supplies are not covered, and the plan requires prior authorization for some services.
Diagnostic and Radiological Services are covered, with prior authorization required. Diagnostic Procedures/Tests have a copay between $0 and $150, Lab Services have no copay, Diagnostic Radiological Services have a copay up to $225, Therapeutic Radiological Services have a $60 copay, and Outpatient X-Ray Services have a $50 copay.
Home Health Services are covered by the Cigna True Choice Medicare (PPO) plan with no copay and no coinsurance, but additional hours of care and personal care services are not covered. Prior authorization is required for this benefit.
Cardiac Rehabilitation Services are covered under the Cigna True Choice Medicare (PPO) plan, but the specific services are not covered. Prior authorization is required for this benefit.
Skilled Nursing Facility (SNF) services are covered by the Cigna True Choice Medicare (PPO) plan, but require prior authorization. For days 1-20, there is no copay, and for days 21-100, the copay is $214. Additional days beyond Medicare-covered SNF stays and non-Medicare-covered SNF stays are not covered.
Other services are not covered under the Cigna True Choice Medicare (PPO) plan. The plan does not cover acupuncture, over-the-counter items, meal benefits, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services.
