Monthly Premium
The Monthly Premium for this plan is $0.00. This is the amount you must pay every month.
This plan does not come with a Part B Premium reduction. You must continue to pay your Part B premium.
Deductibles
This plan has a $500.00 health deductible. This means, every calendar year, you pay this amount towards covered services before your insurance coverage kicks in.
This plan has no drug deductible. Your prescription medication coverage will start immediately.
Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $10100.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $10100.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

The Cigna True Choice Medicare (PPO) plan has a $0 deductible for prescription drugs. During the initial coverage phase, you'll pay a copay for your prescriptions, which varies depending on the drug tier and the pharmacy you use. For example, preferred generic drugs have a $4 copay at preferred pharmacies. Once your total drug costs reach $2,000, you enter the catastrophic coverage phase, where you'll pay nothing for covered drugs. However, you may still pay a share of the costs for excluded drugs covered under any enhanced benefit.
The Cigna True Choice Medicare (PPO) plan offers a variety of benefits with varying costs. This plan includes coverage for inpatient and outpatient services, with copays ranging from $0 to $375, as well as coverage for emergency, primary care, and preventive services. This plan also provides coverage for hearing, vision, and dental services, with set copays and maximum benefits. Additionally, it covers home health, skilled nursing, and other services like ambulance, diagnostic, and medical equipment with specific copays or coinsurance.
Inpatient Hospital services are covered, with a copay of $350 for days 1-5, and no copay for days 6-90 for Inpatient Hospital-Acute. Inpatient Hospital Psychiatric services are also covered, with a copay of $295 for days 1-6, and no copay for days 7-90.
Outpatient Services include coverage for all outpatient hospital services, observation services, ambulatory surgical center services, outpatient substance abuse services, and outpatient blood services. Outpatient Hospital Services have a copay between $0 and $375, while Observation Services have a $375 copay. Ambulatory Surgical Center Services have no copay, and both Individual and Group Sessions for Outpatient Substance Abuse have a copay of $35. Outpatient blood services are covered, including services not usually covered by Medicare plans.
Partial Hospitalization is covered by the Cigna True Choice Medicare (PPO) plan, with prior authorization required. You will pay an $80 copay.
Ambulance and Transportation Services are covered by the Cigna True Choice Medicare (PPO) plan. Ground ambulance services have a $240 copay, while air ambulance services have a 20% coinsurance. Transportation services to a plan-approved health-related location are covered for up to 20 one-way trips per year, using rideshare services, bus/subway, medical transport, or other methods, but transportation to any health-related location is not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered by the Cigna True Choice Medicare (PPO) plan. Emergency Services and Worldwide Emergency Coverage have a $110 copay, while Urgently Needed Services has a $45 copay, with no coinsurance for any of these services. Worldwide Emergency Services coverage has a maximum benefit of $50,000.
Primary Care benefits include coverage for Primary Care Physician Services, Chiropractic Services, Occupational Therapy Services, Physician Specialist Services, Mental Health Specialty Services, Other Health Care Professional, Psychiatric Services, Physical Therapy and Speech-Language Pathology Services, Additional Telehealth Benefits, and Opioid Treatment Program Services. Chiropractic Services have a $15 copay, Physician Specialist Services have a $35 copay, Occupational Therapy Services, Physical Therapy and Speech-Language Pathology Services have a $35 copay, Additional Telehealth Benefits have a $0-$35 copay, and Opioid Treatment Program Services have a $35 copay. Mental Health Specialty Services and Psychiatric Services do not cover individual or group sessions, Podiatry Services are not covered, and Routine Chiropractic Care is not covered.
The Cigna True Choice Medicare (PPO) plan covers preventive services, including Medicare-covered preventive services, annual physical exams, health education, kidney disease education services, glaucoma screening, diabetes self-management training, barium enemas, digital rectal exams, and EKGs following a Welcome Visit. However, this plan does not cover in-home safety assessments, personal emergency response systems, medical nutrition therapy, post-discharge in-home medication reconciliation, re-admission prevention, wigs for hair loss related to chemotherapy, weight management programs, alternative therapies, therapeutic massage, adult day health services, nutritional/dietary benefits, home-based palliative care, in-home support services, support for caregivers of enrollees, additional sessions of smoking and tobacco cessation counseling, enhanced disease management, telemonitoring services, remote access technologies, home and bathroom safety devices and modifications, or counseling services.
Hearing Services include routine hearing exams with a $35 copay, and fitting/evaluation for hearing aids. Prescription hearing aids (all types) are covered with a copay between $399 and $1800, while prescription hearing aids for the inner ear, outer ear, and over the ear are not covered. OTC hearing aids are also not covered.
Vision services include eye exams with a copay of $0-$35, and routine eye exams once per year. Eyewear is covered with a combined maximum plan benefit of $150 per year for both in-network and out-of-network services, and contact lenses, eyeglasses (lenses and frames), eyeglass lenses, eyeglass frames, and upgrades are also covered.
Dental services are covered, including oral exams with a $35 copay, and other services like dental x-rays, cleaning, and orthodontics. The plan offers a maximum benefit of $1750 per year for both in-network and out-of-network services.
Home Infusion bundled Services are covered, but require prior authorization. For Medicare Part B Insulin Drugs, there is a $35 copay and 0-20% coinsurance. For Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs, there is 0-20% coinsurance.
Dialysis Services are covered by the Cigna True Choice Medicare (PPO) plan, but require prior authorization. There is a 20% coinsurance for these services.
Medical equipment benefits are covered, including durable medical equipment (DME) with 20% coinsurance and authorization required, prosthetic devices with 20% coinsurance, medical supplies with 20% coinsurance, and diabetic therapeutic shoes/inserts with 20% coinsurance. Durable Medical Equipment for use outside the home and Diabetic Supplies are not covered.
Diagnostic and Radiological Services, including all diagnostic services, are covered under the Cigna True Choice Medicare (PPO) plan. Diagnostic Procedures/Tests have a copay between $0 and $50, while Lab Services have no copay. Diagnostic Radiological Services have a copay up to $225, Therapeutic Radiological Services have a copay up to $60, and Outpatient X-Ray Services have a $40 copay.
Home Health Services are covered by the Cigna True Choice Medicare (PPO) plan with no copay or coinsurance, but additional hours of care and personal care services are not covered. Authorization is required for this benefit.
Cardiac Rehabilitation Services are covered by the Cigna True Choice Medicare (PPO) plan, but none of the listed sub-services are covered. Prior authorization is required for these services.
Skilled Nursing Facility (SNF) services are covered by the Cigna True Choice Medicare (PPO) plan, requiring prior authorization. For days 1-20, there is no copay, and for days 21-100, the copay is $214. Additional days beyond Medicare-covered for SNF and non-Medicare-covered stays for SNF are not covered.
The Cigna True Choice Medicare (PPO) plan covers Over-the-Counter (OTC) items with a maximum benefit of $105.00 every three months, and also covers a meal benefit for chronic illnesses or medical conditions requiring the enrollee to remain at home. Acupuncture, Dual Eligible SNPs with Highly Integrated Services, and several other services are not covered.
