Routine dental, vision, and hearing exams are covered with no copays, although prescription hearing aids require a copay between $699 and $999. Diagnostic lab tests and X-rays have no copay, whereas durable medical equipment and dialysis services require a 20% coinsurance. This plan does not cover transportation, cardiac rehabilitation, chiropractic care, or over-the-counter items.
