Monthly Premium

Deductibles
This plan does not have a health deductible. Your insurance coverage on covered health services will start immediately.
This plan has no drug deductible. Your prescription medication coverage will start immediately.
Out-of-Pocket Maximums
This plan has a Maximum Out-Of-Pocket cost of $2900.00 for out-of-network services. You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $0.00 for in-network covered services, the plan will pay 100% of in-network covered costs for the rest of the year.

The Cigna Preferred Savings Medicare (HMO) plan has a $0 deductible for prescription drugs. In the initial coverage phase, you'll pay a copay for your prescriptions, depending on the drug tier and whether you use a preferred or standard pharmacy. For example, preferred generic drugs have a $4 copay, while standard generic drugs have a $45 copay. Once your total drug costs reach $2,000, you enter the catastrophic coverage phase, where you will pay nothing for your Part D covered drugs. You may still pay a share of the costs for excluded drugs covered under any enhanced benefit.
The Cigna Preferred Savings Medicare (HMO) plan offers a range of benefits, including inpatient hospital stays with a $150 copay for the first six days and no copay thereafter, as well as outpatient services with varying copays. The plan also covers emergency services, primary care, preventive services, and home health services with no copay. Additional benefits include hearing and vision services, with copays ranging from $0 to $15 for exams and up to $150 annually for eyewear. Dental services have a $1,000 annual maximum, with copays of $15 for Medicare dental services and no copay for many other dental procedures. The plan also provides coverage for ambulance services, home infusion, medical equipment, and some transportation services.
The Cigna Preferred Savings Medicare (HMO) plan covers inpatient hospital stays, including services not usually covered by Medicare, with a $150 copay for days 1-6 and no copay for days 7-90. Inpatient hospital psychiatric services are also covered with a $150 copay for days 1-6 and no copay for days 7-90, while additional days and non-Medicare-covered stays for both inpatient hospital and psychiatric services are not covered.
Outpatient services are covered by the Cigna Preferred Savings Medicare (HMO) plan, including outpatient hospital services with a copay between $0 and $275, observation services with a $150 copay, ambulatory surgical center services with no copay, outpatient substance abuse services with a $15 copay for both individual and group sessions, and outpatient blood services. Prior authorization and a doctor referral are required for some services.
Partial Hospitalization is covered under the Cigna Preferred Savings Medicare (HMO) plan with a $130 copay. Prior authorization is required for this benefit.
Ambulance and Transportation Services are covered by the Cigna Preferred Savings Medicare (HMO) plan. Ground ambulance services have a $200 copay, and air ambulance services have a 20% coinsurance. Transportation Services to a plan-approved health-related location are covered for up to 30 one-way trips per year. Transportation services to any other health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered under the Cigna Preferred Savings Medicare (HMO) plan. Emergency Services and Worldwide Emergency Coverage have a $140 copay, while Urgently Needed Services have a $10 copay; all have no coinsurance. Worldwide Emergency Services, Worldwide Urgent Coverage, and Worldwide Emergency Transportation each have a $140 copay.
The Cigna Preferred Savings Medicare (HMO) plan covers Primary Care Physician Services, Occupational Therapy Services, Physician Specialist Services, Physical Therapy and Speech-Language Pathology Services, and Opioid Treatment Program Services with a $15 copay. Chiropractic Services are covered with a $15 copay, but routine care is not covered. Mental Health and Psychiatric Services are partially covered, as individual and group sessions are not covered. Additional Telehealth benefits are covered with a copay between $0 and $15. Podiatry Services are not covered.
The Cigna Preferred Savings Medicare (HMO) plan covers preventive services, including annual physical exams, health education, glaucoma screenings, diabetes self-management training, barium enemas, digital rectal exams, and EKGs following a welcome visit. Other benefits, such as in-home safety assessments, personal emergency response systems, medical nutrition therapy, and more, are not covered.
Hearing services include hearing exams with a $15 copay, routine hearing exams, fitting/evaluation for hearing aids, and prescription hearing aids. Prescription Hearing Aids - Inner Ear, Prescription Hearing Aids - Outer Ear, Over the Ear hearing aids, and OTC hearing aids are not covered.
The Cigna Preferred Savings Medicare (HMO) plan covers vision services, including eye exams with a copay of $0-$15, and eyewear. Eyewear includes contact lenses, eyeglasses (lenses and frames), eyeglass lenses, and eyeglass frames, with a combined maximum benefit of $150 every year.
Dental services are covered under the Cigna Preferred Savings Medicare (HMO) plan, with a $1,000 maximum per year. You will pay a $15 copay for Medicare Dental Services and no copay or coinsurance for oral exams, dental x-rays, other diagnostic dental services, prophylaxis (cleaning), fluoride treatment, other preventive dental services, restorative services, adjunctive general services, endodontics, periodontics, prosthodontics (removable and fixed), maxillofacial prosthetics, implant services, oral and maxillofacial surgery, and orthodontics.
The Cigna Preferred Savings Medicare (HMO) plan covers Home Infusion bundled Services, including Medicare Part B Insulin Drugs with a $35 copay and a coinsurance between 0% and 20%. The plan also covers Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs with a coinsurance between 0% and 20%.
Dialysis Services are covered under the Cigna Preferred Savings Medicare (HMO) plan, but require prior authorization and a doctor's referral. You will pay 20% coinsurance for these services.
Medical equipment is covered under the Cigna Preferred Savings Medicare (HMO) plan. Durable Medical Equipment (DME) has a 20% coinsurance, and Durable Medical Equipment for use outside the home is not covered; Prosthetics/Medical Supplies and Diabetic Therapeutic Shoes/Inserts have a 20% coinsurance, while Diabetic Supplies are not covered.
Diagnostic and Radiological Services are covered, including diagnostic procedures and lab services with a copay between $0 and $30, and outpatient X-ray services with a $15 copay. Therapeutic Radiological Services have a coinsurance of at most 20%, and Diagnostic Radiological Services have a copay of at most $200.
Home Health Services are covered by the Cigna Preferred Savings Medicare (HMO) plan with no copay and no coinsurance; however, additional hours of care and personal care services are not covered. Authorization is required for this benefit.
Cardiac Rehabilitation Services are not covered under the Cigna Preferred Savings Medicare (HMO) plan. Note that prior authorization and a doctor referral are required for this benefit.
Skilled Nursing Facility (SNF) services are covered by the Cigna Preferred Savings Medicare (HMO) plan, with a $0 copay for days 1-20 and a $214 copay for days 21-100. Additional days beyond Medicare-covered and non-Medicare-covered SNF stays are not covered.
The Cigna Preferred Savings Medicare (HMO) plan covers Over-the-Counter (OTC) items with a maximum benefit of $45 every three months, including nicotine replacement therapy and Naloxone coverage. Meal benefits are also covered for chronic illnesses or medical conditions that require the enrollee to remain at home. Acupuncture, Dual Eligible SNPs with Highly Integrated Services, and several other services are not covered.
