Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $13900.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $13900.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

Prescription drugs are not covered by Anthem Veteran (PPO).
The Anthem Veteran (PPO) plan offers robust coverage for core medical needs, featuring no copay and no coinsurance for primary care visits, preventive services, and home health care. Specialist visits require a $45 copay, while emergency room services carry a $115 copay with no coinsurance. Inpatient hospital stays require a daily copay for the first five days with no copays for subsequent days, and outpatient services generally feature no coinsurance. This Medicare Advantage plan also provides comprehensive additional benefits, including routine dental, vision, and hearing exams with no copay or coinsurance. Members benefit from a $2,000 annual allowance for dental care with a 25% coinsurance, a $2,000 allowance for prescription hearing aids, and a $200 allowance for eyewear. Additionally, the plan covers diabetic equipment and supplies with no copay and provides a quarterly $30 allowance for over-the-counter items.
Anthem Veteran (PPO) covers inpatient hospital services with no coinsurance, requiring a daily copay of $445 for days 1 to 5 of an acute stay and $415 for days 1 to 5 of a psychiatric stay, with no copay for subsequent days. Cardiac rehabilitation services are covered under the Anthem Veteran (PPO) with no coinsurance, though prior authorization is required. While some services are covered, cardiac rehabilitation, intensive cardiac rehabilitation, pulmonary rehabilitation, and supervised exercise therapy (SET) for symptomatic peripheral artery disease (PAD) services are not covered.
Anthem Veteran (PPO) covers skilled nursing facility (SNF) services with no coinsurance, requiring prior authorization but no prior three-day inpatient hospital stay. There is no copay for days 1 through 20, a $218 daily copay for days 21 through 100, and additional days beyond the Medicare-covered limit are not covered.
Anthem Veteran (PPO) partially covers other services with no copay and no coinsurance, including Medicare Community Resource Support, chronic illness meal benefits, and up to $30 every three months for over-the-counter (OTC) items. Acupuncture is not covered under this benefit.
