Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $6350.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $6350.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

Drug Coverage
The Zing Open Choice TN (PPO) plan has a $0 deductible for prescription drugs. In the initial coverage phase, you will pay different amounts depending on the drug tier and pharmacy you use. For example, if you use a standard pharmacy, you will pay a $15 copay for preferred generic drugs, and a 33% coinsurance for preferred brand or non-preferred drugs. If you reach $2000 in total drug costs, you enter the catastrophic coverage phase, where you pay nothing for covered drugs.
Additional Benefits
The Zing Open Choice TN (PPO) plan offers comprehensive coverage, including inpatient and outpatient hospital services with varying copays. It also includes benefits for primary care, hearing, vision, and dental services, with some services having no copay. Inpatient Hospital See details
Inpatient Hospital services, including acute and psychiatric, are covered. For Inpatient Hospital-Acute, you will pay a $339 copay for days 1-6, and no copay for days 7-90; for Inpatient Hospital Psychiatric, you will pay a $339 copay for days 1-6, and no copay for days 7-90.
Outpatient Services See details
Outpatient Services include coverage for all outpatient hospital services with a $275 copay, observation services with a $220 copay, ambulatory surgical center services with a $175 copay, and outpatient substance abuse services with no copay for individual and group sessions. Outpatient blood services have no copay.
Partial Hospitalization See details
Partial Hospitalization is covered under the Zing Open Choice TN (PPO) plan, with a $70 copay. Prior authorization is required for this benefit.
Ambulance and Transportation Services See details
Ambulance and Transportation Services are covered, with prior authorization required for all ambulance services. Ground ambulance services have a $250 copay, while air ambulance services have a 20% coinsurance. Transportation services to health-related locations are not covered.

Physician specialist services have a $30 copay, while mental health specialty services and psychiatric services have no copay for individual and group sessions. Podiatry services have a $35 copay, other health care professional services have no copay, physical therapy and speech-language pathology services have a $35 copay, and additional telehealth benefits have a copay between $0 and $40. Opioid treatment program services have a $40 copay.
Preventive Services See details
The Zing Open Choice TN (PPO) plan covers preventive services, including an annual physical exam with no copay. Additional preventive services, including fitness benefits, remote access technologies, and in-home support services have a copay, while other services such as health education, in-home safety assessments, and personal emergency response systems are not covered.
Hearing Services See details
Hearing services include hearing exams with a $40 copay, routine hearing exams with no copay, and fitting/evaluation for hearing aids with no copay. Prescription hearing aids are covered up to a maximum of $750 per ear every three years, and OTC hearing aids are not covered.
Vision Services See details
Vision services include coverage for eye exams with a $40 copay, and for eyewear with no copay, including contact lenses, eyeglasses (lenses and frames), and eyeglass lenses, and eyeglass frames, with a combined maximum benefit of $200 per year; upgrades are not covered. Routine eye exams have no copay, with one visit covered every year.
Dental Services See details
Dental services are covered with no copay for Medicare and other dental services, with a maximum benefit of $2000 per year. Oral exams, dental x-rays, prophylaxis (cleaning), and fluoride treatments are covered with no copay, subject to visit limits. Restorative services, adjunctive general services, endodontics, periodontics, prosthodontics (removable and fixed), and oral and maxillofacial surgery are covered with no copay. Maxillofacial prosthetics, implant services, and orthodontics are not covered.
Home Infusion bundled Services See details
Home Infusion bundled Services are covered by the Zing Open Choice TN (PPO) plan and require prior authorization. For Medicare Part B Insulin Drugs, there is a $35 copay and coinsurance between 0% and 20%. Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs have coinsurance between 0% and 20%.

Dialysis Services are covered by the Zing Open Choice TN (PPO) plan. You will pay 20% coinsurance for these services.
Medical Equipment See details
Medical Equipment is covered by the Zing Open Choice TN (PPO) plan, including Durable Medical Equipment (DME) with 20% coinsurance, and Prosthetics/Medical Supplies with 20% coinsurance. Skilled Nursing Facility (SNF) See details
Skilled Nursing Facility (SNF) benefits are covered by the Zing Open Choice TN (PPO) plan, but require prior authorization. For days 1-20, there is no copay, but for days 21-100, there is a copay of $214 per day.
Other Services See details
The Zing Open Choice TN (PPO) plan's "Other Services" benefit includes coverage for Over-the-Counter (OTC) Items with no copay and a maximum benefit of $190 every three months, and Meal Benefits with no copay.
