Monthly Premium

Out-of-Pocket Maximums

Once your total out-of-pocket costs reach $0.00 for in-network covered services, the plan will pay 100% of in-network covered costs for the rest of the year.

Prescription drugs are not covered by Humana USAA Honor Giveback (HMO).
The Humana USAA Honor Giveback (HMO) plan offers a wide range of benefits, including no copay for primary care, preventive services, vision eyewear, and most dental services. Hospital stays have a copay, while emergency services, outpatient services, and specialist visits have varying copays. The plan also covers hearing exams, hearing aids, and home health services. Additional preventive services and kidney disease education services are covered, but the copay information is not specified. Hearing services include hearing exams with a $40 copay, routine hearing exams with no copay, and fitting/evaluation for a hearing aid with no copay. Prescription hearing aids (all types) are covered with a copay between $499 and $799, but inner ear, outer ear, and over the ear hearing aids are not covered, and OTC hearing aids are not covered.
Vision services include eye exams with a copay of $0-$40 and eyewear with no copay. Contact lenses and eyeglasses (lenses and frames) are covered under eyewear with no copay, while eyeglass lenses, eyeglass frames, and upgrades are not covered.
The Humana USAA Honor Giveback (HMO) plan covers a variety of dental services, including oral exams, dental x-rays, other diagnostic dental services, prophylaxis (cleaning), other preventive dental services, restorative services, adjunctive general services, and periodontics, all with no copay. However, fluoride treatment, endodontics, prosthodontics, maxillofacial prosthetics, implant services, oral and maxillofacial surgery, and orthodontics are not covered.
Home Infusion bundled Services are covered, with prior authorization required. For Medicare Part B Insulin Drugs, there is a $35 copay and a coinsurance between 0% and 20%. For Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs, there is a coinsurance between 0% and 20%.
Dialysis Services are covered by the Humana USAA Honor Giveback (HMO) plan, but require prior authorization. You will pay 20% coinsurance.
Medical Equipment benefits are covered under the Humana USAA Honor Giveback (HMO) plan. Durable Medical Equipment (DME) has a 20% coinsurance, while Durable Medical Equipment for use outside the home is not covered. Prosthetic devices and medical supplies have a 20% coinsurance, and diabetic supplies have a 10% coinsurance. Diabetic therapeutic shoes/inserts have no copay.
Diagnostic and Radiological Services, including all diagnostic services, diagnostic procedures/tests, and outpatient X-ray services, are covered with a copay of up to $90.00 for diagnostic procedures/tests and no copay for lab services and outpatient X-ray services; therapeutic radiological services have a coinsurance of at least 20%, and diagnostic radiological services have a copay of up to $500.
Home Health Services are covered by the Humana USAA Honor Giveback (HMO) plan with no copay and no coinsurance. Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are covered, but the plan does not cover the sub-services of Medicare-covered Supervised Exercise Therapy (SET) for Symptomatic Peripheral Artery Disease (PAD) Services, Additional Cardiac Rehabilitation Services, Medicare-covered Intensive Cardiac Rehabilitation Services, and Medicare-covered Pulmonary Rehabilitation Services. Prior authorization is required.
Skilled Nursing Facility (SNF) services are covered under the Humana USAA Honor Giveback (HMO) plan. For days 1-20, the copay is $10, and for days 21-100, the copay is $214; additional days beyond Medicare-covered and non-Medicare-covered stays are not covered.
Other Services includes acupuncture, which has a $40 copay and a limit of 20 treatments per year, and meal benefits with no copay. Over-the-counter items, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered.
