Monthly Premium

Deductibles
This plan does not have a health deductible. Your insurance coverage on covered health services will start immediately.
This plan has no drug deductible. Your prescription medication coverage will start immediately.
Out-of-Pocket Maximums
This plan has a Maximum Out-Of-Pocket cost of $1000.00 for out-of-network services. You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $0.00 for in-network covered services, the plan will pay 100% of in-network covered costs for the rest of the year.

The Humana Gold Plus - Diabetes and Heart (HMO C-SNP) plan has a $0 deductible for prescription drugs. During the initial coverage phase, you'll pay varying copays or coinsurance depending on the drug tier and pharmacy type. For example, preferred generic drugs have no copay at preferred pharmacies and a $20 copay at standard mail order pharmacies. For preferred brand drugs, you'll pay 35% coinsurance at preferred pharmacies and 44% coinsurance at standard mail order pharmacies. Once your total drug costs reach $2000, you enter the catastrophic coverage phase, where you pay nothing for Part D covered drugs.
The Humana Gold Plus - Diabetes and Heart (HMO C-SNP) plan offers comprehensive coverage with a focus on managing diabetes and heart health. Many services have no copay, including inpatient hospital stays, outpatient services, preventive services, primary care, home health, hearing exams, vision exams, and dental services. The plan also covers ambulance services, emergency services, and offers coverage for prescription hearing aids with copays between $699 and $999. Other services like diagnostic and radiological services and skilled nursing facilities have copays or coinsurance.
Inpatient Hospital benefits, including Inpatient Hospital-Acute and Inpatient Hospital Psychiatric, are covered with no copay for Medicare-covered stays. Additional days for Inpatient Hospital-Acute are covered with no copay, while Non-Medicare-covered Stay and Upgrades for Inpatient Hospital-Acute and Additional Days and Non-Medicare-covered Stay for Inpatient Hospital Psychiatric are not covered.
Outpatient Services include coverage for all outpatient hospital services with a copay between $0 and $30, observation services with no copay, ambulatory surgical center services with no copay, outpatient substance abuse services with a copay between $0 and $30 for individual and group sessions, and outpatient blood services with no copay. Prior authorization and a doctor referral are required for some services.
Partial Hospitalization is covered by the Humana Gold Plus - Diabetes and Heart (HMO C-SNP) plan. This benefit has a $30 copay, and requires prior authorization.
Ambulance and Transportation Services are covered, including both ground and air ambulance services, as well as transportation services to a plan-approved health-related location. Ground ambulance services have a copay of $315, and air ambulance services have a copay of $630. Transportation services to a plan-approved health-related location have no copay, with a limit of 36 one-way trips per year. Transportation services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered by the Humana Gold Plus - Diabetes and Heart (HMO C-SNP) plan. Emergency Services and Worldwide Emergency Coverage have a $140 copay, while Urgently Needed Services have a $10 copay. There is no coinsurance for any of these services.
The Humana Gold Plus - Diabetes and Heart (HMO C-SNP) plan covers primary care physician services, physician specialist services, individual and group sessions for mental health and psychiatric services, and routine foot care with no copay. Chiropractic services have a $20 copay, while occupational therapy services have a $0-$10 copay. Physical therapy and speech-language pathology services, as well as additional telehealth benefits, also have a copay of $0-$10.
Preventive Services include coverage for Medicare-covered preventive services, annual physical exams with no copay, and additional preventive services. Other covered preventive services like Glaucoma Screening, Diabetes Self-Management Training, Barium Enemas, Digital Rectal Exams, and EKG following Welcome Visit, also have no copay.
Hearing Services include hearing exams, routine hearing exams, fitting/evaluation for hearing aids, prescription hearing aids, and OTC hearing aids. Hearing exams have no copay. Routine hearing exams have no copay. Fitting/evaluation for hearing aids have no copay. Prescription hearing aids (all types) have a copay between $699 and $999, while prescription hearing aids for inner ear, outer ear, and over the ear are not covered. OTC hearing aids are covered with a maximum amount of $60 every three months.
Vision Services includes coverage for eye exams and eyewear, with a $0 copay for routine eye exams, contact lenses, and eyeglasses (lenses and frames). Eyeglass lenses, eyeglass frames, and upgrades are not covered.
The Humana Gold Plus - Diabetes and Heart (HMO C-SNP) plan covers dental services, including oral exams, dental x-rays, other diagnostic dental services, prophylaxis (cleaning), other preventive dental services, restorative services, adjunctive general services, endodontics, periodontics, and prosthodontics (fixed), with no copay. Fluoride treatment, prosthodontics (removable), maxillofacial prosthetics, implant services, and orthodontics are not covered.
Home Infusion bundled Services are covered, with prior authorization required. For Medicare Part B Insulin Drugs, there is a $35 copay with 0-20% coinsurance, while Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs have 0-20% coinsurance.
Dialysis Services are covered, but require prior authorization and a doctor's referral. You will pay 20% coinsurance for these services.
Medical Equipment coverage includes Durable Medical Equipment (DME) with 20% coinsurance and Prosthetics/Medical Supplies with a coinsurance for Medicare-covered medical supplies and copays for Medicare-covered prosthetic devices. Diabetic equipment is also covered, with no copay for Diabetic Supplies and Diabetic Therapeutic Shoes/Inserts.
Diagnostic and Radiological Services are covered, including diagnostic procedures/tests with a copay between $0 and $50, lab services with no copay, diagnostic radiological services with a copay at most $50, therapeutic radiological services with at least 20% coinsurance, and outpatient X-ray services with no copay. All services require prior authorization and a doctor referral.
Home Health Services are covered by the Humana Gold Plus - Diabetes and Heart (HMO C-SNP) plan with no copay and no coinsurance, but additional hours of care and personal care services are not covered. Authorization and a referral are required for this benefit.
Cardiac Rehabilitation Services are covered by the Humana Gold Plus - Diabetes and Heart (HMO C-SNP) plan, but the plan does not cover Intensive Cardiac Rehabilitation Services, Pulmonary Rehabilitation Services, or SET for PAD Services. A doctor's referral and prior authorization are required for this benefit.
Skilled Nursing Facility (SNF) services are covered by the Humana Gold Plus - Diabetes and Heart (HMO C-SNP) plan, but require prior authorization and a doctor's referral. For days 1-20, the copay is $10, and for days 21-100, the copay is $214. Additional days beyond Medicare-covered, and non-Medicare-covered stays, are not covered.
The Humana Gold Plus - Diabetes and Heart (HMO C-SNP) plan covers acupuncture with no copay, but requires prior authorization and is limited to 20 treatments per year. Over-the-counter items are covered, with a $60 maximum benefit every three months, and the plan also offers a meal benefit with no copay and prior authorization required. Several additional services are not covered.
