For Tier 1 preferred generics, Tier 2 generics, and Tier 6 select care drugs, members enjoy no copay for 1-month or 3-month supplies at standard pharmacies and standard mail order. Higher-tier medications, including Tier 3 preferred brands, Tier 4 non-preferred brands, and Tier 5 specialty drugs, require a 25% coinsurance. This 25% coinsurance rate applies to both 1-month and 3-month supplies at standard pharmacies, as well as standard mail order for Tier 3 and Tier 4. Emergency services are available with a one hundred fifteen dollar copay, which is waived if admitted, while urgently needed care requires no copay. Supplemental benefits include no copay for preventive dental care up to a three thousand dollar yearly limit and a five hundred dollar annual allowance for eyewear with no copay. Additionally, routine hearing exams and transportation services for up to thirty-six one-way trips per year are provided with no copay. Prescription hearing aids are available with a flat one hundred forty-nine dollar copay, while select durable medical equipment carries no copay with coinsurance up to twenty percent.

Ambulance and transportation services are covered by Champion Choice (HMO C-SNP), with ground ambulance costing between no copay to a $125 copay and no coinsurance to 20% coinsurance, and air ambulance requiring 20% coinsurance. Transportation services are partially covered with no copay and no coinsurance for up to 36 one-way trips per year to plan-approved locations, while transportation to any health-related location is not covered.
Champion Choice (HMO C-SNP) covers emergency services with a $115 copay and no coinsurance, which is waived if you are admitted to the hospital within 24 hours. Urgently needed services have no copay and no coinsurance, and worldwide emergency and urgent services are partially covered up to a $10,000 maximum with no copay or coinsurance, though worldwide emergency transportation is not covered.

Prescription hearing aids are partially covered with a $149 copay and no coinsurance for up to two devices every three years, though inner ear, outer ear, over the ear, and over-the-counter (OTC) hearing aids are not covered.
Champion Choice (HMO C-SNP) offers partially covered vision services with no copay, no coinsurance, and no deductible, including one routine eye exam annually and eyeglasses (lenses and frames) with a $500 yearly maximum. Other eye exam services, contact lenses, individual eyeglass lenses, and individual eyeglass frames are not covered.
Dental services are partially covered by Champion Choice (HMO C-SNP), offering preventive and diagnostic care with no copay and no coinsurance up to a $3,000 yearly maximum. Comprehensive dental services are covered with no copay and 20% to 40% coinsurance, though adjunctive general services, maxillofacial prosthetics, implant services, and orthodontics are not covered.
Home infusion bundled services are covered by Champion Choice (HMO C-SNP) with no copay and no coinsurance, though prior authorization is required. Under this benefit, Medicare Part B insulin drugs have a $0 to $35 copay and no coinsurance, while chemotherapy, radiation, and other Part B drugs require no copay and 0% to 20% coinsurance.
Dialysis services are covered by Champion Choice (HMO C-SNP) with no copay and a 20% coinsurance.

Home Health Services are covered under the Champion Choice (HMO C-SNP) plan with no copay and no coinsurance. Both a referral and prior authorization are required to receive these services.
Champion Choice (HMO C-SNP) covers some cardiac rehabilitation services with no copay, requiring prior authorization and referrals. However, standard cardiac rehabilitation, intensive cardiac rehabilitation, pulmonary rehabilitation, and supervised exercise therapy (SET) for symptomatic peripheral artery disease (PAD) services are not covered and require a 20% coinsurance.
Champion Choice (HMO C-SNP) covers skilled nursing facility (SNF) care with no coinsurance, featuring no copay for days 1 through 20 and a $218 daily copay for days 21 through 100. Prior authorization and referrals are required, and additional days beyond the Medicare-covered limit are not covered.
