The Champion Plus (HMO C-SNP) Medicare plan features an Enhanced Alternative drug benefit with an annual prescription drug deductible of $615.00. Under this plan, you will have no copay for Tier 1 preferred generic drugs and Tier 5 specialty tier drugs at standard pharmacies. For Tier 2 standard generics, Tier 3 preferred brands, and Tier 4 non-preferred drugs, you will pay a 25% coinsurance. These cost-sharing rates apply during the initial coverage phase until your total drug costs reach $2,100.00. Once your yearly out-of-pocket drug costs exceed $2,100.00, you enter the catastrophic coverage phase and pay nothing for covered Part D drugs. Additionally, if you qualify for the low-income subsidy, your Part D premium may be reduced to $12.00.

For inpatient hospital stays, cost-sharing aligns with Original Medicare, and skilled nursing facility stays are covered with no copay for the first 20 days. This plan also features generous supplemental benefits, including preventive dental care with no copay and comprehensive dental services covered up to a $3,000 annual limit with 20% to 40% coinsurance.
