Monthly Premium

Deductibles

Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $8950.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $8950.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

Prescription drugs are not covered by Humana USAA Honor Giveback (PPO).
The Humana USAA Honor Giveback (PPO) plan offers a range of benefits with varying costs. Inpatient hospital stays have a copay, with no copay for days 6-90, while outpatient services have copays ranging from $0 to $800. You'll also find coverage for primary care, specialist visits, and other services, often with a copay, and preventive services are available with no copay. The plan includes coverage for hearing, vision, and dental services, but with specific limitations on what is covered, and with copays or coinsurance required for some services. Additionally, the plan offers benefits such as ambulance services, emergency services, and home health services, as well as prescription hearing aids, home infusion services, and dialysis services.
Inpatient Hospital benefits, including Inpatient Hospital-Acute and Inpatient Hospital Psychiatric, are covered. For Inpatient Hospital-Acute, you will pay a $495 copay for days 1-5, and no copay for days 6-90, with no coinsurance. For Inpatient Hospital Psychiatric, you will pay a $450 copay for days 1-5, and no copay for days 6-90, with no coinsurance. Additional Days for Inpatient Hospital-Acute are covered with no copay or coinsurance for days 91-999. Non-Medicare-covered stays and upgrades for Inpatient Hospital-Acute and Additional Days for Inpatient Hospital Psychiatric are not covered.
Outpatient Services include coverage for all outpatient hospital services, with a copay between $40 and $800, and observation services with a $495 copay. Ambulatory Surgical Center (ASC) Services have a $300 copay, outpatient substance abuse services have a copay between $40 and $70, and outpatient blood services have no copay.
Partial Hospitalization is covered under the Humana USAA Honor Giveback (PPO) plan, and requires prior authorization. You will pay a $55 copay for this benefit.
Ambulance and Transportation Services are covered, with prior authorization required. Ground and air ambulance services have a $315 copay, while transportation services to a plan-approved health-related location have no copay, up to 24 one-way trips per year. Transportation services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered. Emergency Services have a $125 copay, and Urgently Needed Services have a $55 copay; all services have no coinsurance. Worldwide Emergency Coverage, Worldwide Urgent Coverage, and Worldwide Emergency Transportation each have a $125 copay.
The Humana USAA Honor Giveback (PPO) plan covers primary care physician services and chiropractic services with no copay, and covers occupational therapy services with a $40 copay. Specialist visits have a $40 copay, and physical therapy and speech-language pathology services have a $40 copay. Additional telehealth benefits have a copay between $0 and $55, and mental health and psychiatric services have a $40 copay.
The Humana USAA Honor Giveback (PPO) plan covers preventive services with no copay for an annual physical exam and other services. Additional preventive services, including fitness benefits, are covered with a copay, and other services such as health education, in-home safety assessments, and more are not covered.
Hearing exams are covered with a $40 copay, and routine hearing exams are covered with no copay for 1 visit every year. Prescription Hearing Aids are partially covered, with all types of hearing aids covered with a copay between $399 and $699 for 2 visits every year, but inner ear, outer ear, and over the ear hearing aids are not covered. OTC hearing aids are covered, with a maximum benefit of $50 every three months.
The Humana USAA Honor Giveback (PPO) plan covers vision services, including eye exams with a copay of $0-$40 and eyewear with no copay. Contact lenses and eyeglasses are covered with no copay. However, eyeglass lenses, eyeglass frames, and upgrades are not covered.
Dental services are covered, with a $1,000 annual maximum benefit. Medicare Dental Services require a $40 copay, while Oral Exams, Dental X-Rays, Other Diagnostic Dental Services, Prophylaxis (Cleaning), and Other Preventive Dental Services have no copay. Restorative Services and Prosthodontics, fixed have a 30% to 40% coinsurance, and Orthodontic Services are covered under Diagnostic and Preventive Dental. Fluoride Treatment, Prosthodontics, removable, Maxillofacial Prosthetics, Implant Services, and Orthodontics are not covered.
Home Infusion bundled Services are covered, including Medicare Part B Insulin Drugs with a $35 copay and a coinsurance between 0% and 20%, and Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs with a coinsurance between 0% and 20%. Prior authorization is required.
Dialysis Services are covered by the Humana USAA Honor Giveback (PPO) plan, but require prior authorization. There is a 20% coinsurance for this benefit.
The Humana USAA Honor Giveback (PPO) plan covers Durable Medical Equipment (DME) with a 16% coinsurance and requires authorization. Prosthetic Devices are covered with 19% coinsurance. Medical supplies have a 15% coinsurance. Diabetic Supplies have a 10% coinsurance with no copay, and Diabetic Therapeutic Shoes/Inserts have no copay. Durable Medical Equipment for use outside the home is not covered.
Diagnostic and Radiological Services are covered. Diagnostic Procedures/Tests have a copay between $0 and $90, while Lab Services have no copay. Diagnostic Radiological Services have a copay between $40 and $350, and Therapeutic Radiological Services have a 20% coinsurance. Outpatient X-Ray Services have no copay.
Home Health Services are covered by the Humana USAA Honor Giveback (PPO) plan with no copay and no coinsurance. However, additional hours of care and personal care services are not covered.
Cardiac Rehabilitation Services are not covered by the Humana USAA Honor Giveback (PPO) plan. Prior authorization is required for these services.
Skilled Nursing Facility (SNF) services are covered under the Humana USAA Honor Giveback (PPO) plan, with a $0 copay for days 1-20 and a $196 copay for days 21-100. Additional days beyond Medicare-covered for SNF and non-Medicare-covered stays for SNF are not covered.
Other Services includes acupuncture, over-the-counter (OTC) items, and a meal benefit. Acupuncture has a $40 copay per visit and requires prior authorization, while OTC items are covered up to $50 every three months, including nicotine replacement therapy and naloxone; the meal benefit has no copay and requires prior authorization. The following services are not covered: Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services.
