Plan Costs:
The cost of a Medicare Advantage Plan is made up of four main parts.

Monthly Premium
The Monthly Premium for this plan is $50.60. This is the amount you must pay every month.
This plan does not come with a Part B Premium reduction. You must continue to pay your Part B premium.
Deductibles
This plan does not have a health deductible. Your insurance coverage on covered health services will start immediately.
This plan has a $590.00 drug deductible. You will need to pay this amount towards covered prescriptions before your insurance coverage for prescription medications kicks in.
Out-of-Pocket Maximums
This plan has a Maximum Out-Of-Pocket cost of $9350.00 for out-of-network services. You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $0.00 for in-network covered services, the plan will pay 100% of in-network covered costs for the rest of the year.

Drug Coverage
The UCare Connect + Medicare (HMO D-SNP) plan has a defined standard drug benefit. The plan has a deductible of $590.00. If you qualify for the low-income subsidy, you will pay a monthly premium of $50.60. During the initial coverage phase, you will pay the costs for drugs in each tier until your total drug costs reach $2000.00. After your yearly out-of-pocket drug costs reach $2000.00, you pay nothing for Medicare Part D covered drugs.
Additional Benefits
The UCare Connect + Medicare (HMO D-SNP) plan offers a range of benefits, with a focus on outpatient and primary care services. Many services, including outpatient, emergency, primary care, preventive, vision, dental, home infusion, dialysis, medical equipment, diagnostic, and radiological services, are covered, but often with a 20% coinsurance. Home health services, however, come with no copay. This plan also covers ambulance and transportation services, and some other services like acupuncture and over-the-counter items, though some services may have limitations. Be aware that certain services are not covered, including some hearing, vision, and dental services, as well as cardiac rehabilitation.
Inpatient Hospital See details
Inpatient Hospital benefits, including acute and psychiatric care, are covered, but the plan does not cover additional days, non-Medicare-covered stays, or upgrades for either type of inpatient hospital care. You will be responsible for the Medicare-defined cost share, and you should consult the plan details for more information about the copay.
Outpatient Services See details
Outpatient Services are covered under the UCare Connect + Medicare (HMO D-SNP) plan, with 20% coinsurance for outpatient hospital services and observation services. Ambulatory Surgical Center (ASC) Services and Outpatient Substance Abuse Services are covered with a coinsurance between 20% and 20%. Outpatient Blood Services are not covered.
Partial Hospitalization See details
Partial Hospitalization is covered under the UCare Connect + Medicare (HMO D-SNP) plan with a 20% coinsurance.
Ambulance and Transportation Services See details
Ambulance and Transportation Services are covered, including ground and air ambulance services with a 20% coinsurance. Transportation Services to a plan-approved health-related location are covered for 3 round trips per week to a participating fitness center via rideshare services, van, medical transport, or other transportation, but transportation services to any other health-related location are not covered.
Emergency Services See details
Emergency Services, including Urgently Needed Services, are covered by UCare Connect + Medicare (HMO D-SNP). Emergency Services have a 20% coinsurance, and Urgently Needed Services also have a 20% coinsurance. Worldwide Emergency Services are not covered.
Primary Care See details
The UCare Connect + Medicare (HMO D-SNP) plan covers primary care physician services, chiropractic services, occupational therapy, physician specialist services, mental health specialty services, podiatry services, other health care professional, psychiatric services, physical therapy, speech-language pathology, additional telehealth benefits, and opioid treatment program services. Primary care physician services, chiropractic services, physician specialist services, physical therapy, speech-language pathology, and additional telehealth benefits have a 20% coinsurance, while occupational therapy, mental health specialty services, podiatry services, other health care professional, and psychiatric services also have a 20% coinsurance.
Preventive Services See details
Preventive services are covered, though annual physical exams, health education, in-home safety assessments, personal emergency response systems, medical nutrition therapy, post-discharge in-home medication reconciliation, wigs for hair loss, weight management programs, alternative therapies, adult day health services, nutritional/dietary benefits, home-based palliative care, in-home support services, additional sessions of smoking and tobacco cessation counseling, enhanced disease management, telemonitoring services, home and bathroom safety devices and modifications, and counseling services are not covered. Kidney disease education services, glaucoma screening, diabetes self-management training, barium enemas, digital rectal exams, and EKGs following a welcome visit have a 20% coinsurance. Re-admission prevention, therapeutic massage, support for caregivers of enrollees, fitness benefits, and remote access technologies are also covered.
Hearing Services See details
Hearing services are partially covered by the UCare Connect + Medicare (HMO D-SNP) plan, with a coinsurance of at most 20% for hearing exams, but routine hearing exams, fitting/evaluation for hearing aids, and all types of prescription hearing aids are not covered. OTC hearing aids are also not covered.
Vision Services See details
Vision services are covered by UCare Connect + Medicare (HMO D-SNP), including eye exams with 20% coinsurance, however routine eye exams are not covered. Eyewear is covered with 20% coinsurance, but contact lenses, eyeglasses (lenses and frames), eyeglass lenses, and eyeglass frames are not covered.
Dental Services See details
Dental services are covered, including Medicare Dental Services with 20% coinsurance. Other dental services include oral exams and dental x-rays, and restorative services, all of which have limits on the number of visits per year. Other services like prophylaxis (cleaning), fluoride treatment, adjunctive general services, endodontics, prosthodontics, maxillofacial prosthetics, implant services, oral and maxillofacial surgery, and orthodontics are not covered.
Home Infusion bundled Services See details
Home Infusion bundled Services, including Medicare Part B Insulin Drugs, Medicare Part B Chemotherapy/Radiation Drugs, and Other Medicare Part B Drugs, are covered. For Medicare Part B Insulin Drugs, the copay is $35. For Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs, the coinsurance is between 0% and 20%.

Dialysis Services are covered under the UCare Connect + Medicare (HMO D-SNP) plan. You will pay a 20% coinsurance for these services.
Medical Equipment See details
Medical Equipment is covered, including Durable Medical Equipment (DME), Prosthetics/Medical Supplies, and Diabetic Equipment. DME has a 20% coinsurance, while Durable Medical Equipment for use outside the home is not covered. Prosthetic Devices, Medical Supplies, Diabetic Supplies, and Diabetic Therapeutic Shoes/Inserts also have a 20% coinsurance.
Diagnostic and Radiological Services See details
Diagnostic and Radiological Services are covered under the UCare Connect + Medicare (HMO D-SNP) plan. Diagnostic Procedures/Tests, Diagnostic Radiological Services, Therapeutic Radiological Services, and Outpatient X-Ray Services have a coinsurance of at most 20%, while Medicare-covered lab services are not covered.

Cardiac Rehabilitation Services See details
