Monthly Premium
The Monthly Premium for this plan is $2.80. This is the amount you must pay every month.
This plan does not come with a Part B Premium reduction. You must continue to pay your Part B premium.
Deductibles
This plan does not have a health deductible. Your insurance coverage on covered health services will start immediately.
This plan has a $615.00 drug deductible. You will need to pay this amount towards covered prescriptions before your insurance coverage for prescription medications kicks in.
Out-of-Pocket Maximums
This plan has a Maximum Out-Of-Pocket cost of $9250.00 for out-of-network services. You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $0.00 for in-network covered services, the plan will pay 100% of in-network covered costs for the rest of the year.

The Advantage D-SNP (HMO) (HMO D-SNP) plan features an Enhanced Alternative drug benefit with an annual prescription drug deductible of $615.00. During the initial coverage phase, members pay a 25% coinsurance for Tier 1 through Tier 4 drugs at standard pharmacies, while Tier 5 specialty drugs have no copay. These standard pharmacy costs apply after the deductible is met and continue until total drug expenditures reach $2,100.00. For individuals who qualify for the low-income subsidy or Extra Help, the Part D cost is reduced to $2.80. Once your yearly out-of-pocket drug costs reach $2,100.00, you enter the catastrophic coverage phase and will pay nothing for covered Part D prescription drugs. This plan offers a structured cost-sharing design to help you manage your medication expenses.
The Advantage D-SNP (HMO) (HMO D-SNP) plan offers comprehensive medical coverage, with many key services requiring no copay and a standard 20% coinsurance. This cost-sharing structure applies to primary care, outpatient services, emergency care, dialysis, and durable medical equipment, though prior authorization is required for several of these benefits. Inpatient hospital stays and skilled nursing facility care are also covered but are subject to Medicare-defined copays and coinsurance. For supplemental care, the plan provides routine hearing and vision exams with no copay and a 20% coinsurance, featuring allowances of up to $1,500 annually for prescription hearing aids and $300 every two years for eyewear. Members also receive an over-the-counter benefit of $150 every three months for health-related items. However, standard dental services are limited to Medicare-covered dental benefits with a 20% coinsurance, and routine transportation is not covered.
Inpatient hospital benefits are partially covered by Advantage D-SNP (HMO) (HMO D-SNP), requiring prior authorization and charging Medicare-defined copays and coinsurance. The plan does not cover additional days, non-Medicare-covered stays, or upgrades for acute and psychiatric inpatient hospitalizations.
Outpatient services are covered by Advantage D-SNP (HMO) (HMO D-SNP) with no copays and a 20% coinsurance for outpatient hospital, observation, ambulatory surgical center, substance abuse, and blood services. Prior authorization is required for outpatient hospital and ambulatory surgical center services.
Partial hospitalization benefits are covered under the Advantage D-SNP (HMO) (HMO D-SNP) plan, subject to a 20% coinsurance and no copay. Prior authorization is required before receiving these services.
Ambulance and Transportation Services are partially covered by Advantage D-SNP (HMO) (HMO D-SNP), requiring a 20% coinsurance and no copay for ground and air ambulance services. Transportation services to plan-approved health-related locations and any health-related locations are not covered.
Advantage D-SNP (HMO) (HMO D-SNP) partially covers emergency services, offering emergency and urgently needed services with a 20% coinsurance and no copay. Worldwide emergency coverage, worldwide urgent coverage, and worldwide emergency transportation are not covered.
Advantage D-SNP (HMO) (HMO D-SNP) partially covers Primary Care benefits with no copay and 20% coinsurance for covered services. Podiatry and routine chiropractic services are not covered.
Advantage D-SNP (HMO) (HMO D-SNP) offers partially covered preventive services, including Medicare-covered zero-dollar services with no copay and no coinsurance. While kidney disease education and other specific screenings require no copay and a 20% coinsurance, annual physical exams and additional benefits like fitness and health education are not covered.
Hearing services are partially covered by Advantage D-SNP (HMO) (HMO D-SNP), which offers one routine hearing exam and one fitting evaluation annually with no copay and a 20% coinsurance. While prescription hearing aids are covered up to $1,500 per year for both ears combined, OTC hearing aids and inner ear, outer ear, and over-the-ear prescription hearing aids are not covered.
Advantage D-SNP (HMO) (HMO D-SNP) covers routine eye exams with no copay, no deductible, and a 20% coinsurance. Eyewear is partially covered with no copay, no deductible, a 20% coinsurance, and a $300 maximum limit every two years, though individual eyeglass lenses, eyeglass frames, and upgrades are not covered.
Advantage D-SNP (HMO) (HMO D-SNP) partially covers dental services, offering Medicare dental benefits with a 20% coinsurance and no copay, subject to prior authorization. Standard dental sub-services are not covered, including restorative, endodontics, periodontics, prosthodontics, implants, orthodontics, oral surgery, maxillofacial prosthetics, and adjunctive general services.
Home Infusion bundled Services are partially covered by Advantage D-SNP (HMO) (HMO D-SNP) with prior authorization, as Part D home infusion drugs are not covered as a bundled service. Covered Part B chemotherapy, radiation, and other Part B drugs require no copay and range from no coinsurance to 20% coinsurance, while Medicare Part B insulin drugs require a $35 copay and range from no coinsurance to 20% coinsurance.
Dialysis Services are covered by the Advantage D-SNP (HMO) (HMO D-SNP) plan with no copay and a 20% coinsurance. This coverage helps you manage the costs of essential dialysis treatments with clear and predictable cost-sharing.
Advantage D-SNP (HMO) (HMO D-SNP) covers medical equipment, including durable medical equipment, prosthetics, medical supplies, and diabetic equipment, with no copay and a 20% coinsurance. Prior authorization is required for durable medical equipment and prosthetic devices or medical supplies.
Advantage D-SNP (HMO) (HMO D-SNP) covers diagnostic and radiological services with no copay and a 20% coinsurance. Prior authorization is required for these covered benefits, which include diagnostic tests, lab services, therapeutic radiological services, and outpatient X-rays.
Home Health Services are covered under the Advantage D-SNP (HMO) (HMO D-SNP) plan, with prior authorization required to receive these benefits.
Cardiac Rehabilitation Services are not covered under the Advantage D-SNP (HMO) (HMO D-SNP) plan, meaning there is no copay or coinsurance, and all sub-services—including intensive cardiac, pulmonary, and SET for PAD rehabilitation—are excluded from coverage.
Advantage D-SNP (HMO) (HMO D-SNP) partially covers Skilled Nursing Facility (SNF) services subject to Medicare-defined copays and coinsurance, requiring prior authorization and a prior three-day inpatient hospital stay. Supplemental Part C SNF services and additional days beyond Medicare-covered SNF stays are not covered.
Advantage D-SNP (HMO) (HMO D-SNP) partially covers Other Services, providing a maximum benefit of $150 every three months for Over-the-Counter (OTC) items. Acupuncture, meal benefits, Nicotine Replacement Therapy, Naloxone, and Dual Eligible SNPs with Highly Integrated Services are not covered.
