Plan Costs:

The cost of a Medicare Advantage Plan is made up of four main parts.

Monthly Premium

The Monthly Premium for this plan is $139.00. This is the amount you must pay every month.

This plan does not come with a Part B Premium reduction. You must continue to pay your Part B premium.

Deductibles

This plan does not have a health deductible. Your insurance coverage on covered health services will start immediately.

This plan has a $590.00 drug deductible. You will need to pay this amount towards covered prescriptions before your insurance coverage for prescription medications kicks in.

Out-of-Pocket Maximums

This plan has a combined Maximum Out-Of-Pocket cost of $6300.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $6300.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

Specialist Visits:

Visits to specialists are covered and will have a copay of $0.00 and coinsurance of 0% (no coinsurance). Specialist visits may require a referral from your primary care doctor or prior authorization.

Emergency Room:

Trips to the Emergency Room are covered, and will have a copay of $60.00 and coinsurance of 0% (no coinsurance). Coverage may vary for in-network and out-of-network hospitals.

Urgent Care:

Trips to Urgent Care arecovered and will have a copay of $30.00 and coinsurance of 0% (no coinsurance). Coverage may vary for in-network and out-of-network hospitals.

The Aetna Medicare Platinum (PPO) plan has an enhanced alternative drug benefit. The plan has a deductible of $590. After the deductible, you will pay the following costs for your prescriptions. For preferred generic drugs, you will pay no copay at preferred pharmacies and preferred mail order. For standard generic drugs, you will pay 24% coinsurance. For preferred brand and non-preferred drugs, you will pay 25% coinsurance. Once your total drug costs reach $2000, you enter the catastrophic coverage phase where you pay nothing for covered drugs.

Inpatient Hospital See details

Inpatient Hospital benefits are covered, including Inpatient Hospital-Acute and Inpatient Hospital Psychiatric. For Inpatient Hospital-Acute, you pay a $250 copay for days 1-5, and no copay for days 6-90. For Inpatient Hospital Psychiatric, you pay a $250 copay for days 1-5, and no copay for days 6-90. Partial Hospitalization See details

Preventive Services See details

The Aetna Medicare Platinum (PPO) plan covers preventive services, including an annual physical exam with no copay. Additional preventive services are covered, including health education, wigs for hair loss related to chemotherapy, nutritional/dietary benefits, additional sessions of smoking and tobacco cessation counseling, fitness benefits, glaucoma screening, diabetes self-management training, barium enemas, digital rectal exams, and EKG following Welcome Visit, all with no copay. Kidney disease education services have a 20% coinsurance.

Hearing Services See details

The Aetna Medicare Platinum (PPO) plan covers hearing exams, routine hearing exams, and fitting/evaluation for hearing aids with no copay, and prescription hearing aids with a copay up to $1700 per year, but does not cover prescription hearing aids - inner ear, outer ear, or over the ear, or OTC hearing aids. Routine hearing exams and fitting/evaluation for hearing aids are limited to one visit per year, and prescription hearing aids are limited to two visits per year.

Vision Services See details

Vision services, including eye exams and eyewear, are covered by the Aetna Medicare Platinum (PPO) plan. Eye exams and eyewear services have no copay, and eyewear has a combined maximum benefit of $200 per year.

Dental Services See details

The Aetna Medicare Platinum (PPO) plan covers dental services with a $1,250 annual maximum. Home Infusion bundled Services are covered by the Aetna Medicare Platinum (PPO) plan. Medicare Part B Insulin Drugs have a $35 copay, while Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs have a coinsurance between 0% and 20%.

Medical Equipment See details
