Monthly Premium
The Monthly Premium for this plan is $0.00. This is the amount you must pay every month. Additionally, this plan comes with a Part B Premium reduction of $25.00. You must continue to pay paying your reduced Part B Premium.
Deductibles
This plan does not have a health deductible. Your insurance coverage on covered health services will start immediately.
Drugs are not covered by this plan, so a prescription drug deductible is not applicable.
Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $9500.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $9500.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

Prescription drugs are not covered by Aetna Medicare Eagle Plus (PPO).
The Aetna Medicare Eagle Plus (PPO) plan offers comprehensive coverage, including inpatient hospital stays with a $430 copay for the first four days, and no copay for days 5-90. The plan also covers a wide range of outpatient services, primary care, preventive services, hearing, vision, and dental services, often with no copay. Additional benefits include coverage for ambulance services, emergency services, and medical equipment. The plan also covers home health services, skilled nursing facility stays, and over-the-counter items.
The Aetna Medicare Eagle Plus (PPO) plan covers inpatient hospital stays with a copay of $430 for days 1-4, and no copay for days 5-90. Additional days for inpatient hospital-acute are covered with no copay, while non-Medicare-covered stays and upgrades for inpatient hospital-acute are not covered. Inpatient Hospital Psychiatric has a copay of $430 for days 1-4, and no copay for days 5-90, while additional days and non-Medicare-covered stays are not covered.
Outpatient Services include coverage for all outpatient hospital services with a copay ranging from $0 to $350, observation services with a $430 copay, ambulatory surgical center services with no copay, and outpatient substance abuse services with a $40 copay for individual and group sessions. Outpatient blood services are covered with no copay.
Partial Hospitalization is covered by the Aetna Medicare Eagle Plus (PPO) plan, with a $60 copay. Prior authorization is required for this benefit.
Ambulance and Transportation Services are covered by the Aetna Medicare Eagle Plus (PPO) plan, including ground ambulance services with a $265 copay, and air ambulance services with 20% coinsurance. Transportation services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered by the Aetna Medicare Eagle Plus (PPO) plan. Emergency Services have a $125 copay, Urgently Needed Services have a $40 copay, and Worldwide Emergency Coverage, Worldwide Urgent Coverage, and Worldwide Emergency Transportation have copays of $125, $125, and $265, respectively. There is no coinsurance for any of these services.
Primary Care benefits include coverage for Primary Care Physician Services, Chiropractic Services, Occupational Therapy Services, Physician Specialist Services, Mental Health Specialty Services, Other Health Care Professional, Psychiatric Services, Physical Therapy and Speech-Language Pathology Services, Additional Telehealth Benefits, and Opioid Treatment Program Services. Primary Care Physician Services and Physician Specialist Services have no copay. Chiropractic Services have a $15 copay, and Routine Care is not covered. Occupational Therapy Services, Individual Sessions for Mental Health Specialty Services, Group Sessions for Mental Health Specialty Services, Individual Sessions for Psychiatric Services, Group Sessions for Psychiatric Services, and Opioid Treatment Program Services have a $40 copay. Physical Therapy and Speech-Language Pathology Services have a $20 copay. Additional Telehealth Benefits have a 20% coinsurance and a copay between $0 and $40. Other Health Care Professional services have a copay between $0 and $40.
The Aetna Medicare Eagle Plus (PPO) plan covers preventive services, including an annual physical exam with no copay. Additional preventive services like Health Education, Wigs for Hair Loss Related to Chemotherapy, Additional Sessions of Smoking and Tobacco Cessation Counseling, Remote Access Technologies, and Fitness Benefit are covered, with no copay for Health Education, Wigs for Hair Loss Related to Chemotherapy, Additional Sessions of Smoking and Tobacco Cessation Counseling, Remote Access Technologies, or Fitness Benefit. Kidney Disease Education Services are covered with 20% coinsurance. Other preventive services like Glaucoma Screening, Diabetes Self-Management Training, Barium Enemas, Digital Rectal Exams, and EKG following Welcome Visit are covered with no copay.
Hearing exams, routine hearing exams, and fitting/evaluation for hearing aids are covered with no copay. Prescription hearing aids (all types) are covered, with a maximum plan benefit of $1250.00 per year, and no copay.
Vision services include eye exams and eyewear. Eye exams, including routine eye exams and other eye exam services, have no copay. Eyewear, including contact lenses, eyeglasses (lenses and frames), eyeglass lenses, eyeglass frames, and upgrades, also have no copay. There is a combined maximum plan benefit of $300 per year for eyewear.
Dental Services are covered, with a $3,000 maximum benefit per year. Medicare Dental Services have a $40 copay, while Oral Exams, Dental X-Rays, Other Diagnostic Dental Services, Prophylaxis (Cleaning), Fluoride Treatment, Other Preventive Dental Services, Restorative Services, Adjunctive General Services, Endodontics, Periodontics, Prosthodontics (removable and fixed), and Oral and Maxillofacial Surgery have no copay. Maxillofacial Prosthetics, Implant Services, and Orthodontics are not covered.
Home Infusion bundled Services are covered, with prior authorization required. Medicare Part B Insulin Drugs have a $35 copay. Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs have a coinsurance between 0% and 20%.
Dialysis Services are covered under the Aetna Medicare Eagle Plus (PPO) plan, but require prior authorization. You will pay 20% coinsurance for these services.
Aetna Medicare Eagle Plus (PPO) covers medical equipment, including Durable Medical Equipment (DME) with a coinsurance between 0% and 20%, and Prosthetics/Medical Supplies with a coinsurance for Medicare-covered devices and supplies, and Diabetic Equipment, which has a coinsurance for diabetic therapeutic shoes or inserts and a copay for Medicare-covered diabetes supplies. Durable Medical Equipment for use outside the home is not covered.
Diagnostic and Radiological Services are covered under the Aetna Medicare Eagle Plus (PPO) plan. Diagnostic Procedures/Tests have a copay between $0 and $10, while Lab Services have no copay. Diagnostic Radiological Services have a copay of up to $150, and Therapeutic Radiological Services have a coinsurance of at least 20%. Outpatient X-Ray Services have no copay.
Home Health Services are covered by the Aetna Medicare Eagle Plus (PPO) plan with no copay and no coinsurance. Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are not covered by the Aetna Medicare Eagle Plus (PPO) plan. The plan does not cover Medicare-covered Intensive Cardiac Rehabilitation Services, Medicare-covered Pulmonary Rehabilitation Services, Medicare-covered Supervised Exercise Therapy (SET) for Symptomatic Peripheral Artery Disease (PAD) Services, or Additional Cardiac Rehabilitation Services.
Skilled Nursing Facility (SNF) services are covered by the Aetna Medicare Eagle Plus (PPO) plan, but require prior authorization. You will have no copay for days 1-20, and a $203 copay per day for days 21-100. Additional days beyond Medicare-covered and non-Medicare-covered stays for SNF are not covered.
The Aetna Medicare Eagle Plus (PPO) plan covers Over-the-Counter (OTC) Items with no copay, up to a maximum of $75 every three months. Other services like acupuncture, meal benefits, and various other services are not covered.
