Specialist Visits:

Visits to specialists are covered and will have a copay of $0.00 - $40.00 and coinsurance of 0% (no coinsurance). Specialist visits may require a referral from your primary care doctor or prior authorization.

Emergency Room:

Trips to the Emergency Room are covered, and will have a copay of $125.00 and coinsurance of 0% (no coinsurance). Coverage may vary for in-network and out-of-network hospitals.

Urgent Care:

Hearing Services See details

Hearing Services include hearing exams, routine hearing exams, fitting/evaluation for hearing aids, and prescription hearing aids. Hearing exams, routine hearing exams, and fitting/evaluation for hearing aids have no copay. Prescription hearing aids have a maximum copay of $1700 and are limited to 2 per year, but hearing aids for the inner ear, outer ear, and over the ear are not covered. OTC hearing aids are not covered.

Vision Services See details

The Aetna Medicare Eagle (PPO) plan offers vision services including eye exams and eyewear, with no copay for eye exams, contact lenses, eyeglasses (lenses and frames), eyeglass lenses, eyeglass frames, and upgrades. There is a combined maximum of $300 per year for eyewear.

Dental Services See details

Dental services include a $35 copay for Medicare dental services, and no copay for oral exams, dental x-rays, other diagnostic dental services, prophylaxis (cleaning), fluoride treatment, other preventive dental services, restorative services, adjunctive general services, endodontics, periodontics, prosthodontics (removable and fixed), and oral and maxillofacial surgery; however, maxillofacial prosthetics, implant services, and orthodontics are not covered, and there is a $2,000 maximum benefit per year for all services.

Home Infusion bundled Services are covered, with prior authorization required. Medicare Part B Insulin Drugs have a $35 copay, while Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs have a coinsurance between 0% and 20%.
