This plan does not have a health deductible. Your insurance coverage on covered health services will start immediately.
This plan has a $250.00 drug deductible. You will need to pay this amount towards covered prescriptions before your insurance coverage for prescription medications kicks in.
Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $8950.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $8950.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

The Aetna Medicare Value (PPO) plan has a $250 deductible for prescription drugs. After the deductible, you will pay a copay or coinsurance depending on the drug tier and pharmacy. For preferred generic drugs, you'll pay a $10 copay at preferred pharmacies and $12 at standard pharmacies. Standard generic drugs have a 25% coinsurance, while preferred brand drugs have a 26% coinsurance. Non-preferred drugs have a 30% coinsurance. Once your total drug costs reach $2000, you enter the catastrophic coverage phase, where you pay nothing for covered drugs.
The Aetna Medicare Value (PPO) plan offers a wide array of benefits. You'll have a $250 copay for inpatient hospital stays for days 1-8, with no copay for days 9-90, and outpatient services have copays that range from $0 to $300. Preventive services, including annual physical exams, are covered with no copay, and there are also no copays for many other services like vision, dental, and home health. The plan also includes coverage for hearing services, ambulance, and emergency services.
Inpatient Hospital benefits are covered, including Inpatient Hospital-Acute and Inpatient Hospital Psychiatric, both with a $250 copay for days 1-8 and no copay for days 9-90. Additional days for Inpatient Hospital-Acute are covered with no copay, while non-Medicare-covered stays and upgrades for Inpatient Hospital-Acute, along with additional days and non-Medicare-covered stays for Inpatient Hospital Psychiatric, are not covered.
Outpatient Services include coverage for all outpatient hospital services, with a copay between $0 and $300, and observation services with a $250 copay. Ambulatory Surgical Center (ASC) Services and Outpatient Blood Services have no copay, and Outpatient Substance Abuse Services have a $40 copay for individual and group sessions.
Partial Hospitalization is covered by the Aetna Medicare Value (PPO) plan, with a $40 copay. Prior authorization is required.
Ambulance and Transportation Services are covered by Aetna Medicare Value (PPO). Ground and air ambulance services have a $250 copay, while transportation services to a plan-approved health-related location have no copay for up to 12 one-way trips per year. Transportation services to any other health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered by the Aetna Medicare Value (PPO) plan. Emergency Services have a $125 copay, Urgently Needed Services have a $45 copay, and Worldwide Emergency Coverage and Worldwide Urgent Coverage have a $125 copay, while Worldwide Emergency Transportation has a $250 copay.
The Aetna Medicare Value (PPO) plan covers primary care physician services with a $5 copay. Chiropractic services have a $20 copay, while occupational therapy services have a $40 copay. Physician specialist services have a copay between $0 and $35, while physical therapy and speech-language pathology services have a $40 copay. Additional telehealth benefits have a 20% coinsurance, with a copay between $0 and $45. Mental health, psychiatric services, and opioid treatment programs have a $40 copay for individual and group sessions. Podiatry services and other health care professional services have a copay between $40 and $0 to $35, respectively. Routine chiropractic care is not covered.
Preventive Services includes coverage for Medicare-covered preventive services, annual physical exams with no copay, and additional preventive services. Additional preventive services include coverage for Health Education, Additional Sessions of Smoking and Tobacco Cessation Counseling, Fitness Benefit, and Remote Access Technologies (including Web/Phone-based technologies and Nursing Hotline), all with no copay. Kidney Disease Education Services has a 20% coinsurance, and Other Preventive Services include Glaucoma Screening, Diabetes Self-Management Training, Barium Enemas, Digital Rectal Exams, and EKG following Welcome Visit, all with no copay.
Hearing exams are covered with a $40 copay, and routine hearing exams and fitting/evaluation for hearing aids have no copay. Prescription hearing aids are covered with a maximum benefit of $1500 per ear every year, and prescription hearing aids (all types) have no copay, but prescription hearing aids for the inner ear, outer ear, and over the ear are not covered. OTC hearing aids are not covered.
The Aetna Medicare Value (PPO) plan covers vision services, including eye exams with a copay between $0 and $40, and eyewear with no copay. The plan provides a combined maximum of $280 per year for eyewear.
Dental services include a $1,000 annual maximum benefit, with no copay for services such as oral exams, dental x-rays, other diagnostic services, cleaning, fluoride treatment, other preventive services, restorative services, adjunctive general services, endodontics, periodontics, prosthodontics (removable and fixed), and oral and maxillofacial surgery. Orthodontic services are covered under diagnostic and preventive dental, but maxillofacial prosthetics, implant services, and orthodontics are not covered.
Home Infusion bundled Services are covered, and require prior authorization. Medicare Part B Insulin Drugs have a $35 copay, while Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs have a coinsurance between 0% and 20%.
Dialysis Services are covered under the Aetna Medicare Value (PPO) plan, but require prior authorization. You will pay 20% coinsurance for these services.
Medical Equipment is covered by the Aetna Medicare Value (PPO) plan, with 0-20% coinsurance for Durable Medical Equipment, Prosthetic Devices, Medical Supplies, and Diabetic Supplies; Diabetic Therapeutic Shoes/Inserts have no copay and no coinsurance. Durable Medical Equipment for use outside the home is not covered.
Diagnostic and Radiological Services are covered, with prior authorization required. Diagnostic Procedures/Tests have a copay between $0 and $50, while Lab Services have no copay. Diagnostic Radiological Services have a copay of up to $200, Therapeutic Radiological Services have a coinsurance of at least 20%, and Outpatient X-Ray Services have a $5 copay.
Home Health Services are covered by the Aetna Medicare Value (PPO) plan with no copay and no coinsurance, but additional hours of care and personal care services are not covered. Authorization is required for this benefit.
Aetna Medicare Value (PPO) covers Cardiac Rehabilitation Services, but does not cover Intensive Cardiac Rehabilitation Services, Pulmonary Rehabilitation Services, or SET for PAD Services. The plan has a copay for some Cardiac and Pulmonary Rehabilitation Services, but the exact amount is not specified.
Skilled Nursing Facility (SNF) services are covered by the Aetna Medicare Value (PPO) plan. For days 1-20, there is no copay, and for days 21-100, the copay is $214. Additional days beyond Medicare-covered for SNF and Non-Medicare-covered stays for SNF are not covered.
The Aetna Medicare Value (PPO) plan covers Over-the-Counter (OTC) Items with no copay and a maximum benefit of $135 every three months.
