Once your total out-of-pocket costs reach $0.00 for in-network covered services, the plan will pay 100% of in-network covered costs for the rest of the year.

Visits to specialists are covered and will have a copay of $25.00 and coinsurance of 0% (no coinsurance). Specialist visits may require a referral from your primary care doctor or prior authorization.

Emergency Room:

Trips to the Emergency Room are covered, and will have a copay of $125.00 and coinsurance of 0% (no coinsurance). Coverage may vary for in-network and out-of-network hospitals.

Urgent Care:

Trips to Urgent Care arecovered and will have a copay of $40.00 and coinsurance of 0% (no coinsurance). Coverage may vary for in-network and out-of-network hospitals.

The Wellcare Low Premium (HMO-POS) plan has a $420 deductible for prescription drugs. In the initial coverage phase, after the deductible is met, you'll pay a copay or coinsurance depending on the drug tier and pharmacy type. For example, preferred generic drugs have no copay at preferred pharmacies and a $10 copay at standard pharmacies. For preferred brand drugs, you pay 36% coinsurance regardless of the pharmacy. Once your total drug costs reach $2,000, you enter the catastrophic coverage phase and pay nothing for Part D covered drugs.

Partial Hospitalization See details

Partial Hospitalization is covered under the Wellcare Low Premium (HMO-POS) plan, but requires prior authorization. You will have a $105 copay for this benefit.

Ambulance and Transportation Services See details

The Wellcare Low Premium (HMO-POS) plan covers hearing exams with a $25 copay, routine hearing exams with no copay, and fitting/evaluation for hearing aids with no copay. Prescription hearing aids are covered, with a maximum plan benefit of $750 per year, and two visits per year, with no copay.

Vision Services See details

The Wellcare Low Premium (HMO-POS) plan covers vision services including eye exams and eyewear. Eye exams have a copay of $0-$25, while routine eye exams have no copay. Eyewear, contact lenses, eyeglasses (lenses and frames), eyeglass lenses, eyeglass frames, and upgrades all have no copay.

Other Services includes coverage for over-the-counter items with no copay, and a meal benefit with no copay, but acupuncture, dual eligible SNPs, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered. The over-the-counter benefit has a maximum coverage amount of $72 every three months.
