Monthly Premium
The Monthly Premium for this plan is $0.00. This is the amount you must pay every month. Additionally, this plan comes with a Part B Premium reduction of $150.00. You must continue to pay paying your reduced Part B Premium.
Deductibles
This plan does not have a health deductible. Your insurance coverage on covered health services will start immediately.
This plan has no drug deductible. Your prescription medication coverage will start immediately.
Out-of-Pocket Maximums
This plan has a Maximum Out-Of-Pocket cost of $3100.00 for out-of-network services. You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $0.00 for in-network covered services, the plan will pay 100% of in-network covered costs for the rest of the year.

The Cigna Preferred Savings Medicare (HMO) plan has an enhanced alternative drug benefit. There is no deductible for prescription drugs. In the initial coverage phase, you will pay a copay for your prescriptions. For preferred generic drugs, the copay is $4.00, while standard generic drugs have a $45.00 copay. Preferred brand drugs have a $100.00 copay, and non-preferred drugs have 33% coinsurance. Once your total drug costs reach $2000.00, you enter the catastrophic coverage phase, where you pay nothing for covered drugs.
The Cigna Preferred Savings Medicare (HMO) plan offers a range of benefits with varying costs. Inpatient hospital stays have a copay, while outpatient services have copays between $0 and $85. Emergency services and worldwide emergency coverage have a $135 copay. This plan includes no copays for many services, such as routine eye exams and home health services. It also covers hearing exams and hearing aids with a copay, as well as dental services. The plan provides additional benefits, including up to $120 every three months for over-the-counter items.
Inpatient hospital services are covered by the Cigna Preferred Savings Medicare (HMO) plan. For Inpatient Hospital-Acute and Inpatient Hospital Psychiatric, you will pay a $60 copay for days 1-5, and no copay for days 6-90.
Outpatient Services are covered by the Cigna Preferred Savings Medicare (HMO) plan, including outpatient hospital services, with a copay between $0 and $85, and observation services with a copay of $85. Ambulatory Surgical Center (ASC) Services have no copay, while outpatient substance abuse services are not covered.
Partial Hospitalization is covered under the Cigna Preferred Savings Medicare (HMO) plan, but requires prior authorization. You will have a $100 copay for this benefit.
Ambulance and Transportation Services are covered by the Cigna Preferred Savings Medicare (HMO) plan. Ground ambulance services have a $200 copay, while air ambulance services have a 20% coinsurance. Transportation services to plan-approved health-related locations are covered for up to 20 one-way trips per year, but transportation to any health-related location is not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered under the Cigna Preferred Savings Medicare (HMO) plan. Emergency Services and Worldwide Emergency Coverage, Worldwide Urgent Coverage, and Worldwide Emergency Transportation have a $135 copay and no coinsurance.
The Cigna Preferred Savings Medicare (HMO) plan covers primary care physician services, occupational therapy services with a $15 copay, physician specialist services, and physical therapy and speech-language pathology services with a $15 copay. Routine chiropractic care, individual and group sessions for mental health and psychiatric services, and podiatry services are not covered. Additional telehealth benefits and opioid treatment program services are also covered.
Preventive Services are covered by the Cigna Preferred Savings Medicare (HMO) plan, including Medicare-covered preventive services, annual physical exams, health education, glaucoma screening, diabetes self-management training, barium enemas, digital rectal exams, EKG following Welcome Visit, and a fitness benefit. This plan does not cover in-home safety assessment, Personal Emergency Response System (PERS), Medical Nutrition Therapy (MNT), Post discharge In-Home Medication Reconciliation, Re-admission Prevention, wigs for hair loss related to chemotherapy, weight management programs, alternative therapies, therapeutic massage, adult day health services, nutritional/dietary benefit, home-based palliative care, in-home support services, support for caregivers of enrollees, additional sessions of smoking and tobacco cessation counseling, enhanced disease management, telemonitoring services, remote access technologies, home and bathroom safety devices and modifications, or counseling services.
Hearing services with the Cigna Preferred Savings Medicare (HMO) plan include routine hearing exams and fitting/evaluation for hearing aids, both covered once per year. Prescription hearing aids are partially covered, with a copay between $399 and $1800 for prescription hearing aids (all types), while inner ear, outer ear, and over the ear hearing aids are not covered.
The Cigna Preferred Savings Medicare (HMO) plan covers vision services including routine eye exams with no copay, and eyewear with a combined maximum benefit of $150 per year. The plan also covers contact lenses and upgrades.
The Cigna Preferred Savings Medicare (HMO) plan covers dental services, including oral exams, dental x-rays, other diagnostic dental services, prophylaxis (cleaning), fluoride treatment, other preventive dental services, restorative services, adjunctive general services, endodontics, periodontics, prosthodontics (removable and fixed), maxillofacial prosthetics, implant services, oral and maxillofacial surgery, and orthodontics. This plan has a maximum benefit of $1,850 per year for other dental services.
Home Infusion bundled Services are covered, and prior authorization is required. For Medicare Part B Insulin Drugs, there is a $35 copay with 0-20% coinsurance. For Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs, there is a 0-20% coinsurance.
Dialysis Services are covered under the Cigna Preferred Savings Medicare (HMO) plan, but require prior authorization and a doctor's referral. You will pay a 20% coinsurance for these services.
Medical Equipment benefits, including Durable Medical Equipment, Prosthetics/Medical Supplies, and Diabetic Equipment, are covered under the Cigna Preferred Savings Medicare (HMO) plan. However, Durable Medical Equipment for use outside the home, Prosthetic Devices, Medical Supplies, Diabetic Supplies, and Diabetic Therapeutic Shoes/Inserts are not covered. There is no copay or coinsurance for covered services, but authorization is required.
Diagnostic and Radiological Services includes coverage for Lab Services with no copay, Diagnostic Radiological Services with a copay of at most $50, and Therapeutic Radiological Services with a copay of at most $60. Diagnostic Procedures/Tests and Outpatient X-Ray Services are not covered.
Home Health Services are covered by the Cigna Preferred Savings Medicare (HMO) plan with no copay or coinsurance, but additional hours of care and personal care services are not covered. Prior authorization is required for this benefit.
Cardiac Rehabilitation Services are technically covered, but the Cigna Preferred Savings Medicare (HMO) plan does not cover Cardiac Rehabilitation Services, Intensive Cardiac Rehabilitation Services, Pulmonary Rehabilitation Services, or SET for PAD Services. Prior authorization and a doctor referral are required for these services.
Skilled Nursing Facility (SNF) services are covered by the Cigna Preferred Savings Medicare (HMO) plan, but require prior authorization. There is no copay for days 1-20, and a $75 copay for days 21-100; additional days beyond Medicare-covered and non-Medicare-covered stays are not covered.
Other Services offered by Cigna Preferred Savings Medicare (HMO) include Over-the-Counter (OTC) items and Meal Benefits, while acupuncture, Dual Eligible SNPs with Highly Integrated Services, and other additional services are not covered. The plan provides up to $120 every three months for OTC items, including Nicotine Replacement Therapy and Naloxone.
