Deductibles
This plan does not have a health deductible. Your insurance coverage on covered health services will start immediately.
This plan has no drug deductible. Your prescription medication coverage will start immediately.
Out-of-Pocket Maximums
This plan has a Maximum Out-Of-Pocket cost of $2200.00 for out-of-network services. You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $0.00 for in-network covered services, the plan will pay 100% of in-network covered costs for the rest of the year.

The Cigna Preferred Medicare (HMO) plan has a $0 deductible for prescription drugs. During the initial coverage phase, you will pay a copay for your prescriptions. The copays vary depending on the drug tier and whether you use a preferred or standard pharmacy. For preferred generic drugs, the copay is $4.00. For standard generic drugs, the copay is $45.00. Preferred brand drugs have a copay of $100.00. Non-preferred drugs have a 33% coinsurance.
The Cigna Preferred Medicare (HMO) plan offers comprehensive coverage with several benefits. This plan includes no copay for inpatient hospital stays, primary care, and home health services. Additionally, it provides coverage for emergency services, hearing, vision, and dental, with copays and coinsurance varying by service. This plan also offers additional benefits such as outpatient services, ambulance, and transportation services, along with coverage for partial hospitalization and skilled nursing facilities. The plan includes coverage for medical equipment, diagnostic and radiological services, and home infusion bundled services. The plan also covers preventive services with a variety of services not covered.
Inpatient Hospital benefits are covered, including Inpatient Hospital-Acute and Inpatient Hospital Psychiatric. There is no copay for a Medicare-covered stay, and additional days for Inpatient Hospital-Acute are covered. Non-Medicare-covered stays and upgrades for Inpatient Hospital-Acute, and additional days and non-Medicare-covered stays for Inpatient Hospital Psychiatric are not covered.
Outpatient Services are covered, including all outpatient hospital services, observation services, ambulatory surgical center services, and outpatient blood services, but individual and group sessions for outpatient substance abuse are not covered. Outpatient blood services include a waived three-pint deductible.
Cigna Preferred Medicare (HMO) covers partial hospitalization with a $100 copay. Prior authorization is required for this benefit.
Ambulance and Transportation Services are covered by the Cigna Preferred Medicare (HMO) plan. Ground ambulance services have a $200 copay, while air ambulance services have a 20% coinsurance. Transportation services to a plan-approved health-related location are covered for up to 50 one-way trips per year.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered under the Cigna Preferred Medicare (HMO) plan. Emergency Services, Worldwide Emergency Coverage, Worldwide Urgent Coverage, and Worldwide Emergency Transportation have a $135 copay, with no coinsurance, while Urgently Needed Services has no copay or coinsurance.
Primary Care Physician Services, Occupational Therapy Services, Physician Specialist Services, Physical Therapy and Speech-Language Pathology Services, Additional Telehealth Benefits, and Opioid Treatment Program Services are covered, with no copay and no coinsurance for Occupational Therapy Services and Physical Therapy and Speech-Language Pathology Services. Chiropractic Services, Mental Health Specialty Services, and Psychiatric Services are partially covered, with Routine Chiropractic Care, Individual Sessions for Mental Health Specialty Services, Group Sessions for Mental Health Specialty Services, Individual Sessions for Psychiatric Services, and Group Sessions for Psychiatric Services not covered. Podiatry Services are not covered.
The Cigna Preferred Medicare (HMO) plan covers preventive services, including annual physical exams and additional preventive services. Some additional preventive services are not covered, including In-Home Safety Assessment, Personal Emergency Response System (PERS), Medical Nutrition Therapy (MNT), Post discharge In-Home Medication Reconciliation, Re-admission Prevention, Wigs for Hair Loss Related to Chemotherapy, Weight Management Programs, Alternative Therapies, Therapeutic Massage, Adult Day Health Services, Nutritional/Dietary Benefit, Home-Based Palliative Care, In-Home Support Services, Support for Caregivers of Enrollees, Additional Sessions of Smoking and Tobacco Cessation Counseling, Enhanced Disease Management, Telemonitoring Services, Remote Access Technologies, Home and Bathroom Safety Devices and Modifications, and Counseling Services.
Hearing services with Cigna Preferred Medicare (HMO) include hearing exams, routine hearing exams, and fitting/evaluation for hearing aids, all covered annually with no deductible. Prescription hearing aids are covered with a copay between $399 and $1800, while inner ear, outer ear, and over the ear prescription hearing aids are not covered, and OTC hearing aids are not covered.
Vision Services includes routine eye exams covered once per year, and eyewear benefits including contact lenses, eyeglasses (lenses and frames), eyeglass lenses, eyeglass frames, and upgrades. Eyewear has a combined maximum plan benefit coverage of $300 every year.
The Cigna Preferred Medicare (HMO) plan covers a variety of dental services with a maximum benefit of $4,000 per year. The plan covers services such as oral exams, dental x-rays, other diagnostic dental services, cleanings, fluoride treatments, other preventive dental services, restorative services, and more with no copay or coinsurance.
Home Infusion bundled Services are covered, including Medicare Part B Insulin Drugs, Medicare Part B Chemotherapy/Radiation Drugs, and Other Medicare Part B Drugs. For Medicare Part B Insulin Drugs, there is a $35 copay and coinsurance between 0% and 20%; for Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs, coinsurance is between 0% and 20%.
Dialysis Services are covered under the Cigna Preferred Medicare (HMO) plan, but require prior authorization and a doctor's referral. You will pay 20% coinsurance for these services.
Medical Equipment benefits include Durable Medical Equipment (DME) with 20% coinsurance, Prosthetics/Medical Supplies - Non-Medicare benefit with 20% coinsurance, and Diabetic Therapeutic Shoes/Inserts with 20% coinsurance. Durable Medical Equipment for use outside the home and Diabetic Supplies are not covered.
Diagnostic and Radiological Services include coverage for Diagnostic Procedures/Tests with a copay between $0 and $25, and Lab Services with no copay. Therapeutic Radiological Services are covered with at most 20% coinsurance, and Diagnostic Radiological Services have a copay of at most $100. Outpatient X-Ray Services are not covered.
Home Health Services are covered by the Cigna Preferred Medicare (HMO) plan with no copay and no coinsurance, but authorization is required. Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are covered by the Cigna Preferred Medicare (HMO) plan, but the plan does not cover the sub-services: Cardiac Rehabilitation Services, Intensive Cardiac Rehabilitation Services, Pulmonary Rehabilitation Services, and SET for PAD Services. A doctor referral and prior authorization are required.
Skilled Nursing Facility (SNF) services are covered by Cigna Preferred Medicare (HMO), but require prior authorization. For days 1-20, there is no copay, and for days 21-100, the copay is $50.
Under the Cigna Preferred Medicare (HMO) plan, "Other Services" includes Over-the-Counter (OTC) Items with a maximum benefit of $225 every three months, as well as a meal benefit, but acupuncture, Dual Eligible SNPs with Highly Integrated Services, and several other services are not covered. Nicotine Replacement Therapy (NRT) and Naloxone coverage are offered as OTC benefits.
