The AARP Medicare Advantage from UHC NC-0008 (HMO-POS) plan has a $340 deductible for prescription drugs. After meeting the deductible, you will pay a copay or coinsurance depending on the drug tier and pharmacy used. For example, you'll pay a $10 copay for standard generic drugs at a standard pharmacy. For preferred brand drugs, the copay is $100 regardless of the pharmacy. Non-preferred drugs have a 29% coinsurance. Once your total drug costs reach $2000, you enter the catastrophic coverage phase where you pay nothing for covered drugs.
The AARP Medicare Advantage from UHC NC-0008 (HMO-POS) plan offers a wide range of benefits. The plan covers inpatient hospital stays with a copay, and outpatient services such as hospital services, substance abuse, and blood services, with varying copays. Outpatient substance abuse services are covered with a copay between $0 and $25 for individual sessions and a $15 copay for group sessions. Outpatient blood services are covered with no copay.
Partial Hospitalization is covered by the AARP Medicare Advantage from UHC NC-0008 (HMO-POS) plan with a $55 copay, and prior authorization is required.
Ambulance and Transportation Services are covered by AARP Medicare Advantage from UHC NC-0008 (HMO-POS). Ground and air ambulance services have a $200 copay, with no coinsurance, while transportation services to any health-related location are not covered.
Emergency services are covered by the AARP Medicare Advantage from UHC NC-0008 (HMO-POS) plan, with a $125 copay and no coinsurance, though the copay is waived if admitted to the hospital within 24 hours. Other services, such as Health Education, In-Home Safety Assessment, Personal Emergency Response System, Medical Nutrition Therapy, and more, are not covered.
Hearing exams are covered with no copay, but routine hearing exams are limited to one per year. Prescription hearing aids are covered, with a copay between $199 and $1249, and OTC hearing aids are covered with a copay between $99 and $829. Fitting/Evaluation for Hearing Aid, Prescription Hearing Aids - Inner Ear, Prescription Hearing Aids - Outer Ear, and Prescription Hearing Aids - Over the Ear are not covered.
Vision services include eye exams with no copay, routine eye exams with no copay, and eyewear benefits with no copay for contact lenses, eyeglass frames, and eyeglass lenses with a copay ranging from $0 to $153. Eyeglasses (lenses and frames) and upgrades are not covered.
Dental Services include 20% coinsurance for Medicare dental services, with prior authorization required. Oral exams, dental x-rays, prophylaxis (cleaning), fluoride treatment, and other preventive dental services are covered with no copay, while orthodontic, restorative, and other dental services are not covered.
Home Infusion bundled Services are covered, and require prior authorization. The plan has a $35 copay for Medicare Part B Insulin Drugs, with a coinsurance between 0% and 20%. Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs are covered with a coinsurance between 0% and 20%.
Dialysis Services are covered by the AARP Medicare Advantage from UHC NC-0008 (HMO-POS) plan, with a coinsurance of 20%. Prior authorization is required.
Medical Equipment benefits are covered, including Durable Medical Equipment, Prosthetics/Medical Supplies, and Diabetic Equipment. For Durable Medical Equipment and Prosthetic Devices, you will pay 20% coinsurance. For Diabetic Supplies, there is no copay. For Diabetic Therapeutic Shoes/Inserts, you will pay 20% coinsurance. Durable Medical Equipment for use outside the home is not covered.
Diagnostic and Radiological Services are covered, including diagnostic procedures and tests with a $50 copay, and lab services with no copay. Outpatient X-ray services have a $20 copay, while diagnostic radiological services have a copay of at most $225, and therapeutic radiological services have a coinsurance of at least 20%.
Home Health Services are covered by the AARP Medicare Advantage from UHC NC-0008 (HMO-POS) plan with no copay and no coinsurance, but additional hours of care and personal care services are not covered.
Cardiac Rehabilitation Services are covered, but the specific services are not covered. Prior authorization is required for this benefit.
Skilled Nursing Facility (SNF) services are covered with prior authorization. You will pay no copay for days 1-20, and a $203 copay for days 21-100, with no coinsurance.
Other Services includes coverage for Over-the-Counter (OTC) Items and Meal Benefits, with no copay for either. Acupuncture, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered.
