Once your total out-of-pocket costs reach $3900.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

The AARP Medicare Advantage from UHC NC-0021 (HMO-POS) plan has a $255 deductible for prescription drugs. After the deductible, you will pay a copay or coinsurance depending on the drug tier and pharmacy. For generic drugs at a standard pharmacy, you'll pay a $5 copay for preferred generics and a $47 copay for standard generics. For preferred brand drugs, you'll pay a $100 copay. For non-preferred drugs, you will pay 30% coinsurance. Once your total drug costs reach $2000, you will enter the catastrophic coverage phase where you pay nothing for covered drugs.
The AARP Medicare Advantage from UHC NC-0021 (HMO-POS) plan offers a range of benefits. This plan includes coverage for inpatient hospital stays with a $300 copay for the first five days, and no copay for days 6-90. It also provides coverage for outpatient services, with varying copays depending on the service. The plan covers emergency services with a $140 copay, and primary care with no copay. Partial Hospitalization is covered by the AARP Medicare Advantage from UHC NC-0021 (HMO-POS) plan, with a $55 copay. Prior authorization is required.
Ambulance and Transportation Services are covered by the AARP Medicare Advantage from UHC NC-0021 (HMO-POS) plan. Ground and Air Ambulance Services have a $275 copay, with no coinsurance. Transportation Services to any health-related location are not covered by this plan.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered. Emergency Services have a $140 copay, while Urgently Needed Services have a copay between $0 and $65. Some preventive services, such as Health Education, In-Home Safety Assessment, Personal Emergency Response System, Medical Nutrition Therapy, Post discharge In-Home Medication Reconciliation, Re-admission Prevention, Wigs for Hair Loss Related to Chemotherapy, Weight Management Programs, Alternative Therapies, Therapeutic Massage, Adult Day Health Services, Nutritional/Dietary Benefit, Home-Based Palliative Care, In-Home Support Services, Support for Caregivers of Enrollees, Additional Sessions of Smoking and Tobacco Cessation Counseling, Enhanced Disease Management, Telemonitoring Services, Remote Access Technologies, and Counseling Services are not covered.
Hearing exams are covered with no copay, while fitting/evaluation for hearing aids are not covered. Prescription hearing aids are covered with a copay between $199 and $1249, while OTC hearing aids have a copay between $99 and $829.
Vision services include eye exams and eyewear. Eye exams have no copay. Eyewear includes contact lenses, eyeglass lenses, and eyeglass frames, all with no copay, and a combined maximum plan benefit of $300 every two years. Eyeglasses (lenses and frames) and upgrades are not covered.
Dental Services include coverage for Medicare Dental Services with 20% coinsurance, Oral Exams, Dental X-Rays, Other Diagnostic Dental Services, Prophylaxis (Cleaning), Fluoride Treatment, and Other Preventive Dental Services with no copay. Restorative Services, Adjunctive General Services, Endodontics, Periodontics, Prosthodontics (removable and fixed), Maxillofacial Prosthetics, and Oral and Maxillofacial Surgery are covered with no copay, but have varying coinsurance from 0% to 50%. Orthodontic Services are covered under Diagnostic and Preventive Dental, while Implant and Orthodontics are not covered.
Home Infusion bundled Services are covered, including Medicare Part B Insulin Drugs, Medicare Part B Chemotherapy/Radiation Drugs, and Other Medicare Part B Drugs. For Medicare Part B Insulin Drugs, there is a $35 copay and a coinsurance between 0% and 20%.
Dialysis Services are covered, but require prior authorization. You will pay 20% coinsurance for these services.
Medical Equipment is covered, including Durable Medical Equipment with 20% coinsurance, Prosthetic Devices with 20% coinsurance, and Medical Supplies with 20% coinsurance. Diabetic Supplies have no copay, and Diabetic Therapeutic Shoes/Inserts have 20% coinsurance.
Diagnostic and Radiological Services include coverage for all diagnostic services, with a $40 copay for Diagnostic Procedures/Tests, and no copay for Lab Services. Diagnostic Radiological Services have a maximum copay of $125, while Therapeutic Radiological Services have a minimum coinsurance of 20%, and Outpatient X-Ray Services have a $25 copay.
Home Health Services are covered by the AARP Medicare Advantage from UHC NC-0021 (HMO-POS) plan with no copay and no coinsurance. Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are covered, but the plan does not cover the following services: Cardiac Rehabilitation Services, Intensive Cardiac Rehabilitation Services, Pulmonary Rehabilitation Services, and SET for PAD Services. Prior authorization is required.
Skilled Nursing Facility (SNF) services are covered by AARP Medicare Advantage from UHC NC-0021 (HMO-POS). There is no copay for days 1-20, and a $203 copay for days 21-100.
Other Services for AARP Medicare Advantage from UHC NC-0021 (HMO-POS) includes Over-the-Counter (OTC) Items and Meal Benefits, with OTC items having no copay. Acupuncture, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered.
