The Humana USAA Honor Giveback with Rx (PPO) plan has a $350 deductible for prescription drugs. After the deductible, you will pay a copay or coinsurance depending on the drug tier and pharmacy. For preferred generic drugs, you'll pay a $10 copay at preferred pharmacies and mail order, and a $20 copay at standard pharmacies. For standard generic drugs, the copay is $47. Preferred and non-preferred brand drugs have a 45% and 28% coinsurance, respectively. After your total drug costs reach $2000, you pay nothing for Part D covered drugs.

Prior authorization is required for some of these services.
Partial Hospitalization is covered by the Humana USAA Honor Giveback with Rx (PPO) plan with a $55 copay, and prior authorization is required.
Ambulance and Transportation Services are covered by the Humana USAA Honor Giveback with Rx (PPO) plan. Both Medicare-covered ground and air ambulance services have a $315 copay, with no coinsurance. Transportation services to any health-related location are not covered.
Emergency Services are covered under the Humana USAA Honor Giveback with Rx (PPO) plan. Emergency Services have a $110 copay, while Urgently Needed Services have a $45 copay, and there is no coinsurance for either service. Worldwide Emergency Services, including Worldwide Emergency Coverage, Worldwide Urgent Coverage, and Worldwide Emergency Transportation, are covered with a $110 copay.

Prescription hearing aids are partially covered, with a copay between $699 and $999 for prescription hearing aids of all types, but not for inner ear, outer ear, or over the ear hearing aids. OTC hearing aids are not covered.
The Humana USAA Honor Giveback with Rx (PPO) plan covers vision services, including routine eye exams with a copay between $0 and $50. Eyewear is covered with no copay, and contact lenses and eyeglasses are covered with no copay, up to a combined maximum of $300 per year. Eyeglass lenses, eyeglass frames, and upgrades are not covered.
Dental Services include coverage for Medicare Dental Services with a $50 copay, oral exams, dental x-rays, other diagnostic dental services, prophylaxis (cleaning), other preventive dental services, restorative services, adjunctive general services, endodontics, periodontics, and prosthodontics fixed, and oral and maxillofacial surgery with no copay. Fluoride treatment, prosthodontics removable, maxillofacial prosthetics, implant services, and orthodontics are not covered. There is a maximum plan benefit coverage of $1,000 per year for both in-network and out-of-network services.
Home Infusion bundled Services are covered, including Medicare Part B Insulin Drugs, Medicare Part B Chemotherapy/Radiation Drugs, and Other Medicare Part B Drugs. The plan has a $35 copay for Medicare Part B Insulin Drugs, with a coinsurance between 0-20% for all other drugs.
Dialysis Services are covered by the Humana USAA Honor Giveback with Rx (PPO) plan, but require prior authorization. The coinsurance for these services is 20%.
Medical Equipment is covered, including Durable Medical Equipment (DME) with 20% coinsurance, Prosthetics/Medical Supplies with 20% coinsurance, and Diabetic Equipment. Diabetic Supplies have a 10-20% coinsurance with no copay, while Diabetic Therapeutic Shoes/Inserts have a $10 copay.
Diagnostic and Radiological Services include coverage for all diagnostic services, with a copay for Medicare-covered diagnostic procedures/tests and lab services, and a coinsurance for all diagnostic services. Outpatient X-Ray services have no copay.
Home Health Services are covered by the Humana USAA Honor Giveback with Rx (PPO) plan with no copay and no coinsurance. Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are covered, but the plan does not cover the Intensive Cardiac Rehabilitation Services, Pulmonary Rehabilitation Services, or SET for PAD Services. Prior authorization is required for this benefit, and copays apply.
Skilled Nursing Facility (SNF) services are covered by the Humana USAA Honor Giveback with Rx (PPO) plan, with prior authorization required. For days 1-20, there is no copay, and for days 21-100, the copay is $214.
Other Services include acupuncture and a meal benefit. Acupuncture has a $50 copay and a limit of 20 treatments per year, while the meal benefit has no copay.
