Monthly Premium
The Monthly Premium for this plan is $0.00. This is the amount you must pay every month. Additionally, this plan comes with a Part B Premium reduction of $30.00. You must continue to pay paying your reduced Part B Premium.
Deductibles
This plan has a $100.00 health deductible. This means, every calendar year, you pay this amount towards covered services before your insurance coverage kicks in.
Drugs are not covered by this plan, so a prescription drug deductible is not applicable.
Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $6000.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $6000.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

Prescription drugs are not covered by Humana USAA Honor Giveback (PPO).
The Humana USAA Honor Giveback (PPO) plan offers a range of benefits with varying costs. Inpatient hospital stays have a $450 copay for the first five days, and no copay thereafter. Outpatient services have a mix of copays and coinsurance, while emergency services have a $125 copay. The plan also includes coverage for primary care with no copay, along with some specialist visits and therapy services with copays. Preventive services, including an annual physical, are covered with no copay, and hearing and vision services are also covered. Dental services have a $40 copay for Medicare dental services, and a $2,500 maximum benefit for other dental services.
Inpatient Hospital benefits, including Inpatient Hospital-Acute and Inpatient Hospital Psychiatric, are covered. For Inpatient Hospital-Acute, you'll pay a $450 copay for days 1-5, and no copay for days 6-90, with additional days (91-999) having no copay; however, Non-Medicare-covered Stay and Upgrades are not covered. For Inpatient Hospital Psychiatric, you'll pay a $450 copay for days 1-5, and no copay for days 6-90, but Additional Days and Non-Medicare-covered Stay are not covered.
Outpatient Services include coverage for all outpatient hospital services, with a 20% coinsurance and a copay between $0 and $450, and observation services with a $450 copay. Ambulatory Surgical Center (ASC) Services and outpatient blood services have no copay, and outpatient substance abuse services have a minimum 20% coinsurance and a minimum $40 copay.
Partial Hospitalization is covered under the Humana USAA Honor Giveback (PPO) plan with an $85 copay. Prior authorization is required for this benefit.
Ambulance and Transportation Services are covered by the Humana USAA Honor Giveback (PPO) plan. Ground Ambulance Services have a $315 copay, while Air Ambulance Services have a $1250 copay; there is no coinsurance for these services. Transportation Services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered by the Humana USAA Honor Giveback (PPO) plan. Emergency Services and Worldwide Emergency Coverage have a $125 copay, Urgently Needed Services have a $55 copay, and Worldwide Urgent Coverage and Worldwide Emergency Transportation also have a $125 copay.
The Humana USAA Honor Giveback (PPO) plan covers primary care physician services with no copay, chiropractic services with a $20 copay, occupational therapy services with a $25 copay, physician specialist services with a $40 copay, and physical therapy and speech-language pathology services with a $25 copay. Mental health specialty services and psychiatric services have varying copays depending on the service. Additional telehealth benefits have a copay between $0 and $55, and opioid treatment program services have a 20% coinsurance and a $40 copay. Routine chiropractic care and podiatry services are not covered.
The Humana USAA Honor Giveback (PPO) plan covers preventive services, including an annual physical exam with no copay. The plan also covers additional preventive services, kidney disease education services, and other preventive services, with no copay for glaucoma screening, diabetes self-management training, barium enemas, digital rectal exams, and EKG following a Welcome Visit. Some preventive services, such as health education, in-home safety assessments, and others, are not covered.
Hearing exams are covered by the Humana USAA Honor Giveback (PPO) plan with a $40 copay, and routine hearing exams have no copay. Prescription hearing aids are partially covered, with a copay between $699 and $999 for prescription hearing aids of all types, but no coverage for inner ear, outer ear, or over the ear hearing aids. OTC hearing aids are not covered.
The Humana USAA Honor Giveback (PPO) plan covers vision services, including eye exams with a copay of $0-$40, and eyewear with no copay. Eyeglass lenses, eyeglass frames, and upgrades are not covered.
Dental services include coverage for Medicare dental services with a $40 copay, and other dental services with a $2,500 maximum benefit per year. Oral exams, dental x-rays, other diagnostic dental services, prophylaxis (cleaning), other preventive dental services, restorative services, adjunctive general services, endodontics, periodontics, prosthodontics (removable and fixed), and oral and maxillofacial surgery are covered with no copay. Fluoride treatment, maxillofacial prosthetics, implant services, and orthodontics are not covered.
Home Infusion bundled Services are covered by the Humana USAA Honor Giveback (PPO) plan, with a $35 copay for Medicare Part B Insulin Drugs and coinsurance between 0% and 20% for other covered drugs. Prior authorization is required for this benefit.
Dialysis Services are covered under the Humana USAA Honor Giveback (PPO) plan, with prior authorization required. You will pay 20% coinsurance for these services.
Medical Equipment is covered by the Humana USAA Honor Giveback (PPO) plan, including Durable Medical Equipment (DME) with 15% coinsurance, Prosthetics/Medical Supplies with 20% coinsurance, and Diabetic Equipment with varying copays and coinsurance depending on the service. Durable Medical Equipment for use outside the home is not covered.
Diagnostic and Radiological Services include coverage for all diagnostic services with a copay, and lab services with no copay. Diagnostic Procedures/Tests have a maximum copay of $55.00, while Diagnostic Radiological Services have a maximum copay of $450.00. Therapeutic Radiological Services have a minimum coinsurance of 20%, and Outpatient X-Ray Services have no copay.
Home Health Services are covered by the Humana USAA Honor Giveback (PPO) plan with no copay and no coinsurance. Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are not covered by the Humana USAA Honor Giveback (PPO) plan. Prior authorization is required.
Skilled Nursing Facility (SNF) services are covered by the Humana USAA Honor Giveback (PPO) plan, but prior authorization is required. For days 1-20, there is a $10 copay; for days 21-50, the copay is $214; and for days 51-100, there is no copay.
Other Services include acupuncture, with a $40 copay and a limit of 20 treatments per year, and a meal benefit with no copay; however, over-the-counter items, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered.
