Monthly Premium
The Monthly Premium for this plan is $0.00. This is the amount you must pay every month. Additionally, this plan comes with a Part B Premium reduction of $30.00. You must continue to pay paying your reduced Part B Premium.
Deductibles
This plan has a $100.00 health deductible. This means, every calendar year, you pay this amount towards covered services before your insurance coverage kicks in.
Drugs are not covered by this plan, so a prescription drug deductible is not applicable.
Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $6000.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $6000.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

Prescription drugs are not covered by Humana USAA Honor Giveback (PPO).
The Humana USAA Honor Giveback (PPO) plan offers a range of benefits with varying costs. Inpatient hospital stays have copays, while outpatient services have copays and coinsurance depending on the service. Emergency services have copays, and primary care visits are covered with no copay. Preventive services, routine eye exams, and many dental services have no copay. The plan also covers hearing exams, and offers coverage for durable medical equipment, with coinsurance applying to some services.
Inpatient Hospital benefits include coverage for Inpatient Hospital-Acute and Inpatient Hospital Psychiatric services. For the first 5 days of an Inpatient Hospital-Acute stay, there is a $425 copay, and days 6-90 have no copay, while Inpatient Hospital Psychiatric has the same cost structure. Additional days for Inpatient Hospital-Acute have no copay. Non-Medicare-covered stays and upgrades for Inpatient Hospital-Acute and Additional days and Non-Medicare-covered stays for Inpatient Hospital Psychiatric are not covered.
Outpatient Services include outpatient hospital services with a 20% coinsurance and a copay between $0 and $400, observation services with a $425 copay, ambulatory surgical center services with no copay, outpatient substance abuse services with a 20% coinsurance and a copay between $50 and $50, and outpatient blood services with no copay.
Partial Hospitalization is covered under the Humana USAA Honor Giveback (PPO) plan with prior authorization required, and a copay of $85.
Ambulance and Transportation Services are covered by the Humana USAA Honor Giveback (PPO) plan. Ground ambulance services have a $315 copay, while air ambulance services have a $1250 copay; there is no coinsurance for either. Transportation Services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered under the Humana USAA Honor Giveback (PPO) plan. Emergency Services and Worldwide Emergency Coverage have a $125 copay, while Urgently Needed Services have a $55 copay, and there is no coinsurance for any of these services.
Primary Care Physician Services are covered with no copay, while Chiropractic Services require a $20 copay. Occupational Therapy Services have a $25 copay, and Physician Specialist Services require a $50 copay. Mental Health Specialty Services, including individual and group sessions, have no copay. Other Health Care Professional services have a copay ranging from $0 to $50. Psychiatric Services, including individual and group sessions, have no copay. Physical Therapy and Speech-Language Pathology Services have a $25 copay. Additional Telehealth Benefits have a copay between $0 and $55. Opioid Treatment Program Services have a 20% coinsurance and a copay between $50 and $50.
Preventive Services include Medicare-covered services, an annual physical exam with no copay, and additional services. Glaucoma screening, Diabetes Self-Management Training, Barium Enemas, Digital Rectal Exams, and EKG following Welcome Visit have no copay, and Fitness Benefit has no copay. Other services such as Health Education, In-Home Safety Assessment, Personal Emergency Response System (PERS), Medical Nutrition Therapy (MNT), Post discharge In-Home Medication Reconciliation, Re-admission Prevention, Wigs for Hair Loss Related to Chemotherapy, Weight Management Programs, Alternative Therapies, Therapeutic Massage, Adult Day Health Services, Nutritional/Dietary Benefit, Home-Based Palliative Care, In-Home Support Services, Support for Caregivers of Enrollees, Additional Sessions of Smoking and Tobacco Cessation Counseling, Enhanced Disease Management, Telemonitoring Services, Remote Access Technologies (including Web/Phone-based technologies and Nursing Hotline), Home and Bathroom Safety Devices and Modifications, and Counseling Services are not covered.
Hearing Services include hearing exams with a $50 copay, routine hearing exams with no copay, and fitting/evaluation for hearing aids with no copay. Prescription Hearing Aids are partially covered, with Prescription Hearing Aids (all types) having a copay between $699 and $999, while Prescription Hearing Aids - Inner Ear, Prescription Hearing Aids - Outer Ear, and Prescription Hearing Aids - Over the Ear are not covered. Over-the-counter hearing aids are not covered.
The Humana USAA Honor Giveback (PPO) plan covers vision services, including eye exams with a copay of $0-$50 and eyewear with no copay, but eyeglass lenses, eyeglass frames, and upgrades are not covered. Routine eye exams, contact lenses, and eyeglasses (lenses and frames) have no copay.
The Humana USAA Honor Giveback (PPO) plan covers Medicare Dental Services with a $50 copay, and other dental services with a $2,000 maximum benefit per year. Oral exams, dental x-rays, other diagnostic dental services, prophylaxis (cleaning), other preventive dental services, restorative services, adjunctive general services, endodontics, periodontics, prosthodontics (removable and fixed), and oral and maxillofacial surgery are covered with no copay, but the plan does not cover fluoride treatment, maxillofacial prosthetics, implant services, and orthodontics.
Home Infusion bundled Services are covered by the Humana USAA Honor Giveback (PPO) plan, but require prior authorization. The plan covers Medicare Part B Insulin Drugs with a $35 copay and 0-20% coinsurance, and covers Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs with 0-20% coinsurance.
Dialysis Services are covered by the Humana USAA Honor Giveback (PPO) plan. The plan requires prior authorization, and has a coinsurance of 20%.
Medical Equipment benefits are covered, including Durable Medical Equipment (DME) with 15% coinsurance, Prosthetic Devices with 20% coinsurance, and Medical Supplies with 20% coinsurance. Diabetic Supplies have a coinsurance between 10% and 20%, and Diabetic Therapeutic Shoes/Inserts have no copay.
Diagnostic and Radiological Services include coverage for all diagnostic services with a copay for Medicare-covered procedures and lab services, and a copay for Medicare-covered diagnostic and therapeutic radiological services. Diagnostic Procedures/Tests have a maximum copay of $55, and Lab Services have no copay. Outpatient X-Ray Services have no copay, and Therapeutic Radiological Services have at least 20% coinsurance.
Home Health Services are covered by the Humana USAA Honor Giveback (PPO) plan with no copay and no coinsurance, but additional hours of care and personal care services are not covered.
Cardiac Rehabilitation Services are not covered by the Humana USAA Honor Giveback (PPO) plan. Prior authorization is required for this benefit.
Skilled Nursing Facility (SNF) services are covered by the Humana USAA Honor Giveback (PPO) plan. For days 1-20, there is a $10 copay, for days 21-50 there is a $214 copay, and for days 51-100, there is no copay.
Other Services include acupuncture with a $50 copay and a limit of 20 treatments per year, and a meal benefit with no copay. Over-the-counter items, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered.
