Monthly Premium
The Monthly Premium for this plan is $0.00. This is the amount you must pay every month. Additionally, this plan comes with a Part B Premium reduction of $65.00. You must continue to pay paying your reduced Part B Premium.
Deductibles
This plan does not have a health deductible. Your insurance coverage on covered health services will start immediately.
Drugs are not covered by this plan, so a prescription drug deductible is not applicable.
Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $5100.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $5100.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

Prescription drugs are not covered by Humana USAA Honor Giveback (PPO).
The Humana USAA Honor Giveback (PPO) plan offers a range of benefits with varying costs. Inpatient hospital stays have a copay, while outpatient services have a copay depending on the service. Emergency and urgent care services have a copay, and primary care visits are covered with no copay, but specialist visits have a copay. The plan also covers preventive services, hearing, vision, and dental services with copays or coinsurance. Medical equipment, home health, and skilled nursing facilities are covered with copays or coinsurance. Other covered services include ambulance, diagnostic and radiological services, and home infusion services.
Inpatient Hospital benefits, including acute and psychiatric care, are covered. For days 1-5, there is a $280 copay, and for days 6-90, there is no copay; additional days for inpatient hospital-acute have no copay.
Outpatient Services are covered by the Humana USAA Honor Giveback (PPO) plan, including outpatient hospital services, ambulatory surgical center services, outpatient substance abuse services, and outpatient blood services. Outpatient Hospital Services have a copay between $0 and $250, Observation Services have a $280 copay, Ambulatory Surgical Center Services have no copay, Individual and Group Sessions for Outpatient Substance Abuse have a $25 copay, and Outpatient Blood Services have no copay.
Partial Hospitalization is covered by the Humana USAA Honor Giveback (PPO) plan, requiring prior authorization and a $25 copay.
Ambulance and Transportation Services are covered by the Humana USAA Honor Giveback (PPO) plan, with prior authorization required for all ambulance services. Ground ambulance services have a $315 copay, while air ambulance services have a 20% coinsurance. Transportation Services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered. Emergency Services have a $140 copay and no coinsurance, while Urgently Needed Services have a $65 copay and no coinsurance. Worldwide Emergency Coverage, Worldwide Urgent Coverage, and Worldwide Emergency Transportation each have a $140 copay.
The Humana USAA Honor Giveback (PPO) plan covers primary care physician services with no copay, chiropractic services with a $20 copay, occupational therapy services with a $20 copay, physician specialist services with a $25 copay, mental health specialty services with a $25 copay, physical therapy and speech-language pathology services with a $20 copay, additional telehealth benefits with a copay between $0 and $65, and opioid treatment program services with a $25 copay. Podiatry Services are not covered.
Preventive Services include coverage for Medicare-covered preventive services, annual physical exams with no copay, and additional services including Fitness Benefit, Glaucoma Screening, Diabetes Self-Management Training, Barium Enemas, Digital Rectal Exams, and EKG following Welcome Visit, all with no copay. Health Education, In-Home Safety Assessment, Personal Emergency Response System, Medical Nutrition Therapy, Post discharge In-Home Medication Reconciliation, Re-admission Prevention, Wigs for Hair Loss Related to Chemotherapy, Weight Management Programs, Alternative Therapies, Therapeutic Massage, Adult Day Health Services, Nutritional/Dietary Benefit, Home-Based Palliative Care, In-Home Support Services, Support for Caregivers of Enrollees, Additional Sessions of Smoking and Tobacco Cessation Counseling, Enhanced Disease Management, Telemonitoring Services, Remote Access Technologies, Home and Bathroom Safety Devices and Modifications, and Counseling Services are not covered.
Hearing Services include coverage for Hearing Exams with a $25 copay, Routine Hearing Exams with no copay, and Fitting/Evaluation for Hearing Aid with no copay. Prescription Hearing Aids (all types) are covered with a copay between $399 and $699, but Prescription Hearing Aids - Inner Ear, Prescription Hearing Aids - Outer Ear, and Prescription Hearing Aids - Over the Ear are not covered, and OTC Hearing Aids are not covered.
The Humana USAA Honor Giveback (PPO) plan covers vision services, including eye exams with a copay between $0 and $25, and eyewear with no copay. Eyeglass lenses, eyeglass frames, and upgrades are not covered.
Dental services are covered, including Medicare dental services with a $25 copay, oral exams, dental X-rays, other diagnostic dental services, prophylaxis (cleaning), other preventive dental services, restorative services with 30% to 40% coinsurance, adjunctive general services, endodontics, periodontics, and prosthodontics fixed, and oral and maxillofacial surgery with no copay. Fluoride treatment, prosthodontics removable, maxillofacial prosthetics, implant services, and orthodontics are not covered. There is a $2,000 maximum plan benefit per year for both in-network and out-of-network services.
Home Infusion bundled Services are covered, with prior authorization required, and includes coverage for Medicare Part B insulin drugs with a $35 copay and 0-20% coinsurance, and Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs with 0-20% coinsurance.
Dialysis Services are covered by the Humana USAA Honor Giveback (PPO) plan, but require prior authorization. You will pay 20% coinsurance for these services.
Medical Equipment is covered by the Humana USAA Honor Giveback (PPO) plan. Durable Medical Equipment (DME) has a 15% coinsurance and requires authorization. Prosthetic Devices have a 20% coinsurance, while Medical Supplies have a 20% coinsurance. Diabetic Supplies have between 10% to 20% coinsurance with no copay, while Diabetic Therapeutic Shoes/Inserts have a $10 copay.
The Humana USAA Honor Giveback (PPO) plan covers diagnostic and radiological services. Diagnostic Procedures/Tests have a copay between $0 and $65, while Lab Services have no copay. Diagnostic Radiological Services have a copay up to $325, Therapeutic Radiological Services have a copay up to $45, and Outpatient X-Ray Services have no copay.
Home Health Services are covered by the Humana USAA Honor Giveback (PPO) plan with no copay and no coinsurance, but additional hours of care and personal care services are not covered. Prior authorization is required for this benefit.
Cardiac Rehabilitation Services are not covered by the Humana USAA Honor Giveback (PPO) plan. The plan does not cover Medicare-covered Intensive Cardiac Rehabilitation Services, Medicare-covered Pulmonary Rehabilitation Services, Medicare-covered Supervised Exercise Therapy (SET) for Symptomatic Peripheral Artery Disease (PAD) Services, or Additional Cardiac Rehabilitation Services.
Skilled Nursing Facility (SNF) services are covered by the Humana USAA Honor Giveback (PPO) plan, with a copay of $20 for days 1-20 and $214 for days 21-100. Additional days beyond Medicare-covered and non-Medicare-covered stays for SNF are not covered.
Other Services includes coverage for acupuncture with a $25 copay, limited to 20 treatments per year, and a meal benefit with no copay. Over-the-counter items, Dual Eligible SNPs, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered.
