Monthly Premium
The Monthly Premium for this plan is $0.00. This is the amount you must pay every month. Additionally, this plan comes with a Part B Premium reduction of $100.00. You must continue to pay paying your reduced Part B Premium.
Deductibles
This plan does not have a health deductible. Your insurance coverage on covered health services will start immediately.
Drugs are not covered by this plan, so a prescription drug deductible is not applicable.
Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $6700.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $6700.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

Prescription drugs are not covered by Humana USAA Honor Giveback (PPO).
The Humana USAA Honor Giveback (PPO) plan offers a range of benefits, including inpatient hospital stays with copays up to $425, and outpatient services with copays ranging from $0-$800. Preventive services, hearing exams, and vision services such as eye exams and eyewear are covered with no or low copays. The plan also provides dental services with no copay for many services and a $1,000 annual maximum, as well as home health services with no copay. Additional benefits include coverage for ambulance and transportation services with a $315 copay, and emergency services with a $125 copay. The plan covers primary care, mental health, and other health professional services with various copays. The plan also covers home infusion, dialysis, medical equipment, and diagnostic services with copays or coinsurance.
Inpatient Hospital coverage includes acute and psychiatric care, with a copay of $425 for days 1-7 and no copay for days 8-90 for acute care, and a copay of $380 for days 1-6 and no copay for days 7-90 for psychiatric care; additional days for inpatient hospital psychiatric are not covered.
Outpatient Services include coverage for outpatient hospital services with a copay between $0 and $800, observation services with a $425 copay, ambulatory surgical center services with no copay, individual and group outpatient substance abuse sessions with a copay between $40 and $85, and outpatient blood services with no copay. Prior authorization is required for all services.
Partial Hospitalization is covered by the Humana USAA Honor Giveback (PPO) plan. The plan has a $55 copay for this benefit.
Ambulance and Transportation Services include coverage for ground and air ambulance services with a $315 copay, as well as transportation services to a plan-approved health-related location with no copay for up to 24 one-way trips per year. Transportation services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered. Emergency Services has a $125 copay, Urgently Needed Services has a $55 copay, and Worldwide Emergency Coverage, Worldwide Urgent Coverage, and Worldwide Emergency Transportation all have a $125 copay.
The Humana USAA Honor Giveback (PPO) plan covers Primary Care Physician Services with a $10 copay, Chiropractic Services with a $15 copay (routine care not covered), Occupational Therapy Services with a $10-$40 copay, Physician Specialist Services with a $45 copay, and Mental Health Specialty Services with a $40 copay for individual and group sessions. This plan also covers Other Health Care Professional services with a $10-$45 copay, Psychiatric Services with a $40 copay for individual and group sessions, Physical Therapy and Speech-Language Pathology Services with a $10-$40 copay, Additional Telehealth Benefits with a $0-$55 copay, and Opioid Treatment Program Services with a $40-$85 copay.
Preventive Services include coverage for Medicare-covered services with no copay, Annual Physical Exams with no copay, and other services. Additional Preventive Services, including Fitness Benefits, Kidney Disease Education Services, Glaucoma Screening, Diabetes Self-Management Training, Barium Enemas, Digital Rectal Exams, and EKG following Welcome Visit are covered with no copay. Health Education, In-Home Safety Assessment, Personal Emergency Response System (PERS), Medical Nutrition Therapy (MNT), Post discharge In-Home Medication Reconciliation, Re-admission Prevention, Wigs for Hair Loss Related to Chemotherapy, Weight Management Programs, Alternative Therapies, Therapeutic Massage, Adult Day Health Services, Nutritional/Dietary Benefit, Home-Based Palliative Care, In-Home Support Services, Support for Caregivers of Enrollees, Additional Sessions of Smoking and Tobacco Cessation Counseling, Enhanced Disease Management, Telemonitoring Services, Remote Access Technologies, Home and Bathroom Safety Devices and Modifications, and Counseling Services are not covered.
Hearing services include coverage for hearing exams, routine hearing exams, fitting/evaluation for hearing aids, prescription hearing aids, and OTC hearing aids. Hearing exams have a $45 copay, and routine hearing exams have no copay for one exam every year. Fitting/evaluation for hearing aids have no copay. Prescription hearing aids (all types) have a copay between $499 and $799 for two hearing aids every year. OTC hearing aids are covered up to $100 every three months. Prescription hearing aids - inner ear, prescription hearing aids - outer ear, and prescription hearing aids - over the ear are not covered.
Vision services include eye exams with a copay between $0 and $45, and eyewear with no copay, and there is an annual maximum benefit of $75 for eye exams and $250 for eyewear. Contact lenses and eyeglasses (lenses and frames) are covered with no copay, while eyeglass lenses, eyeglass frames, and upgrades are not covered.
Dental services include a $45 copay for Medicare dental services, no copay for oral exams, dental x-rays, other diagnostic dental services, prophylaxis (cleaning), other preventive dental services, restorative services, adjunctive general services, endodontics, periodontics, prosthodontics, fixed, and oral and maxillofacial surgery. Fluoride treatment, prosthodontics, removable, maxillofacial prosthetics, implant services, and orthodontics are not covered. The plan has a maximum benefit of $1,000 per year.
Home Infusion bundled Services are covered, including Medicare Part B Insulin Drugs with a $35 copay and 0-20% coinsurance, and other Medicare Part B drugs with 0-20% coinsurance. Prior authorization is required.
Dialysis Services are covered under the Humana USAA Honor Giveback (PPO) plan and require prior authorization. The coinsurance for dialysis services is 20%.
Medical Equipment coverage includes Durable Medical Equipment (DME) with 18% coinsurance, Prosthetic Devices with 20% coinsurance, and Medical Supplies with 20% coinsurance. Diabetic Supplies have a 10% coinsurance and no copay, while Diabetic Therapeutic Shoes/Inserts have a $10 copay.
Diagnostic and Radiological Services are covered, including diagnostic procedures and tests with a copay ranging from $0 to $55, and lab services with no copay. Radiological services are covered, with a copay of at most $325 for diagnostic services, a coinsurance of at most 20% and a copay of at most $45 for therapeutic services, and a $10 copay for outpatient X-ray services.
Home Health Services are covered by the Humana USAA Honor Giveback (PPO) plan with no copay and no coinsurance, but additional hours of care and personal care services are not covered. Prior authorization is required for this benefit.
Cardiac Rehabilitation Services are not covered by the Humana USAA Honor Giveback (PPO) plan. Prior authorization is required for this benefit.
Skilled Nursing Facility (SNF) services are covered by the Humana USAA Honor Giveback (PPO) plan, with a copay of $10 for days 1-20 and $214 for days 21-100. Additional days beyond Medicare-covered SNF and non-Medicare-covered stays for SNF are not covered.
The Humana USAA Honor Giveback (PPO) plan covers acupuncture with a $45 copay, and covers over-the-counter items, including nicotine replacement therapy, up to $100 every three months. The plan also offers a meal benefit with no copay. The plan does not cover Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, and several other services.
