Most of these medical benefits feature no coinsurance, providing predictable costs for essential care. This plan also features dental, vision, and hearing benefits, including a $2,500 annual dental limit with no copay for most services, as well as no copay for routine eye and hearing exams. For specialized medical needs, durable medical equipment and dialysis services require a 20% coinsurance, while home health services are available with no copay. Please note that cardiac rehabilitation and transportation services are not covered under this plan.
