Specialist office visits require a $30 copay, and dental benefits feature a generous $2,000 annual maximum limit. Standard vision, hearing, and dental services also feature no deductibles and no coinsurance, keeping your out-of-pocket costs predictable. For unexpected medical needs, emergency room visits have a $90 copay, while acute inpatient hospital stays require a $295 daily copay for the first six days and no copay thereafter. Outpatient hospital services range from no copay up to a $295 copay, whereas home health care and lab services have no copay. Durable medical equipment and dialysis require a 20% coinsurance, and skilled nursing care is covered with no copay for the first 20 days.
Humana USAA Honor Giveback (PPO) offers partially covered inpatient hospital services with no coinsurance and prior authorization requirements, excluding upgrades, non-Medicare-covered stays, and additional psychiatric days.
