Drug Coverage
The Zing Select Diabetes & Heart MI (HMO C-SNP) plan has a $0 deductible for prescription drugs. In the initial coverage phase, you will pay different amounts depending on the drug tier and pharmacy. For example, in the initial coverage phase, you pay a $5 copay for preferred generic drugs at a standard pharmacy and no copay for preferred generic drugs through the mail. For specialty tier drugs, there is no copay. After your yearly out-of-pocket drug costs reach $2000, you pay nothing for covered drugs.

Hearing exams are covered with a $30 copay, and routine hearing exams have no copay for one visit per year, while fitting/evaluation for a hearing aid has no copay for one visit every three years. Prescription hearing aids are partially covered, with a maximum benefit of $750 per ear every three years, and prescription hearing aids (all types) have no copay for two visits every three years, but prescription hearing aids for the inner ear, outer ear, and over the ear are not covered. OTC hearing aids are not covered.
