Once your total out-of-pocket costs reach $0.00 for in-network covered services, the plan will pay 100% of in-network covered costs for the rest of the year.

The AARP Medicare Advantage from UHC ID-0007 (HMO-POS) plan has an enhanced alternative drug benefit. The plan has a $340 deductible. After the deductible, you will pay a copay or coinsurance depending on the drug tier and pharmacy. For example, you'll pay a $6 copay for preferred generic drugs at a standard pharmacy. Non-preferred drugs have a 29% coinsurance. Once your total drug costs reach $2000, you enter the catastrophic coverage phase, where you pay nothing for covered drugs.

Prior authorization is required for all services.
Partial Hospitalization is covered with a $55 copay. Prior authorization is required.
Ambulance and Transportation Services are covered by the AARP Medicare Advantage from UHC ID-0007 (HMO-POS) plan. Ground and air ambulance services have a copay of $275, with no coinsurance. Transportation services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered. Emergency Services have a $125 copay, and Urgently Needed Services have a copay between $0 and $55; all services have no coinsurance. Worldwide Emergency Coverage, Worldwide Urgent Coverage, and Worldwide Emergency Transportation have no copay.

Occupational therapy services have a copay between $0 and $25, while physician specialist services have a copay between $0 and $25. Kidney Disease Education Services, Glaucoma Screening, Diabetes Self-Management Training, Barium Enemas, Digital Rectal Exams, and EKG following Welcome Visit are also covered with no copay, while Health Education, In-Home Safety Assessment, Personal Emergency Response System, Medical Nutrition Therapy, Post discharge In-Home Medication Reconciliation, Re-admission Prevention, Wigs for Hair Loss Related to Chemotherapy, Weight Management Programs, Alternative Therapies, Therapeutic Massage, Adult Day Health Services, Nutritional/Dietary Benefit, Home-Based Palliative Care, In-Home Support Services, Support for Caregivers of Enrollees, Additional Sessions of Smoking and Tobacco Cessation Counseling, Enhanced Disease Management, Telemonitoring Services, and Counseling Services are not covered.
Hearing services include routine hearing exams with no copay, and prescription hearing aids with a copay between $199 and $1249, and over-the-counter hearing aids with a copay between $99 and $829. Fitting/evaluation for hearing aids, and prescription hearing aids - inner ear, outer ear, and over the ear are not covered.
Vision services include eye exams with no copay, and eyewear benefits with no copay for contact lenses and eyeglass frames, with a combined maximum benefit of $250 every two years; however, eyeglasses (lenses and frames) and upgrades are not covered. Routine eye exams are covered once per year.
Dental services are covered, with a 20% coinsurance for Medicare Dental Services. Oral exams, dental x-rays, other diagnostic dental services, prophylaxis (cleaning), fluoride treatments, other preventive dental services, and oral and maxillofacial surgery have no copay. Prosthodontics, removable, and prosthodontics, fixed, have a coinsurance between 0% and 50%. Restorative services, endodontics, and periodontics have no copay. Implant services and orthodontics are not covered.
Home Infusion bundled Services are covered, and require prior authorization. For Medicare Part B Insulin Drugs, there is a $35 copay and a coinsurance between 0% and 20%. For Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs, there is a coinsurance between 0% and 20%.
Dialysis Services are covered with a coinsurance of 20%. Prior authorization is required.
Medical Equipment, including Durable Medical Equipment (DME), Prosthetics/Medical Supplies, and Diabetic Equipment, is covered. DME has a 20% coinsurance, and Diabetic Supplies have no copay.
Diagnostic and Radiological Services are covered, including Diagnostic Procedures/Tests with a $50 copay, and Lab Services with no copay. Diagnostic Radiological Services have a copay of up to $225, and Therapeutic Radiological Services have a coinsurance of at least 20%. Outpatient X-Ray Services have a $25 copay.
Home Health Services are covered under the AARP Medicare Advantage from UHC ID-0007 (HMO-POS) plan with no copay and no coinsurance. However, Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are covered, but the plan does not cover Intensive Cardiac Rehabilitation Services, Pulmonary Rehabilitation Services, or SET for PAD Services. Prior authorization is required, and there is a copay for some services.
Skilled Nursing Facility (SNF) services are covered with prior authorization. For days 1-20, there is no copay, and for days 21-100, there is a $203 copay; additional and non-Medicare-covered SNF days are not covered.
Other Services include coverage for Over-the-Counter (OTC) Items and Meal Benefits, with no copay for OTC items, and a $0 copay for Meal Benefits. Acupuncture, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered.
