Monthly Premium

This plan does not come with a Part B Premium reduction. You must continue to pay your Part B premium.
Deductibles
This plan does not have a health deductible. Your insurance coverage on covered health services will start immediately.
This plan has a $420.00 drug deductible. You will need to pay this amount towards covered prescriptions before your insurance coverage for prescription medications kicks in.
Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $6200.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $6200.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

The Wellcare Simple Open (PPO) plan has a $420 deductible for prescription drugs. After meeting the deductible, you'll pay a copay or coinsurance for your medications, depending on the drug tier and the pharmacy you use. For example, you'll have no copay for preferred generic and specialty tier drugs at preferred pharmacies, and 25% coinsurance for standard generic drugs. The plan enters the catastrophic coverage phase once your total drug costs reach $2000, at which point you'll pay nothing for your Part D covered drugs.
The Wellcare Simple Open (PPO) plan offers a range of benefits with varying cost-sharing. Inpatient hospital stays have a copay, while outpatient services have copays that vary by service. Emergency services and primary care visits have copays, and preventive services are often covered with no copay. This plan also includes coverage for hearing and vision services, with copays for exams and eyewear. Dental services are covered with no copay for most services, with some exceptions. Outpatient Services are covered, including outpatient hospital services with a copay between $0 and $280, observation services with a copay between $140 and $280, and ambulatory surgical center services with a $225 copay. Outpatient substance abuse services, including individual and group sessions, have a copay of $25. Outpatient blood services have no copay.
Partial Hospitalization is covered under the Wellcare Simple Open (PPO) plan, with a $130 copay. Prior authorization is required.
Ambulance and Transportation Services are covered under the Wellcare Simple Open (PPO) plan. Ground and Air Ambulance Services have a $275 copay, with no coinsurance. Transportation Services to any health-related location are not covered.
Emergency Services, including Urgently Needed Services and Worldwide Emergency Coverage, are covered under the Wellcare Simple Open (PPO) plan. Emergency Services and Worldwide Emergency Coverage have a $140 copay, while Urgently Needed Services have a $40 copay; all have no coinsurance. Worldwide Emergency Transportation is not covered.
The Wellcare Simple Open (PPO) plan covers Primary Care Physician Services with no copay. Chiropractic Services, Occupational Therapy Services, Physician Specialist Services, Physical Therapy and Speech-Language Pathology Services, and Opioid Treatment Program Services all have a $15 copay. Mental Health Specialty Services have a $25 copay for individual and group sessions. Other Health Care Professional services have a copay between $0 and $15. Additional Telehealth Benefits have a copay between $0 and $40. Routine Chiropractic Care and Podiatry Services are not covered.
Preventive services, including an annual physical exam, are covered by the Wellcare Simple Open (PPO) plan with no copay. Other preventive services like Medicare-covered glaucoma screenings, diabetes self-management training, barium enemas, digital rectal exams, and EKGs following a Welcome Visit are covered with no copay. Kidney disease education services have a 20% coinsurance.
Hearing exams are covered with a $15 copay, and routine hearing exams and fitting/evaluation for hearing aids are covered with no copay. Prescription hearing aids are covered with a plan-specified amount of $750 per ear every year, while prescription hearing aids for the inner ear, outer ear, and over the ear are not covered, and OTC hearing aids are not covered.
The Wellcare Simple Open (PPO) plan covers vision services, including eye exams with a copay between $0 and $15, and eyewear with no copay. Eyewear has a combined maximum plan benefit of $300 per year.
Dental Services include coverage for Medicare Dental Services with a $15 copay, Oral Exams, Dental X-Rays, Other Diagnostic Dental Services, Prophylaxis (Cleaning), Fluoride Treatment, Other Preventive Dental Services, Restorative Services, Adjunctive General Services, Endodontics, Periodontics, Prosthodontics (removable and fixed), and Oral and Maxillofacial Surgery with no copay. Orthodontic Services are covered up to a maximum of $3,000 per year, and Maxillofacial Prosthetics, Implant Services, and Orthodontics are not covered.
Home Infusion bundled Services are covered by the Wellcare Simple Open (PPO) plan and require prior authorization. Medicare Part B Insulin Drugs have a $35 copay, while Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs have a coinsurance between 0% and 20%.
Dialysis Services are covered under the Wellcare Simple Open (PPO) plan with a coinsurance between 20% and 20%.
The Wellcare Simple Open (PPO) plan covers Durable Medical Equipment with 20% coinsurance, Prosthetic Devices with 20% coinsurance, and Medical Supplies with 20% coinsurance. Diabetic Supplies have no copay, and Diabetic Therapeutic Shoes/Inserts have 20% coinsurance.
Diagnostic and Radiological Services include coverage for all diagnostic services, with a copay between $0 and $40, and lab services with no copay. Diagnostic Radiological Services have a copay up to $280, while Therapeutic Radiological Services have a coinsurance of at least 20%, and Outpatient X-Ray Services have a $25 copay.
Home Health Services are covered by the Wellcare Simple Open (PPO) plan with no copay and no coinsurance. Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are covered by the Wellcare Simple Open (PPO) plan, but the plan does not cover any of the sub-services, including Medicare-covered Intensive Cardiac Rehabilitation Services, Medicare-covered Pulmonary Rehabilitation Services, Medicare-covered Supervised Exercise Therapy (SET) for Symptomatic Peripheral Artery Disease (PAD) Services, or Additional Cardiac Rehabilitation Services. There is a copay for these services, but the exact amount is not specified.
Skilled Nursing Facility (SNF) services are covered by the Wellcare Simple Open (PPO) plan, but require prior authorization. For days 1-20 and 51-100, there is no copay, but for days 21-50, the copay is $214.
The Wellcare Simple Open (PPO) plan covers Over-the-Counter (OTC) items with no copay, and a maximum benefit of $145 every three months. Acupuncture, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, Private Duty Nursing Services, Case Management (Long Term Care), Institution for Mental Disease Services for Individuals 65 or Older, Services in an Intermediate Care Facility for Individuals with Intellectual Disabilities, Case Management, Tobacco Cessation Counseling for Pregnant Women, Freestanding Birth Center Services, Respiratory Care Services, Family Planning Services, Nursing Home Services, Home and Community Based Services, Personal Care Services, and Self-Directed Personal Assistance Services are not covered. The plan also covers a meal benefit with no copay and a doctor referral is required.
