Monthly Premium
The Monthly Premium for this plan is $0.00. This is the amount you must pay every month. Additionally, this plan comes with a Part B Premium reduction of $130.00. You must continue to pay paying your reduced Part B Premium.
Deductibles
This plan does not have a health deductible. Your insurance coverage on covered health services will start immediately.
Drugs are not covered by this plan, so a prescription drug deductible is not applicable.
Out-of-Pocket Maximums
This plan has a combined Maximum Out-Of-Pocket cost of $14000.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $14000.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

Prescription drugs are not covered by Anthem Veteran (PPO).
The Anthem Veteran (PPO) plan offers comprehensive coverage, including inpatient hospital stays with a copay for the first three days, and outpatient services with varying copays. Primary care, specialist visits, and mental health services have copays, while preventive services like annual physicals and vision services, including eye exams and eyewear, have no copay. Dental services, including exams and x-rays, are covered up to an annual maximum, and home health services are covered with no copay. This plan also covers emergency services, ambulance services, and transportation services, each with a copay. Other benefits include home infusion, dialysis, and medical equipment coverage with varying cost-sharing structures. However, certain services like hearing aids, and some other services, are not covered.
Inpatient Hospital benefits, including Inpatient Hospital-Acute and Inpatient Hospital Psychiatric, are covered. For days 1-3, there is a $678 copay, and for days 4-90, there is no copay. Additional days for both Inpatient Hospital-Acute and Psychiatric are covered with no copay. Non-Medicare-covered stays and upgrades for Inpatient Hospital-Acute and Non-Medicare-covered stays for Inpatient Hospital Psychiatric are not covered.
Outpatient Services includes coverage for all outpatient hospital services, with copays ranging from $0 to $375. Observation Services have a copay of $375, while Ambulatory Surgical Center (ASC) Services and Outpatient Blood Services have no copay. Outpatient Substance Abuse Services have a copay of $50 for both individual and group sessions.
Partial Hospitalization is covered, but requires prior authorization. You will pay a $70 copay for this benefit.
Ambulance and Transportation Services are covered, including ground and air ambulance services, each with a $285 copay; transportation services to a plan-approved health-related location are covered with no copay, up to 12 one-way trips per year. Transportation services to any health-related location are not covered.
Emergency Services, Urgently Needed Services, and Worldwide Emergency Services are covered. Emergency Services and Worldwide Emergency Coverage, Worldwide Urgent Coverage, and Worldwide Emergency Transportation have a $110 copay. Urgently Needed Services has a $45 copay.
Under the Anthem Veteran (PPO) plan, primary care physician services have a $20 copay, chiropractic services have a $15 copay, occupational therapy services have a $35 copay, physician specialist services have a $50 copay, and physical therapy and speech-language pathology services have a $50 copay. The plan offers individual and group mental health and psychiatric sessions with a $45 copay, podiatry services with a copay between $0 and $50, other health care professional services with a $20 copay, and opioid treatment program services with a $50 copay. Additional telehealth benefits are covered with no copay.
The Anthem Veteran (PPO) plan covers preventive services, including an annual physical exam with no copay. The plan also covers additional preventive services such as Fitness Benefit, Remote Access Technologies, Glaucoma Screening, Diabetes Self-Management Training, Barium Enemas, Digital Rectal Exams, and EKG following Welcome Visit, all with no copay. However, it does not cover Health Education, In-Home Safety Assessment, Personal Emergency Response System (PERS), Medical Nutrition Therapy (MNT), Post discharge In-Home Medication Reconciliation, Re-admission Prevention, Wigs for Hair Loss Related to Chemotherapy, Weight Management Programs, Alternative Therapies, Therapeutic Massage, Adult Day Health Services, Nutritional/Dietary Benefit, Home-Based Palliative Care, In-Home Support Services, Support for Caregivers of Enrollees, Additional Sessions of Smoking and Tobacco Cessation Counseling, Enhanced Disease Management, Telemonitoring Services, Home and Bathroom Safety Devices and Modifications, and Counseling Services.
Hearing Services include hearing exams with a $50 copay, and routine hearing exams with no copay. Prescription hearing aids and OTC hearing aids are not covered, and the fitting/evaluation for hearing aids is not covered.
The Anthem Veteran (PPO) plan covers vision services, including eye exams with a copay of $0-$50, and eyewear with no copay. Contact lenses, eyeglasses (lenses and frames), and eyeglass lenses are covered with no copay, while eyeglass frames are covered with no copay. Upgrades are not covered.
The Anthem Veteran (PPO) plan covers a range of dental services, including oral exams, dental x-rays, and more, with no copay for these services. This plan has a maximum benefit of $2,250 per year for both in-network and out-of-network services.
Home Infusion bundled Services are covered, including Medicare Part B Insulin Drugs with a $35 copay. Medicare Part B Chemotherapy/Radiation Drugs and Other Medicare Part B Drugs are also covered, with a coinsurance between 0% and 20%.
Dialysis Services are covered under the Anthem Veteran (PPO) plan, with a coinsurance between 20% and 20%.
Medical equipment, including Durable Medical Equipment (DME), Prosthetics/Medical Supplies, and Diabetic Equipment, is covered. DME has a coinsurance between 0% and 20%, and requires authorization. Medical Supplies and Prosthetic Devices have a 20% coinsurance. Diabetic Supplies and Diabetic Therapeutic Shoes/Inserts have no copay.
Diagnostic and Radiological Services are covered under the Anthem Veteran (PPO) plan. Diagnostic Procedures/Tests have a copay between $0 and $140, while Lab Services have no copay. Diagnostic Radiological Services have a copay between $50 and $375, and Outpatient X-Ray Services have a $50 copay. Therapeutic Radiological Services have a coinsurance of at least 20%.
Home Health Services are covered by the Anthem Veteran (PPO) plan with no copay and no coinsurance. Additional Hours of Care and Personal Care Services are not covered.
Cardiac Rehabilitation Services are covered, but the plan does not cover Intensive Cardiac Rehabilitation Services, Medicare-covered Pulmonary Rehabilitation Services, Medicare-covered Supervised Exercise Therapy (SET) for Symptomatic Peripheral Artery Disease (PAD) Services, or Additional Cardiac Rehabilitation Services. Prior authorization is required, and copay information is available in the plan details.
Skilled Nursing Facility (SNF) services are covered by the Anthem Veteran (PPO) plan, but require prior authorization. For days 1-20, there is no copay, and for days 21-100, the copay is $214 per day; additional days beyond Medicare-covered and non-Medicare-covered stays are not covered.
Other Services are not covered, including acupuncture, over-the-counter items, meal benefits, Dual Eligible SNPs with Highly Integrated Services, Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) Services, private duty nursing services, case management (long term care), institution for mental disease services for individuals 65 or older, services in an intermediate care facility for individuals with intellectual disabilities, case management, tobacco cessation counseling for pregnant women, freestanding birth center services, respiratory care services, family planning services, nursing home services, home and community based services, personal care services, and self-directed personal assistance services. Referrals and authorizations are not required for these services.
