Plan Costs:

The cost of a Medicare Advantage Plan is made up of four main parts.

  • First, the monthly premium — the amount you pay every month.
  • Second, the deductible — the amount you pay out of pocket for covered services before the plan starts paying.
  • Third, the copayments and coinsurance — the amounts you pay out of pocket for covered services, usually after meeting the deductible (if applicable). Copays are fixed dollar amounts; coinsurance is a percentage of the cost.
  • Fourth, the Out-of-Pocket Maximum — the maximum amount you could have to pay out of pocket in a year. This may be different for in-network and out-of-network services.

Monthly Premium

The Monthly Premium for this plan is $38.00. This is the amount you must pay every month.

This plan does not come with a Part B Premium reduction. You must continue to pay your Part B premium.

Deductibles

This plan does not have a health deductible. Your insurance coverage on covered health services will start immediately.

This plan has no drug deductible. Your prescription medication coverage will start immediately.

Out-of-Pocket Maximums

This plan has a combined Maximum Out-Of-Pocket cost of $8950.00 (in-network or out-of-network combined). You will pay copays, coinsurance, and deductibles toward this amount. Once your total out-of-pocket costs reach $8950.00 for covered services, the plan will pay 100% of covered costs for the rest of the year.

Doctor Visits:

Regular visits to your primary care doctor are covered and will have a copay of $0.00 and coinsurance of 0% (no coinsurance).

Specialist Visits:

Visits to specialists are covered and will have a copay of $35.00 and coinsurance of 0% (no coinsurance). Specialist visits may require a referral from your primary care doctor or prior authorization.

Emergency Room:

Trips to the Emergency Room are covered, and will have a copay of $125.00 and coinsurance of 0% (no coinsurance). Coverage may vary for in-network and out-of-network hospitals.

Urgent Care:

Trips to Urgent Care arecovered and will have a copay of $35.00 and coinsurance of 0% (no coinsurance). Coverage may vary for in-network and out-of-network hospitals.

Drug Coverage IconDrug Coverage

The Anthem Medicare Advantage 2 (PPO) plan has an enhanced alternative drug benefit. There is no deductible for prescription drugs. In the initial coverage phase, you will pay a copay or coinsurance for your prescriptions. For example, you'll pay a $6 copay at a preferred pharmacy for preferred generic drugs, while standard mail order has no copay. Once your total drug costs reach $2000, you will enter the next coverage phase. After your yearly out-of-pocket drug costs reach $2000, you pay nothing for Medicare Part D covered drugs.

Inpatient hospital stays have a copay, while outpatient services have a mix of copays and no copays. Inpatient Hospital See details

Inpatient Hospital benefits, including Inpatient Hospital-Acute and Inpatient Hospital Psychiatric, are covered. For Inpatient Hospital-Acute, you pay a $345 copay for days 1-7, and no copay for days 8-90; for Inpatient Hospital Psychiatric, you pay a $345 copay for days 1-6, and no copay for days 7-90. Additional days for both Inpatient Hospital-Acute and Psychiatric are covered with no copay. Non-Medicare-covered stays and upgrades for Inpatient Hospital-Acute and Non-Medicare-covered stays for Inpatient Hospital Psychiatric are not covered.

Outpatient Services See details

Outpatient services are covered by the Anthem Medicare Advantage 2 (PPO) plan. Outpatient Hospital Services have a copay between $0 and $345, Observation Services have a $345 copay, and Ambulatory Surgical Center Services have no copay. Outpatient Substance Abuse services have a $35 copay for individual and group sessions, and Outpatient Blood Services have no copay.

Partial Hospitalization See details

Partial Hospitalization is covered by the Anthem Medicare Advantage 2 (PPO) plan, but requires prior authorization. You will have a $40 copay for this benefit.

Ambulance and Transportation Services See details

Routine chiropractic care is not covered.

The Anthem Medicare Advantage 2 (PPO) plan covers preventive services, including an annual physical exam with no copay. Additional preventive services may have a copay, and additional benefits like fitness and home safety devices are covered, with no copay for some services. Other preventive services, such as glaucoma screenings, diabetes self-management training, barium enemas, digital rectal exams, and EKG following a welcome visit, are covered with no copay. Some preventive services are not covered, including health education, in-home safety assessments, personal emergency response systems, and more.

Hearing Services See details

Hearing Services include hearing exams with a $35 copay, routine hearing exams with no copay, and fitting/evaluation for hearing aids with no copay. Prescription Hearing Aids are covered with a maximum benefit of $3000, while OTC Hearing Aids are covered with no copay and a maximum benefit of $300. Prescription Hearing Aids - Inner Ear, Prescription Hearing Aids - Outer Ear, and Prescription Hearing Aids - Over the Ear are not covered.

Vision Services See details

The Anthem Medicare Advantage 2 (PPO) plan covers vision services, including eye exams with a copay of $0-$35, and eyewear with no copay. Contact lenses, eyeglass lenses and frames, and upgrades are also covered with no copay.
